WHAT is the hants health and wellness TEAM? The Hants Health and Wellness Team is a health care team that promotes emotional and physical wellness and self management of chronic conditions. We do this by helping individuals to gain knowledge, skills, and confidence to take charge of their own health. Our interdisciplinary clinical team includes: Dietitians, Emotional Wellness Counsellor/Navigator, Internist, Nurse Practitioner, and Wellness Facilitator (Physiotherapist) as well as Administrative Assistant, Team Lead and a Health Service Manager to support day to day operations of the clinic. our services Anyone can self-refer to our Health and Wellness Programs, Wellness Navigation and Emotional Wellness Counselling services. To better serve you, a referral form is preferred for the Chronic Disease Management services but not required. For Complex Care, we ask that you have the support of your primary care provider (family physician or nurse practitioner) and that they complete a referral form. Health and wellness programs We provide free health and wellness programs on healthy eating, physical activity, emotional wellness and risk factor management. We work with a number of community partners to offer our programs at a variety of locations in the community, in addition to our site at the Hants Community Hospital. A brief description of our programs is provided in this flyer. Not sure where to start or want more information about a program? Give us a call and we are happy to provide more details. Targeted Programming Available! In addition to the programs listed in this program flyer, our team will also consider offering our programs in the community by request (Minimum of 10 people required per program request). WELLNESS NAVIGATION & EMOTIONAL WELLNESS COUNSELLING Wellness Navigation: Our team of health professionals know health care, the community and government systems. We work with individuals and families to help link you to existing resources to support your health and wellness in the community. If you are struggling with your health, money, housing or are just wanting to get more connected in your community, give us a call! Emotional Wellness Counselling: In addition to providing wellness navigation services, we also provide individual and group counselling for emotional wellness. If you need a boost to cope with the stressors in your life, we can help you learn skills and strategies to better process your feelings, feel more in control of your emotions and behaviours to improve your quality of life. CHRONIC DISEASE MANAGEMENT We provide individual and group programs for chronic disease management for people with diabetes, high blood pressure, high cholesterol, and/or those who need nutrition education. Our team will help you learn more about your chronic condition as well as skills and strategies to take charge of your health. This can range from lifestyle changes to recommendations on medications based on best practice guidelines to meet your personal needs. COMPLEX CARE We also provide individual appointments to support complex care for citizens who have three or more specific chronic conditions. In partnership with patients, we develop individualized care plans with a focus on improving functional health and health related quality of life. Patients seen in complex care meet with an internist and/or nurse practitioner, as well as other team members as needed to support care planning. Prediabetes, when blood sugars are outside the normal range, offers a warning and gives us a chance to change the future. Not everyone with pre-diabetes will develop diabetes, but the risk increases if steps are not taken to manage it. In this session you will learn about lifestyle changes to manage the risk factors that contribute to developing diabetes. Physical Activity Introduction to Chair Yoga Our Wellness Facilitator (Registered Physiotherapist) who has recently obtained a 200hr yoga certification is inviting you to participate in a four week wellness series about the health benefits of Chair Yoga. You will practice modified poses in sitting and standing as well as breathing exercises to quiet your mind and body. No yoga experience necessary, however, a physical activity screen must be completed prior to registration. Please call no later than five business days before the program start date to complete this form. You will learn about the components of a physical activity program and practice skills in stretching, strengthening, and aerobic exercise. Session 1: Beware your Chair! - Join us for this ground-breaking session as we uncover the impact of sitting on your health and discuss strategies to build more light physical activity into your day (no physical activity screen required). Parenting Incredible Years 10-Week Parenting Program The Incredible Years is a 10-week program that will address a variety of topics to strengthen all of the great parenting skills you already have and give you the opportunity for support from other parents. It is for parents of children who are 5-12 years old. Registration is required. Dates and location to be determined based on interest. In this two week program, we will discuss anxiety often seen in children up to 12 years old. You will learn how to identify early signs of anxious feelings in your child and skills to help your child manage everyday anxiety. You will have the opportunity to practice these new skills at home and come back to discuss with the group. Information on additional health and community resources will also be available. ADHD Information Session - West Hants Community Mental Health For adults supporting children with ADHD, have a diagnosis themselves, or those who would like to learn more about ADHD. Participants will come away with a better understanding of ADHD as well as information about programs and resources to help better manage ADHD. Health Benefits of Tai Chi and Qigong Elements of Health Come out and learn about how you can manage your chronic health issues in a more alternative way through the practice of Tai Chi and Qigong.
