Quality and Safety Responsibility and Integrity Values Summary Comments: After reading all of the descriptions for these areas under each domain, I have determined that I would be considered a Top Performer because many people do come to me for help, I was chosen and completed the Preceptor Academy this year to assist with effectively precepting and training new staff members. I do have a high potential for advancement and I think of the "big picture" consistently Section 2 - Goals Patient Satisfaction/Responsiveness of staff Departmental Goal (05/08/2014-07/01/2015): Throughout the next review period, you will respond to call bells and participate in the No Pass Zone initiative with consistency. This will be measured by observation and callbell response time audits, and by consistent improvement in the unit s HCHAPs scores within the Response of Hospital Staff Domain. Patient-Centered Communication Departmental Goal (05/08/2014-07/01/2015): Throughout the next review period, you will be responsible to update and fully utilize the white communication boards with consistency. This will be measured by observation and bedside shift report audit tools, and by consistent improvement in the unit s HCHAPs scores within the Communication with Nurses Domain. Departmental Goal (05/08/2014-07/01/2015): Throughout the next review period, you will consistently participate in bedside shift report that includes interaction with patients. This will be measured by observation and bedside shift report audit tools, and by consistent improvement in the unit s HCHAPs scores within the Communication with Nurses Domain. Goals Summary Comments: I gave myself superior performer in these goals because they are all goals and everyone has room for improvement in these areas of making the floor run smoother. These goals also depend on the participation on my coworkers and though I attempt to be consistent with these initiatives, I also must rely on their participation. I keep trying though! Responsibilities Summary Comments: Professional development = BSN Professional Practice = PPC, multiple Above & Beyond Nominations, ACES award recipient Structural Empowerment = persistent patient teaching and completion of the Preceptor academy, I also reach out for management input in situations that I found difficult to handle as to keep an open dialogue and grow professionally through those difficulties Quality & Innovation = I consistently attempt to determine a problem then produce a solution. I instituted the front hall calendar as an initiative to assist with the dissatisfaction of the front hall assignment in an attempt to show evidence that the front hall assignment is being distributed among staff members. I also willingly participate in all patient care improvement initiatives Healthy workforce = I have personally reduced the amount of after shift overtime that I have obtained in previous years, I have good relationships with my coworkers inside and outside of the work environment and I am one of the staff members that people are usually happy to be working with. 4
Section 4 - Overall Summary Summary Comments: As a professional as well as a person, there is always room for improvement. So as a person, there are things that I need to work on that will benefit me as a professional. Communication skills in frustrating situations is one that I will continue to work, I will obtain my med surg certification this year to pursue my professional development. I will continue to work on my time management skills and further reduce my after shift overtime. I will also continue to lead the professional experience board for staff updates as they are available. 5
