Caldwell Medical Center Departments Surgery Medical / Surgery Same Day Surgery Lab Education Administration Special Care Unit Women s Center Admission Emergency Services Radiology Cardiac Rehab Admission and Registration The Admission and Registration Department is located just off of the lobby near the main entrance to the hospital. After hours these services are available through the Emergency Department registration. During the admission/registration process you will be asked for personal and insurance information. You will be asked to sign a General Condition of Admission, Consent Form, Assignment of Benefits and a Financial Agreement. If you are being admitted, an ID bracelet will be placed on your wrist. This ID bracelet is for your safety, so we ask that you not remove it. Federal regulations require hospital personnel to ask you about an Advance Directive. An Advance Directive includes the presence of a living will or durable power of attorney. Information on Advance Directives is offered to every patient upon admission. In addition, you will be given a patient privacy notice that will explain your rights under the Health Insurance Portability and Accountability Act (HIPAA). Outpatient Registration is also located just off the main entrance of the hospital. The clerk at the information desk will direct you to one of the registration clerks and then will notify the specific department that you are here for your procedure. The procedure you are having may require a pre-certification of prior authorization from your insurance carrier. This pre-certification is generally obtained by the referring physician s office. If this pre-certification or prior authorization is not obtained prior to starting the procedure your insurance carrier may not pay for the service. The scheduling of procedures is coordinated with the physician s office and our Central Scheduler.

Cardiopulmonary/Cardiac Rehab/Sleep Lab The Cardiopulmonary Department combines care for your lungs and heart as well as post cardiac services in Cardiac Rehab. The Department provides diagnostic and therapeutic procedures to patients of all ages both inpatient and outpatient. Some of the services we provide are: Oxygen Therapy Breathing Treatments Pulmonary Function Tests Bi-Pap Electrocardiogram (EKGs) Holter Monitoring Stress Tests Arterial Blood Gases Cardiac Rehab is an important part of the recovery process for individuals with heart disease or those having recently experienced cardiac surgery. Our medically supervised program which is monitored by a registered nurse combines exercise and education for patients. To enroll in the program patients must have a physician order. Sleep Lab services are offered to diagnose/treat a variety of sleep problems, including sleep apnea and other medically related disorders. Millions of Americans have clinically significant sleep difficulties which when diagnosed properly, can be successfully treated. In the Sleep Lab patients are monitored and observed via cameras while they spend the night sleeping. By monitoring breathing, heart and brain wave activity during sleep, our team can help determine the cause of problems and prescribe the best plan of treatment. Sleep studies are electronically transmitted and interpreted by physicians practicing at Vanderbilt Medical Center. In addition to the above services we also perform Electroencephalogram (EEG) studies. An EEG is a continuous recording of the electrical activity of the brain. The procedure involves taking a brief history, measuring the patient s head, applying leads to the scalp and performing the procedure. It takes approximately 2 hours to complete. The procedure is painless and there are no after effects. Emergency Department The Emergency Department is open and available to see patients 24 hours a day, 7 days a week. Emergency physicians, registered nurses and licensed practical nursing staff our department at all times. We are equipped to care for all types of illness and injuries from minor sprains, lacerations, sore throats to heart attacks and motor vehicle accidents. Our staff is trained in advanced courses such as Advanced Cardiac Life Support (ACLS). Pediatric Advanced Life Support (PALS), Advanced Trauma Life Support (ATLS), Certified Emergency Nurse (CEN), and Trauma Nurse Core Curriculum (TNCC). We are currently in the process of seeking accreditation as a Chest Pain Center and are on target to complete this process by the end of 2012.

Laboratory Fully accredited through the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and CLIA, our laboratory services are available 24 hours a day, 7 days a week. Our most recent Joint Commission survey was in the fall of 2011. Our laboratory offers a wide range of tests on-site. For those tests not performed in our lab we have arrangements with a reference lab. These specimens for the reference lab are picked up daily by a courier and taken to the reference lab. To obtain lab services patients must have a signed physician order that includes the test to be performed and a diagnosis or reason for the testing. Results will be called or faxed to your physician. Critical results are immediately called to your physician. STAT services are also available if your physician needs results immediately. Home Health Caldwell County Hospital Home Health is a Joint Commission Accredited and Medicare Certified agency. Our goal is to provide quality, skilled care to patients in their homes. We currently serve patients in Caldwell, Lyon and Trigg counties. Services: Skilled nursing services are provided by Registered Nurses and Certified Nursing Assistants. Physical Therapy services are provided by Physical Therapists and Physical Therapy Assistants. Occupational Therapy services are provided by Occupational Therapists and Occupational Therapy Assistants. Admission Criteria: The patient must be homebound, meaning the client is unable to leave his/her home without considerable effort. The patient must require some type of skilled care from a registered nurse, physical therapist or occupational therapist. The patient must be under the care of a physician. The patient and family/caregivers must consent to receive the Home Health services and be able to assist with the plan of care. The home environment must be safe and appropriate for the plan of care. Medical/Surgical Nursing Unit The Medical Surgical Unit at Caldwell Medical Center is a 21 bed unit consisting of all private rooms. Each room also has its own bathroom complete with a shower. We provide care for general medical and surgical patients of all ages through a multidisciplinary team approach while still developing an individual plan of care for each patient s needs. Swing Bed Service Sometimes a patient has improved to the point of no longer needing acute hospital care, but is not quite ready to return home. Swing Beds are designed for patients who have been hospitalized, for a minimum of 3 days here or at another facility, but need further care before they are ready to go home or to another care setting. The term swing means that the hospital may clarify beds for acute care or skilled care. Swing Bed patients are here to receive such services as physical or occupational therapies, long term IV antibiotics, specialized wound care and other skilled services. Patients also receive therapeutic recreation while in the Swing Bed program. Radiology The Radiology Department at Caldwell Medical Center Provides quality services using state of the art technology. We provide procedures in the following modalities: Routine X-Ray Fluoroscope Digital Mammography Ultrasound CT Scan Nuclear Medicine DEXA (Bone Density) MRI Digital Mammography is provided in our Women s Center, within the department, by registered Mammography Technologists and meets the guidelines established by the Mammography Quality Standards Act. Our department is filmless and the PACS System allows copies of all imaging to be burned to a CD for referrals or consultations. If your physician is requesting a copy of procedure results we can transmit the results to them via a protected web link. The Radiology Department is accredited with the American College of Radiology and Certified by the FDA. Dr. Larry Long, our Radiologist, is Board Certified and is a fellow in the American College of Radiology.

Special Care Unit (SCU) The Special Care Unit (SCU) at Caldwell Medical Center is a four bed unit providing specialized care for patients who are critically ill or who require intensive monitoring around the clock. Having a loved one in SCU can be an overwhelming experience. The environment can be intimidating with equipment and alarms all around. Recognizing this, we encourage you to ask questions and our staff members will help you understand what is going on with your loved one. Because this is a specialized unit we limit visitors to allow our staff the opportunity to provide optimum care to your loved one. Our unit is access controlled so visitors must stop at the Medical/Surgical nursing station and a staff member will assist you. Sports Medicine Rehab and Fitness Caldwell Sports Medicine is a cooperative service of Caldwell Medical Center and Trover Foundation. The facility is located at 605 South Jefferson Street in Princeton. At Caldwell Sports Medicine we utilize a team approach to speed of recovery. As a vital member of our team, we ask you to take an active role in your recovery. We offer a broad array of services including sports medicine, physical therapy, industrial rehab and aquatic rehab. We also offer the service of a Certified Athletic Trainer free of charge to the schools located in our service area. Fitness Formula offers comprehensive wellness capabilities including overall fitness programs, general exercise and consultations with fitness trainers for persons age 12 and older. Equipment available in the Fitness Center includes therapeutic lap pool, water aerobics, treadmills, stair steppers, stationary bicycles, Nautilus machines and free weights. Showers, dressing rooms and lockers are available for your convenience. The Fitness Formula staff work with a diverse range of clients, utilizing an open design and state of the art technology and exercise equipment to meet your fitness needs. Special membership rates are available for Senior Citizens, Corporate Group Memberships of 10 or more and post Rehabilitation and Cardiac Rehabilitation patients that sign up within 60 days of discharge from their rehab. For membership rates and hours of operation call (270) 365-1585 Surgical Services The Surgical Department of Caldwell Medical Center operates Monday Through Friday for scheduled cases. An on-call team of staff members are available after hours for emergency cases. We provide a variety of surgical specialties including: General and Laparoscopic surgery, Ophthalmology, Urological and ENT. Outpatient or Same Day Surgery is available Monday Through Friday. We provide pre-operative instructions through recovery room care. The Gastroenterology Lab enables our physicians and staff to examine a large portion of the gastrointestinal tract. Our Endoscopic equipment allows the physician to obtain biopsy specimens and diagnose and treat many illnesses. Some of the Endoscopic procedures that we perform are: Colonoscopy Esophagogastroduodenoscopy Sigmoidoscopy
