If a resident is off HH services completely and readmits to HH, the fee will be charged again. Nurse Two-Week Assessment: Any resident who is on HH nursing services (med reminders, med setup, wound care, etc.) receives a 2 week assessment by a nurse to determine if services are still needed or if additional services are required. Clinic Visit: Any visit to the clinic, exluding weekly free blood pressure clinic. Professional Nurse Check: When a professional nurse sees a resident. Med Visit by Home Health Aide: Any visit by a HH Aide that requires assistance. Monthly Scheduled Home Health Aide Services: Any services that require daily assistance other than a well-being visit. (Unna boots, TED hose, etc.) Daily Well-being Visits: a nursing staff member stops by resident room daily for a quick check, no assistance given to the resident. Monitor Blood Pressure/Communicate with Doctor: Weekly monitoring of blood pressure that requires doctor notification. Emergencies: Falls or illnesses that require a trip to the hospital.

Health Center Services Our skilled nursing facility at McCrite Plaza offers a full spectrum of care ranging from Sub Acute Rehabilitation to Long Term Care. Each level of care is designed to meet the specific needs of the resident as determined by personal physicians and nursing staff.
