To fill some of these positions, Comunilife is hosting a job fair for 6 Clinical Supervisors in our Mental Health and HIV/AIDS Programs. WHAT COMUNILIFE IS SEEKING: Comunilife is seeking a total of 6 Clinical Supervisors as follows: Three (3) Clinical Supervisors for mental health scattered site programs located in Brooklyn, Queens, and the Bronx; and Two (2) Clinical Supervisors for HIV/AIDS congregate programs located in Brooklyn and in the South Bronx.; and One (1) Clinical Supervisor for a HIV/AIDS scattered site program located in Brooklyn. QUALIFICATIONS: Clinical Supervisor positions in HIV/AIDS congregate (2) and scattered site (1) programs require a license (LMSW or License in Mental Health Counseling). Clinical Supervisor positions in mental health scattered site programs (3) prefer but do not require a license. Graduate degrees in social work or related fields are required. All positions require at least three years of supervisory experience, preferably in the program type in which the vacancy occurs (mental health or HIV/AIDS) supervising Case Managers in delivering services to formerly homeless persons with co-occurring substance abuse disorders. Bilingual skills are a plus. Comunilife, Inc. Social Worker Full Time Comunilife, Inc., is a multiservice, not for profit agency that provides a continuum of mental health, social services and housing to persons living with HIV/AIDS and/or mental illness, and substance abuse. Comunilife is opening a new project, the Safer Haven Project, which will provide transitional residential services to formerly homeless persons who are seriously mentally ill with co-occurring substance abuse disorders and chronic health conditions. Comunilife is seeking to fill this full time position. This position and the qualifications are highlighted below. In addition to the individual qualifications, the applicant hired for this Project must be able participate in program evaluation activities and deliver (or learn to deliver) services using evidence-based practices, including Individualized Dual Diagnosis Treatment (IDDT); the Multicultural Relational Approach ; Wellness Self- Management (WSM); Motivational Interviewing (MI); and Individualized Wellness Plans (IWP). The project is located in two Bronx sites and the staff will travel between both locations via transportation provided by Comunilife. RESPONSIBILITIES: As a member of the Comprehensive Case Management (CCM) Team, the Social Worker will conduct psychiatric and other assessments and develop and implement person-centered treatment plans, and supportive services for formerly homeless residents in the Safer Haven program who are seriously mentally ill adults with co-occurring substance abuse disorders and/chronic health conditions. Services shall include individual and group counseling; assisting clients in achieving and maintaining functional improvements in self-care, employment, personal relationships, and housing stability; and providing discharge planning and services. QUALIFICATIONS: Must have an MSW and at least three years prior experience working with persons who are formerly homeless, diagnosed with mental illness, living with HIV/AIDS and chronic health conditions. At least two years prior supervisory experience is preferred, and must be able to participate in program evaluation activities and deliver (or learn to deliver) services using evidence-based practices, including Individualized Dual Diagnosis Treatment (IDDT); the Multicultural Relational Approach ; Wellness Self-Management (WSM); Motivational Interviewing (MI); and Individualized Wellness Plans (IWP). Comunilife, Inc. Nurse Practitioner Comunilife offers an exciting opportunity for nurse practitioners interested in providing holistic care and individual counseling for patients of all ages with chronic medical and mental health needs. This is a new clinical arrangement made possible by our being awarded a grant for the Mi Salud program and by our clinic s vision for integrating physical wellness, lifestyle modification, and mental health for all of our patients. The Vida Guidance Center, located 4419 Third Avenue in the Bronx (VIDA) is a multi disciplinary center staffed by clinicians with expertise in many treatment modalities including cognitive behavioral therapy, dynamic therapy, social work, psychopharmacology, group and family therapy, art therapy, and dance and movement therapy. There is an academic affiliation with NYU Medical Center, including a rotation for New York University medical students with a focus on diversity training and community medicine. The nurse practitioner will work collaboratively with our staff physician, Dr. Sharon Sageman who is board certified in Geriatric Psychiatry, General Psychiatry, Holistic Medicine, Addiction Psychiatry and Advanced Psychopharmacology. Collaborating with patients mental health care providers at VIDA, their primary care doctors, and specialists in the community, families, and support systems. Conducting monthly follow-up appointments with patients to track their medical status, treatment adherence, behavioral health, quality of life issues, and to help patients overcome obstacles to optimal care.
