Not only is this harm frequent, expensive and often serious, much of it is also preventable 1. image area Evidence to Act Now Research studies and post implementation results from hospitals with electronic medication management (emm) systems; all articulate the benefits and improvements achieved when moving from paper to electronic systems to manage medications. emm is now considered an imperative. DXC Technology has partnered with healthcare providers since early 2000 to define, plan, configure and implement MedChart electronic medication management into clinical workflows for hospitals, clinics and aged care facilities across Australia, New Zealand and The United Kingdom. MedChart, Proven Results MedChart brings together a range of workflows from admission to discharge, in one system for optimal medication management processes, reducing errors, improving efficiency and enabling greater communication and coordination within the clinical teams for better patient care. In a published study lead by Professor Johanna Westbrook, prescribing error was reduced by 57.5 percent in wards using MedChart. Serious medication errors were reduced by 44 percent. St Vincent s Public Hospital Sydney was Hospital B in the paper 2. St Vincent s completed implementation of MedChart in 2010 across all 340 beds including high dependency and intensive care. St Vincent s achieved an Outstanding Achievement (OA) for their ACHS Accreditation, EQuIP5 1.5.1, in April 2012. 5 Waitemata DHB has MedChart deployed across approximately 970 beds on three campuses with the remaining 130 adult beds scheduled to be deployed by the end of 2017. Over 1000 active prescribers can access MedChart from any device that has access to the DHB s network, and 2,500 nurses use ipad mini devices to administer more than 300,000 doses of medication per month. Feedback has been that MedChart reduces administration errors, and reduces the time spent trying to locate paper medication records. MedChart has certainly had a positive impact on medication management at Waitemata DHB. Analysis of responses to CDS prompts (override, remove, modify etc) logged in MedChart, enables assessment and amendment of CDS to ensure current and effective rules across the facility. 7 emm of Choice, MedChart The selection and successful implementation across diverse care settings from rural to major tertiary teaching hospitals demonstrates the robust, scalable and clinical credibility of MedChart. epma has revolutionised antibiotic stewardship. Being able to identify all patients on broad spectrum antibiotics, as well as why they were prescribed it, has enabled targeted ward rounds. MedChart is used throughout Bethlehem Specialist Palliative Care Service (VIC). A number of implementations are planned for the near future. ACT Health selected DXC for territory-wide emm, June 2014 Two rehab wards are now live at Canberra Hospital. MedChart will be implemented across the 3 major hospitals in ACT Canberra Hospital, Calvary Hospital and the soon to be built University of Canberra Hospital. Northern Territory uses MedChart to enable safer and consistent medicines management MedChart is implemented in the five public hospitals and 50 rural and metropolitan Indigenous Healthcare Centres, advancing statewide healthcare delivery in Northern Territory, supporting the specific requirements of use in acute and primary care settings. Cabrini Health goes mobile with DXC s MedChart, July 2014 DXC and Cabrini Health developed the mobile version of MedChart for doctors to prescribe and review medications through ipads nurse access on Motion F5 tablet PCs. As at May 2017, MedChart is live across all five Cabrini Health Hospitals. Echuca Regional Health integrates MedChart with Automated Dispensing Machines Echuca has introduced automated dispensing cabinets, interfaced with MedChart to create a closed loop from prescribing to drug selection for each medication required for each patient. Hunter New England Health goes live with MedChart, November 2015 Hunter New England has seen positive staff engagement, a significant reduction in costs and improvements in quality of care since implementation. MedChart is currently being rolled out to its largest hospital, John Hunter.
