Background Visible, active clinical engagement is central to the success of our work and will support the safe delivery of clinically effective care to patients. Internationally, the foremost proponent of improving patient safety in primary care is the Scottish Patient Safety Programme in Primary Care, run by NHS Scotland since 2011, with promising results. In Nov 2013, Dr Neil Houston, National Clinical Lead for the Scottish program, presented at the APAC forum and subsequently ran a workshop for Counties Manukau Health to explore the idea of designing and implementing a similar program in the Counties Manukau district. This culminated in the establishment of the Safety in Practice initiative in 2014 which is supported by the four Northern region DHBs, 8 Northern region PHOs, Ko Awatea, the Health, Quality & Safety Commission and the Royal New Zealand College of General Practitioners. This initiative is now moving ahead to incorporate community pharmacy, which is already involved in this programme in Scotland. Initially this will include testing work with 20 community pharmacies from Auckland and Waitemata DHB areas, replicating the work done in general practice. This testing and developing phase will continue for 1 year. The Safety in Practice initiative will seek to improve the delivery of care throughout the patient s journey by providing a targeted, evidence based range of tools, techniques and learning that will improve patient safety. Our ambition is to deliver a programme, which reduces the number of events, which could cause avoidable harm to people from healthcare delivered in any primary care setting. Purpose of the role : Provide Clinical Lead Services for the community pharmacy Safety in Practice programme. This will include provision of subject expertise, clinical advice and visible clinical leadership to inform the implementation of the Safety in Practice initiative in community pharmacy To promote and build capability and capacity in quality improvement and health innovation with regard to the community pharmacy Safety in Practice programme. A one year, fixed term, care redesign, quality improvement and health innovation experience with the opportunity to apply specific graduate study principles in the practical work setting of a healthcare delivery system To develop skills and expertise in care redesign and quality improvement and their application with regard to the community pharmacy Safety in Practice programme to improve patient outcomes and enhance patient and whanau experience To build personal and organisational leadership capability and capacity Waitemata District Health Board -JOB DESCRIPTION - Page 3

To support organizational initiatives in health informatics by bringing the perspective of your current and previous roles/units, past experience and qualifications, and enthusiasm for service improvement It is expected that the Clinical Lead will visit community pharmacies that are involved in the testing phase and respond to any queries about the programme. The may also be required to meet with local stakeholders and provide updates about the programme including feedback from the community pharmacies who are involved. They will be required to attend regular project meetings and pharmacy expert group meetings and act as lead faculty at learning sessions and other events relating to Safety in Practice for community pharmacy. This role will require 0.2FTE, or 1 day per week. Time will be flexible, depending on the scheduling of meetings and learning sessions. This role sits under Waitemata DHB s Institute for Innovation and Improvement Fellows Programme. Fellowships incorporate a project-based attachment in a specific area such as care redesign and quality improvement (up to 0.9 F.T.E) and also include a structured leadership development programme (0.1FTE). There is also scope for the role to be adjusted to accommodate clinical duties, if preferred/appropriate. The Fellows Programme seeks to embed quality improvement, innovation, translational research, education and leadership development in a new model for training health professionals. The programme is sponsored by the CEO and developed and led by the Senior Advisor in Health Leadership.
