To contribute to the improvement of health, education and wellbeing outcomes for children and young people referred for a Gateway Assessment by Oranga Tamariki by: Coordinating appropriate health assessments by an appropriate skilled clinical assessor Coordinating any follow up required Ensuring that an Interagency Child Development Agreement is completed. My role responsibility This role is primarily accountable for: Ensuring that A set of performance objectives will be agreed annually between your manager and yourself reflecting applicable strategic and operational ambitions. Relationship management Tasks (How you achieve) Develop and maintain a productive relationship with Oranga Tamariki in the catchment area and act as the first point of contact with Oranga Tamariki staff in relation to Gateway assessments. Promotion of service to Tamariki social workers regarding the programme. Attend Oranga Tamariki intervention options meeting and/or appropriate meetings. Liaise with providers of other health and social services as a Liaise with families/whanau to encourage and facilitate their meaningful engagement in the assessment process including completion of pre-assessment questionnaires (e.g. SDQ) and the attendance at the assessment. Develop and maintain effective relations with: Families/whanau of children or young people referred for assessment Oranga Tamariki management and social workers School principals and teaching staff Clinical Assessors Key health information sources Coordinators at other Performance Indicators (How you will be measured) Relationships Maintained. Physical Attributes We will make all reasonable efforts to provide a safe and healthy work place for all, including persons with disability. Hearing and speech sufficient to communicate with clients and co-workers enabling direct and telephone, communication and hear emergency alarm. Manual dexterity sufficient to file and retrieve records, operate a computer and use items such as a pen and scissors. Ability to stand, walk, sit, stretch, twist, bend, climb, frequently life/move weights up to 15 kilograms and infrequently lift/move weights over 15 kilograms. Ability to move about the Whanganui Hospital and throughout WDHB campus to undertake the necessary duties of the role. Ability to undertake repetitive activity throughout the day including filing and retrieval of records at low and high levels as well as some computer data entry. Sufficient visual ability to read accurately, write/record, use a computer, enabling accurate performance of essential job duties.
