Skill(s): Proficient reading, writing, grammar, and mathematics skills; excellent interpersonal relations and communicative skills; thorough knowledge of current long-term care nursing practices in accordance with the State Department of Health long-term care licensure regulations and federal health care financing administration; knowledge of drug interactions, side effects, and incompatibilities; IV proficiency and knowledge of current clinical practice guidelines for purposes of educating nursing staff as needed; demonstrated decision-making skill; knowledge of hazardous materials, bloodborne pathogens and infection control procedures; moderate computer skills; proficient organizational skills; proficient assessment skills; demonstrated management and supervisory skills; current C.P.R. certification (or eligibility and pursuit of certification); satisfactory completion of pre-employment physical (post-offer); ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills. Experience: A minimum of two (2) year s of supervisory experience in a long- term care geriatric setting and/or acute care. General Responsibilities Responsible for assisting the Director of Nursing in the overall functioning and clinical development of the Nursing Department. Essential Duties 1. 3. Assists the Director of Nursing in the overall functioning of the Nursing Department as illustrated by the following: a. Has 24-hr a day accountability to manage the direction, organization, and supervision of the assigned nursing unit(s) to assure the deliver of care is within Diakon policies, regulatory and nursing standards. (Applicable in absence of the Unit Manager (NCC) position.) b. Determines work procedures, prepares work assignments, and expedites workflow. c. Supports and assists with direction, organization and supervision of Nursing Department staff in collaboration with Director of Nursing. d. In collaboration with Director of Nursing develops and implements written staffing plan and nursing schedules that reflect the needs of the resident and patient population; ensures that all employees attendance is documented and that these records are maintained. e. Accepts responsibilities of Director of Nursing in his/her absence, including oversight of Director of Nursing s direct reports, e.g., one-to-one meetings with staff, completing needed documents (COR, QI Summary, QI justification & QAPI). f. Monitors factors to achieve compliance with Diakon and regulatory standards for the assigned unit(s). g. In collaboration with Director of Nursing, monitors and utilizes CMS indicators and Diakon clinical outcome report to manage performance improvement and care delivery systems on the assigned unit(s). h. Responsible for management and oversight of electronic health record documentation system according to Diakon policies and procedures, including but not limited to monitoring accuracy of nursing documentation and providing education/direction as needed. i. Communicates and interacts with pharmacy consultant as needed, with regard to medications or to check pharmacy errors. j. Makes rounds on units to check on residents, patients, and clients; monitors guarded and critically ill residents, patients, and clients. k. Understands and monitors ordering/purchasing/receiving items through the agency s protocol and system. l. Ensures residents are accompanied by a volunteer, family member or an employee to medical appointments. m. Oversees pre-employment follow through with candidates for hire as well as the orientation process and provides individualization where and when needed. n. Interviews and hires professional and non-professional nursing applicants; collaborates with Director of Nursing on selected candidates for hire. o. Directs, organizes and supervises nursing staff on the unit; disciplines staff when appropriate. p. Guides, educates and mentors members of nursing team and staff to develop skill sets to provide quality patient care, including leadership development. q. Develops employee performance evaluations; presents performance appraisals in conjunction with Director of Nursing. Infection Control a. Responsible for community s infection prevention program as well as oversight of those involved to meet the benchmark in infection percentage, as well as Act 52 compliance. b. Implements and updates components of the TB Exposure Control Plan. c. Performs infection control surveillance; identifies trends; reviews procedures in departments ancillary to Nursing, as well as contractors on-site, to insure aseptic and sterile techniques are implemented; implements proactive infection prevention techniques where applicable. d. Oversees the annual Influenza Control Program for residents, employees, volunteers and contractors in conjunction with the assigned supervisor. e. Reports reportable diseases to the Department of Health. f. Maintains and updates the Bloodborne Pathogen Exposure Control Plan; assists with resident testing for bloodborne pathogen exposures. g. Collaborates with Occupational Health and Safety by identifying and monitoring employee illness; makes recommendations and takes appropriate action to ensure the health and well being of residents.. h. Facilitates, monitors and provides tuberculosis testing, immunizations such as hepatitis B, influenza and other appropriate medications as deemed necessary by the assigned supervisor to employees and volunteers. i. Maintains an awareness regarding OSHA regulations in the workplace; writes and implements programs where appropriate. j. May prepare documentation and maintain programs to meet State licensure, managed care insurance, and life safety requirements. k. Tracks and maintains records for infection control, following A.P.I.C. guidelines within the health care facility, as designated by the Director of Nursing. Employee Health a. Directs the Worker's Compensation Program including monitoring claims, receiving and responding to reports from providers, communicating incident data to supervisors, and coordinating information between employees' providers of care and claims administrators, as assigned. b. Updates and processes OSHA reports and forms, including completing and managing OSHA logs. c. Develops modified job descriptions in conjunction with department head/supervisor when employee is released by physician to return to work with restrictions, i.e., transitional duties. d. Coordinates Return to Work and all functional capacity assessments with supervisors and TPA. e. Administers tuberculin skin tests to all employees and maintains records; monitors and administers Hepatitis B, flu vaccine to all staff, as designated by the Director of Nursing. f. Maintains employee health files. Case Management a. Functions as an advocate on behalf of the residents and the family as appropriate. b. Assists with receiving and processing of admission applications at the facility. c. Provides information regarding admission or alternative placement for prospective clients, families, and responsible parties. d. Serves on the Quality Assurance Program Improvement Committees. e. Supports and assists with direction and management of specified aspects of nursing services and activities while assuring delivery of resident care is within regulatory guidelines. f. Provides training to nursing staff in accordance with clinical competency guidelines. g. Consults with the Director of Nursing to ensure the annual competencies are completed by all employees as indicated in Diakon policy. h. Assists members of nursing management to analyze new competencies needed to impact acuity of current residents, ability to care for residents in community and admission of residents. i. Communicates with Sales/Admissions team regarding potential clinical denials and developed QAPI plans for increased clinical acuity as appropriate. j. Initiates and promotes the facility s continuous quality improvement efforts. k. Performs utilization review and client medical record audits according to Diakon s policies. l. Participates in the implementation of corrective action plan for deficiencies identified through utilization review, clinical record audit, Medical claim denials, client denials, client satisfaction surveys, state surveys, and the Quality Improvement Plan. m. Participates in the development and implementation or individual and group training. n. Participates in the implementation of standardized policies and operating procedures to be utilized throughout Diakon. o. Assists with education, monitoring and utilization of interactive tools. 4. Adheres to specific procedures and programs. a. Insures an atmosphere which allows for the privacy, dignity, and well being of all residents, patients, and clients in a safe, secure environment. b. Follows standard precautions. c. Maintains awareness of individual responsibilities under the established Health and Safety Program and Fire/Safety Disaster Plan. d. 5. Plans, directs, coordinates, controls, and leads the assigned area of responsibility and provides leadership to colleagues in a manner that is consistent with gracious service and hospitality and the mission of Diakon Lutheran Social Ministries, while complying with Diakon policy and regulatory requirements. 6. Complies with Diakon s policies and procedures, Diakon s Code of Conduct, Corporate Compliance Program including the reporting and educational requirements of the Program. 7. Complies with all applicable federal, state and local laws, regulations and standards. 8. Coordinates specific work tasks with other personnel within the department as well as with other units and departments in order to ensure the smooth and efficient flow of information. 9. Reports pertinent information to the immediate supervisor as requested, or according to an established schedule; compiles information as necessary or as directed and provides data to appropriate personnel. 10. Provides excellent customer service by interacting and responding to needs/requests/issues of employees, families, contractors, colleague agencies, volunteers, visitors and or the general public. 11. Carries out work tasks utilizing ergonomic principles in a manner that maintains health and wellness and reduces injuries. 12.
