We kindly request support in the amount of $30,000 to assist with educational expenses and $5,000 to participate in hands-on training at the Louisiana State University Medical School Innovation Center ($35,000 total). As a reminder, NCVH provides 100% scholarship for all qualified fellows to attend, including airfare, hotel accommodations, meals and annual conference registration. Support of this program includes a hands-on station for one-on-one interaction with fellows along with complimentary access to Fellows Course faculty to serve as proctors. Hands-on stations may consist of product demonstrations, case studies, simulators, models or any other (approved) learning method of your choosing. Workshop space is limited to eight (8) industry partners and will sell out quickly. See page five (5) of this document for deadlines, discounts, and late fees. This course precedes our annual conference scheduled May 31 June 2, 2017. All fellows receive complimentary registration and are encouraged to stay for the entire duration of the conference to enhance their learning experience and networking opportunities. FELLOWS COURSE DESCRIPTION The New Cardiovascular Horizons (NCVH) Fellows Course Complex Strategies in Peripheral Interventions provides an effective and interactive learning format including hands-on training using advanced simulation, one-on-one time with faculty and live case reviews combined with didactic lectures by leading experts in the field of peripheral interventions. Our goal is to provide an optimal learning environment and comprehensive overview of PAD, limb preservation techniques and advanced peripheral interventions. NCVH provides faculty and specialists from around the world who desire to stay abreast of cutting edge techniques, latest peripheral device technologies and new pharmacologic treatments to encourage fellows to strive to be the best in their field. This Fellows Course is designed as a base to build long lasting relationships with faculty and fellow attendees for perpetual learning and interaction. TARGET AUDIENCE Fellows in final two years of an interventional cardiology, interventional radiology, vascular surgery or cardiothoracic surgery U.S.* fellowship program. EXECUTIVE SUMMARY: ABOUT NEW CARDIOVASCULAR HORIZONS FOUNDATION The NCVH Annual Conference is the highlight educational program of the New Cardiovascular Horizons Foundation, a 501(c)(3) nonprofit organization. The educational leadership team includes 37 co-chairmen who contribute in various areas of content development and almost 200 faculty members from the leading institutions around the world. The NCVH Foundation takes great pride in following all nonprofit, ACCME, PhRMA and AdvaMed guidelines to ensure education is delivered in an unbiased format. Now in its 18 th year, the NCVH Annual Conference continues to expand in both attendance and focus to include multidisciplinary sessions for interventional specialists, general practitioners, podiatrist, cath lab professionals, nurses, administrators and allied health professionals. In addition to the annual conference, NCVH Foundation has expanded its reach to include the largest peripheral fellows course in the U.S. This program draws almost 100 interventional and surgery fellows from leading institutions. Course Directors create a scientific platform to bring together young leaders to learn new treatments for patients with peripheral vascular disease by providing didactic lectures combined with hands-on training and practical workshops. The NCVH Foundation will also present 15 one-day regional meetings during the 2016-2017 fiscal year in a variety of cities throughout the United States. Regional conference objectives are developed to reach family physicians, general practice physicians, podiatrists and allied health professionals to help drive the desired message of new interventional technologies and pharmacotherapies available for optimal care for patients with cardiovascular, peripheral vascular and venous disease. International relationships in China continue to grow with four NCVH sessions at nationally recognized meetings to include the Great Wall International Congress of Cardiology, China Interventional Therapeutics, China Endovascular Course and Endovascology Conference. Combined attendance for these four meetings reach 30,000+ participants. 4

INDUSTRY SPONSOR GUIDELINES Limited to 8 industry partners. Didactic lectures, hands-on workshops, and all meals/breaks will be located inside the Waldorf Astoria Ballroom (see floor plan on page 6). Due to the architecture of the room, stations will slightly vary in size, the largest areas will be assigned first come, first served as payments are received. Time for 8 workshop rotations have been allotted during the agenda. Refreshment breaks and meals will be provided inside the room throughout the day for all faculty, fellows and sponsors to enjoy at their leisure. Each faculty member is available for the duration of the day as an expert proctor for the industry hands-on stations, based on a first come, first served basis as support confirmation is received. If an industry partner does not choose a faculty member, one (or more) may be assigned. Each fellow will be assigned to a group numbered 1-8. Each group will have an average of 10-12 participants and will visit each workshop station for at least 20 minutes throughout the day. Hands-on stations may consist of product demonstrations, case studies, simulators, models or any other (approved) learning method of your choosing. If participation for the LSU portion of the training is accepted, additional products, supplies, simulators and/or models will be required to ensure an optimal learning opportunity for the fellows. Sponsors are encouraged to make appointments with fellows during the rotations for detailed follow up during the annual meeting via onsite innovation labs, trade show booths or private meetings.
