JOB SCRIPTION JOB TAILS Job Title: Overnight Community Nursing Health Care Assistant epartment: Out of Hours istrict Nursing Service Location: Beech Cottage, Woodingdean Hours of Work: As agreed Reporting to: Community Nurse Manager Reporting to Post Holder: No staff report to the post holder ate: 25/05/2014 JOB PURPOS This is a clinical position for an enthusiastic team member providing care for patients within the Brighton and Hove locality. The emphasis is on ensuring that decisions made are based on good clinical practice, and inline within the prescribed protocols and both local and corporate policies. The post holder will be required to work as part of a team consisting of Nurses and Health Care Assistants. The expectation is that the Health Care Assistant s role may be working within the mobile team or as a Night Support Assistant. The post holder will be required to assist the nursing staff in the delivery of a high standard of care. The post holder may develop and be deemed competent in skills that facilitates working alongside another healthcare assistants should this need be identified. The post holder may work alone within patient s own homes and is expected to be aware of Health and Safety and Moving and Handling policies and are responsible for adhering to these. They are also responsible for recognising changes in the patient s condition and effectively reporting back to nursing staff. The post holder is jointly responsible with the nursing staff to ensure that good communication and handover is performed. Have a flexible approach to working hours, location and roles dependant on the service needs as identified by the Team Leaders or Manager COMMUNICATION AN KY WORKING RLATIONSHIPS The OOH N service is part of the Community Short Term Services. These services include the Community Rapid Response Service (CRRS), Intermediate care Services and Beds, Community IV Therapy Team. The service may receive referrals from any professional team and these include the IPCT; CRRS; Paramedics; GP s; IV Services; Hospice at Home and 111. Good communication is vital to ensure a continuity of care and an accepted standard for patient experience. The post holder will be responsible for ensuring this.
