Instructions 1. All applicants must have a minimum cumulative grade point average of 3.000 as of the end of Fall Semester 2015 or the term you graduated with your BA/BS (whichever comes first). 2. All applicants must exhaust their four years of athletic eligibility by the end of the 2015-2016 academic year (Summer 2016) or forfeit any remaining eligibility. 3. All applicants must have completed a baccalaureate degree by the end of the 2015-2016 academic year (Summer 2016). 4. All applicants must adhere to the selection criteria according to the special award descriptions included with this packet. 5. All applications must include the following: this application form, a typed personal statement from the applicant, and a letter of recommendation from the applicant's head coach. 6. Application forms MUST BE TYPED. You have two options: a. You can type the information requested and print a hard copy of the application form to submit to the SASSO office in the Younkin Success Center, Room 350, 1640 Neil Avenue. Be sure to "Save As" if you want to save your work. b. You can type the application information directly on your computer and save as a PDF document. Please e-mail the PDF file to swearingen.53@osu.edu prior to the designated deadline. 7. Recommendations from head coaches should be typed on the coach's letterhead and addressed to "Selection Committee" so the same letter can be used for all awards the student has applied for. The letter can be submitted separately from this application form. 8. Your personal statement should be between 500 and 1000 words. In your statement, please explain how your athletic, academic, university, community service activities, and/or work experiences have prepared you for postgraduate education, what your future career goals are, and how graduate/professional school will help you achieve those goals. 9. History and Qualifications for the Special Awards The Corwin A. Fergus Memorial Award (Male or Female student-athlete award) Fergus scholars are male or female scholar-athletes who have excelled in athletics, achieved academically (3.00 GPA minimum), demonstrated positive qualities of character and citizenship, and shown potential for success in a graduate or professional program. Established in 1967, the award is given in memory of Corwin A. Fergus, a member of The Ohio State University football teams in 1912, 1913, and 1914; a graduate of The Ohio State College of Law; and later a successful businessman. The award reflects his strong commitment to the pursuit of academic and athletic excellence and the Fergus' positive contributions to society. These four $5,000 post-graduate scholarships, each renewable for up to three years, are a portion of funds from the Fergus endowment which generously supports our student-athlete educational programs in many ways. This award can be utilized for graduate study or professional programs at The Ohio State University. The Leann Grimes-Davidge Award (Female student-athlete award) This award is a tribute to Leann Grimes-Davidge, recognizing her many contributions to athletics at The Ohio State University. An outstanding athlete, Leann won four state and Big Ten titles, represented Ohio State at the national level, and is a member of Ohio State's Sports Hall of Fame. She graduated Summa Cum Laude, then received her master's degree at Miami University, where she was a successful coach of the women's tennis team. The recipient of this award is a woman scholar-athlete who reflects Leann's outstanding qualities of athletic performance, scholarship (3.00 GPA minimum), character, campus or community involvement, and leadership. The A.K. and Judith A. Pierce Award (Male or Female student-athlete award) Funded from an endowment established in May of 1996, this post-graduate scholarship was presented for the first time in 2003. The award provides a post-graduate scholarship to a senior scholar-athlete who will attend graduate school at The Ohio State University. Preference is given to one who intends to enroll in the Fisher College of Business, but is open to anyone who has excelled in academics, athletics, and personal growth attained through perseverance and hard work. The M/I Homes Foundation Award (Female student-athlete award) Established in 1976 and awarded yearly thereafter, this award was among the first designed specifically to recognize and promote women's achievements in intercollegiate athletics at The Ohio State University. The scholarship provides two outstanding women scholar-athletes with assistance of $5,000 for post-graduate studies in the program of their choice at Ohio State. The recipient of this scholarship should have demonstrated academic (3.00 GPA minimum) and athletic success, but also contributed to the community through involvement in civic or university organizations. The Men's Varsity O Award (Male student-athlete award) This was a new award in 2006. This award will provide an outstanding male scholar-athlete with a one time stipend of $4,000 for post-graduate studies in the program of his choice at Ohio State. The recipient of this scholarship will be a student-athlete who has not only demonstrated academic achievement (3.00 GPA minimum) and athletic success (a varsity letter winner), but one who has also participated in community service, performed charitable work, and/or participated in non-athletic activities or organizations at Ohio State. The Woody and Anne Hayes Award (Male or Female student-athlete award) The Woody and Anne Hayes Scholarship Fund was established in 1979. This award will provide one or more varsity athletes with financial assistance for postgraduate studies. The recipient of this scholarship will be a student-athlete who has demonstrated academic achievement (3.00 GPA minimum) and leadership qualities. The John Dana Cole Award (Male or Female student-athlete award) The John Dana Cole Fund was established in 1966 by a gift through the Development Fund from John D. Cole. This award will provide a one time stipend for post-graduate studies at Ohio State. The recipient of this scholarship will be a student-athlete who shows the promise of success in his/her chosen field, and has demonstrated academic achievement (3.00 GPA minimum).
