As we age, we all need personal, medical and emotional care. Geer Village supports seniors and their families through all the stages of aging with tender, skilled care, available day and night. Geer Village Care Services provides skilled nursing, specialized care for those with memory impairment, physical therapy, pharmacy and compassionate end of life care. We also offer short-term in-patient services for people of any age from our surrounding community, assisting them to recover from an injury or to heal after surgery. Our staff is trained to support the health needs of seniors, including stroke recovery, knee and hip replacement, orthopedic and neurological issues, pulmonary illnesses, pain management, cardiac care and other conditions. We make sure that our residents and their families are as comfortable as possible and also comfortable with the transition to skilled care whether it s for short term rehabilitation or long-term residence. We pay close attention to individual preferences, providing personalized physical, emotional and medical care. Some residents are frail or handicapped in some way, while others are more active. We encourage everyone to take part in the many activities we offer. Patient care is directed by Geer s Medical Director or the resident s own physician, backed by a highly trained, certified and licensed staff of geriatric caregivers. Our team has always had extraordinarily low turnover. More than forty members of our staff have worked at Geer for over twenty years. Many staff members have relatives who worked here in the past, or even now: mothers, daughters, brothers entire families who have dedicated themselves to caring for our seniors at Geer. It s personal, it s heartfelt, and you can feel it when you re here.

Short Term Inpatient Rehabilitation In addition to caring for our residents, we provide short term inpatient medical care for people of all ages in our community. We treat a wide variety of illnesses including cardiac, respiratory and orthopedic problems or injuries. We love to see our patients on their way home - often in a matter of weeks, or even days! There are specialized programs for residents who are experiencing problems with language, comprehension, memory retention and other cognitive difficulties. Our therapists can help to restore patients abilities with speaking, hearing and swallowing. Special Care for the Memory Impaired A separate care unit for the memory impaired provides for their physical, emotional and medical needs. They re safe within a secure environment, with increased staffing levels around the clock, and a dedicated therapeutic recreation staff who encourage residents to engage in a full program of activities and crafts. Clinical care is coordinated by an experienced medical team who is board certified in geriatric care as well as internal medicine. We ve also had specialized training from the Alzheimer s Association. Families can be assured that their loved one who is experiencing memory impairment or the stages of dementia is receiving the best possible care and attention at Geer. Stress Relief Paperwork Support for Residents and Their Families Health care for seniors can be a puzzling maze of Medicare Parts A, B, C and D, Medicaid, supplemental insurance, referrals, regulations, deductibles and co-pays. Our Social Services team helps to cut through the red tape, assist you with submitting required paperwork and ensures that treatments are covered to the maximum extent possible. Because we know the rules and regulations, we can often ensure that your treatment continues as long as possible. We even follow up to arrange visiting nurses, home health aides or other services you may need when you go home. We ve got you covered.

On-Site Pharmacy Our pharmacy serves all of Geer s residents and staff and is located on the first floor of the Care Center. We deliver medications to our residents rapidly when needed. The staff has expertise in senior medication management and experience with Medicare Part D and other prescription insurance plans. Knowing our residents, their medication regimens and their physicians, we re able to carefully monitor their safety and well-being. End of Life Care Geer is dedicated to caring for seniors at all stages of aging. Sometimes that means supporting men and women facing the end of their lives and supporting their families as well. We ve provided end-of-life care for more than four decades. We ve created spacious, quiet, private rooms, specifically designed so that hospice patients can be with family and friends. Our staff attends not only to the physical and emotional comfort of people in their last months, weeks and days, but we also take them into our hearts with love. I have nothing but love and respect for each and every person at Geer: CNAs, nurses, social workers, housekeeping, maintenance, recreation, physical therapy - you have all the most wonderful, caring people in one place! Adult Day Center The Geer Adult Day Center is a warm and loving place for seniors who are living on their own or with family, but who need care or supervision during the day. Their caregivers have the freedom to go to work or attend to other responsibilities, knowing that their loved one is in good hands. Instead of sitting home alone, our participants spend up to five days a week here with close friends who care about one another. We provide door-to-door transportation in our wheelchairaccessible vans and transportation to medical appointments when necessary. Dial-A-Ride Geer Dial-A-Ride provides courteous, careful transportation to your medical appointments and other needs, free of charge. Service is available Monday through Friday for residents of Salisbury, Sharon, Cornwall, Canaan (Falls Village) and North Canaan. We pick you up right at your door for rides to doctor and dental appointments, veteran service offices, therapies, dialysis and other treatments, as well as trips to the pharmacy, shopping, hairdresser, etc. All our vans are handicapped accessible.

Hours: 7:00-5:30 Monday, Tuesday and Thursday 7:00-4:00 Wednesday and Friday Phone: 860-824-3820 Physical Therapy Our goal at Geer Physical Therapy is to return patients to their normal activities, as free of pain as possible, as soon as possible. Physical therapy is very often an extremely effective way to manage pain and mobility issues. After a few visits, many of our clients are feeling better, taking less medication and coping with life more easily. We treat common conditions such as headache, neck and low back pain, shoulder and elbow tendonitis and hip, knee, foot and heel pain. We help patients recover from injuries and manage arthritis, jaw pain, workplace ergonomic issues and more. Our services are available to anyone who resides in Geer Village Residences and they are also available to short term rehab residents who are recovering from recent surgery, an accident or an injury. We also treat outpatients of any age from the surrounding community. Our clients range in age from 2 to 100, and come from all over the tri-state area. Our facility is a warm, friendly and open place. When you visit with us, you know that you re not alone, our staff if here to help - from our front office to our therapists. We ll find the best approach to help you manage your condition, with well-established, non-surgical treatments that offer high benefits with low risk of harmful side effects. Treatments include: Manual therapy Stabilization exercise and conditioning Laser and ultrasound treatments, Ice and heat Balance and fall prevention Western acupuncture methods Aquatic therapy using our Hydrotrack and the swimming pool at the Canaan YMCA, right here at Geer Village We accept Medicare and all major insurance plans. You can be evaluated and treated in our Physical Therapy department without a physician referral, or we d be glad to coordinate your care with your doctor. Our office staff will assist you at every step of the way. We ll help to get whatever approvals you need from your insurance plan and make sure that you know in advance what co-pay responsibilities you may have.

It s the little things that can mean so much. When you just need someone to talk to, we re here for you. Skilled, tender care for all stages of aging. Over 100 acres in the rolling hills of northwest Connecticut. When I had to take my mom from her own place to a nursing home, she was afraid. But after some time at Geer, that fear was replaced by love. No matter when I came to see her, I could tell that she was loved and well cared for. I didn t have to worry about her being alone or ignored because that would never happen at Geer. I would never have put her anywhere else. Our mother recently spent three weeks at Geer. This was a very difficult time for our family, but the folks at Geer made it much easier than we could ever have imagined. The physical therapy staff understood her fear and helped her recuperate from her fall quickly, working hard to meet her goal of returning home soon. Geer also stepped in to arrange what was needed for her to come home: arranging in-home therapy, a home health aide and meals on wheels to be delivered.
