PURPOSE OF THE POSITION The purpose of the position is to support the Regional Work Programme for Primary Care. Whilst the initial focus will be on the development, implementation and ongoing management of clinical pathways as prioritised in the Clinical Pathways Programme, there is an expectation that the Service Delivery Manager - Regional Work Programme would work across a number of key project areas, and would have a highly developed project management skill set that would enable the enactment of other key projects as they are identified by the District Alliance partners over time. Background - the Clinical Pathways Programme In December 2015 the two metro Auckland Alliance Leadership Teams approved a business case for this programme of work with project duration of 31 months then a transfer to business as usual from July 2018 until June 2023. The Clinical Pathways Programme, which is currently hosted on the Auckland Regional HealthPathways platform, went live in August 2015. The Programme Team is currently located at Lambie Drive, Papatoetoe, but works across the metro Auckland Region as required. Funding the Current Clinical Pathways Programme of Work The Auckland Regional HealthPathways and the clinical content development team are funded 50:50 by the metro Auckland DHB s and seven PHO s, with Counties Manukau Health acting as lead DHB on behalf of the region. Contractual Arrangements Counties Manukau Health holds the contract with Streamliners (for Auckland Regional HealthPathways) on behalf of the three Auckland DHBs and seven PHO s. NATURE AND SCOPE OF RESPONSIBILITIES Key Accountabilities Standards / Achievements Relationship management Establish effective working knowledge of the purpose and benefits of Auckland Regional HealthPathways platform and familiarity with establishment and review processes. Maintain a body of health system knowledge. Maintain effective liaison and engagement with key relationships: - Auckland Regional HealthPathways Clinical Lead and the local Clinical Editors - Other HealthPathways Community team(s) - Clinicians and providers - Other HealthPathways stakeholders Manage multi-layer relationships within and between the three DHBs, seven PHOs and the two district Alliance Leadership Teams. Programme co-ordination Work effectively across the business to ensure successful delivery against the requirements of a high maintenance work programme. Support the Programme Manager, Clinical Lead and the local Clinical Editors for Pathway coordination and finalisation. Maintain HealthPathways administration tool (DOT). Work closely with Streamliners Regional Lead to ensure efficient and effective use of the technical writing budget. Coordinate relevant information from Subject Matter Experts and liaise with the HealthPathways Community membership as required. Maintain a pathway review process ensuring collaborative clinical involvement in reviews. Gather and maintain local service provider information including hospital and community based services. Identify and gather news and write content for bimonthly subscriber updates. Monitor dates and ensure actions and cycles are followed, advising key staff and participants of participation requirements. Support the Clinical Lead with the Clinical Pathways Operational Group agenda and meetings. HealthPathways Reporting and Monitoring Ensure reporting requirements for the programme are met. PROBLEM COMPLEXITY This post will be required to support the Programme Manager in delivering this programme of work within a complex stakeholder environment with competing priorities, requirements, and in doing so will be required to develop and build effective relationships with a diverse group of stakeholders. There will be a requirement to be able to prioritise issues and negotiate time frames, while still providing a quality service. There will be demands to meet deadlines, maintain accuracy and quality of information. DIMENSION OF THE POSITION Will be responsible to the Programme Manager, Primary Care Work Programme for the achievement of agreed objectives and operate within the confines of Counties Manukau District Health Board as host on behalf of the two metro-auckland district alliances.
