Purpose of the role: To provide a high quality, efficient and effective Occupational Therapy service as a member of an interdisciplinary team for patients and families / caregivers, whanau, those require community based rehabilitation in the home or community. Services are delivered in accordance with Waitemata DHB and Medicine and Health of Older People Strategic Plans and Waitemata DHB values. KEY TASKS To provide occupational therapy interventions to EDARS patients in the community. EXPECTED OUTCOMES Assessments used relate to patients needs in areas of occupational performance and where possible are standardised and validated Appropriate patient assessments are conducted within specified timeframes and to agreed standards Assessment and intervention approaches promote continuity across services Agreed intervention is based on the findings of the assessment and the patient s needs and goals. Documentation accurately demonstrates interventions and progress and is in accordance with documentation standards set by Waitemata DHB Measurable patient goals are set within an interdisciplinary framework and are achieved within specified timeframes. collaboration with patient, family, whanau and interdisciplinary team as appropriate. There is liaison with community and inpatient groups to ensure excellent continuity of service for patients. Education to Waitemata DHB and external groups occurs in consultation with management. Boundaries of practice are identified / recognised and patients are referred on as appropriate Always seeking and striving for new ideas and solutions To provide training and supervision of peers, and therapy assistants Provision of appropriate training and delegation of tasks to therapy assistants to ensure the best use of resources and promotion of interdisciplinary patient intervention Quality Assurance Supervision is undertaken on a regular basis (as set out by the New Zealand Board of Occupational Therapy and Waitemata DHB Professional Supervision for Allied Health Staff Policy) Participates and completes annual performance review (PDR) which is aligned with the service requirements and Waitemata DHB performance development guidelines. Objectives are reviewed on a regular basis Responsibility is taken for own learning by recognising training and skill needs and undertaking professional development activities on an ongoing basis. Professional Development Continuing Competencies is developed and maintained, including a training record. Completes Waitemata DHB mandatory training Follows ethical guidelines, the code of ethics and conduct and the standards of practice for occupational therapy. Participates in peer review. Contribution is made to development and review of best practice guidelines.
