Infection Prevention prevent hospital-acquired infections proactively Enterprise Software and Consulting Solutions for Improved Population Health

s Enterprise Software and Consulting Solutions Healthcare reform has fundamentally changed reimbursement models and the approach to patient care. Providers need to deliver value-based care by reducing costs while improving outcomes. Bundled payments require hospitals to manage patient populations proactively to control acute and post-acute care costs. Our enterprise software solutions and consulting services enable you to meet the challenges of population health and performance outcomes management. Infection Prevention Hospital-acquired infections are a significant clinical and safety issue. On any given day, about one in 25 hospital patients has at least one healthcare facility-associated infection, according to the Centers for Disease Control and Prevention. Health systems must adopt strategies to mitigate risk factors and streamline information flow and reporting. Effective prevention and control programs promote patient safety and high-quality care through comprehensive surveillance, proactive policies that reduce the risk of patients and hospital employees acquiring and transmitting infections, and evidence-based interventions. Infection Prevention enables hospitals to monitor and track hospital- and communityacquired infections, document detailed reviews, analyze trends that require intervention, and meet reporting requirements. The software integrates critical information from patient feeder systems, including demographic and visit details, operating room procedures, and real-time microbiology tests and results. Care Real-time patient data and microbiology lab results in an HL7 interface Detailed documentation and tracking of patient infections Outcomes Evaluation Bi-Directional EHR Integration Automatic alerts for patients who have histories of drug-resistant organisms, including MRSA and VRE Work list reminders and alerts throughout infection investigation Robust reporting options Quality Patient Safety Performance Performance Outcomes Population Health Across the Continuum Consulting Services Robust Rules Engine Health Assessments Risk Stratification Real-Time HL7 Interface The system includes a real-time HL7 interface that is populated with relevant patient data, including demographic and visit details. Also included are HIM coding; medication orders; and transcription details and labs, such as microbiology, which includes results and pathogen profiles. In addition to documenting current infections, hospitals can set alerts for patients who have histories of drugresistant organisms. Microbiology Work List Microbiology test results are posted into the system via real-time HL7 interfaces. Users can filter the test results online to include only those patients who have positive cultures. Infection prevention specialists review the results online, including pathogen profiles and drug sensitivities. They then link results to detailed infection reviews for classification, tracking and reporting. As new results are posted, users can link them to existing infection reviews. Data Analytics Workflow Automation Best-Practice Content Online access to microbiology test results Identification of ordering physician and order date Specimen details, including collector identification, collection date, site and source Access to detailed pathogen profiles and drug sensitivities Ability to review patient details and complete visit histories 2 3

Infection Prevention Work List The Infection Prevention system includes a work list where infection prevention specialists classify and track nosocomial and community-acquired infections. Users can generate detailed reviews based on microbiology test results. In addition, s rules engine automatically identifies and schedules reviews of admitted patients who have had prior infections. The work list also displays patients with infections referred by nurses and other caregivers. Infection Review Infection prevention specialists document reviews to classify each infection and provide critical details, including infection site, nursing unit, service, shift, operating room procedures and staff involved. Hospitals can also record and monitor: Contributing factors Related devices Treatments Isolation histories Mandatory reports users send to state and federal agencies User-defined rules to identify patients who have infection histories Online access to patient demographics, visit histories, alerts and review details Access to microbiology results to identify patients who require review Ability to filter and sort infection reviews based on reviewer, unit, status, infection classification, pathogen and other variables Links to microbiology tests and results Links to operating room details Ability to classify each infection and document specific infection details, including onset date, source (nosocomial or community), site, unit, service, review status and findings User-defined forms and workflows that support unlimited focused studies Flexible standard and ad hoc reporting options practitioner competence and report the results. The software includes clinically vetted proprietary indicator content and specialty-specific report templates. The system provides an intuitive drag-and-drop interface that enables organizations to configure and download reports. works with organizations to map data from existing hospital systems, so they re able to run performance reports automatically and then review them online. Organizations can use Infection Prevention to automate the identification and tracking of indicators defined in the performance management system. The indicators are displayed on the quality work list, where quality reviewers complete their reviews and confirm attribution. When reviews are complete, the findings are included in s performance reports automatically. A -hosted website, which includes specialty-specific report templates and indicator content Indicators linked to clinical privileges Data mapping to hospital systems Enterprise Solutions The Quality system can stand alone or integrate with s other enterprise solutions. also integrates with credentialing and privileging software from Morrisey, A HealthStream Company. These solutions provide integrated clinical and administrative data that supports effective, efficient operations and reporting.
