Job Description Position Title: Department: Reports To: Director of Care Administrator Purpose Reporting to the Administrator, the Director of Care (DOC) is responsible for planning, coordinating, supervising and leading the nursing care services within the long term care home that supports the Colour It resident led philosophy of care. This includes the effective management of all resources in the nursing envelope and development of a strong nursing team that will deliver optimal care and service, based on legislative requirements and County of Grey Standards. Responsibilities General Administration Supports, interprets and incorporates the Corporate Mission, Vision and Values into all aspects of nursing services Ensures that all Nursing staff function within their scope of practice and demonstrate knowledge and skill required for the work Establishes internal mechanisms and audits to ensure that resident records (paper and electronic) are utilized, stored, maintained, retained and destroyed as per County of Grey protocols Establishes systems to ensure all standards and regulations, such as MOHLTC Program Standards, County of Grey standards, CNO, are adhered to on an ongoing basis Participates in strategic planning and accreditation activities Ensures that all nursing committees are functioning within their terms of reference and areas of responsibility i.e., Falls, Continence, Nursing Practice, Wound Management, Infection Control, PAC Participates in committees as requested and attends the Corporate Director of Care meetings Performs nursing duties as required Page 1
Human Resources Participates in and leads the recruitment, orientation, and performance management process in accordance with County of Grey standards for all employees in the nursing department Establishes, monitors and evaluates staffing patterns and work schedules and assignments that maximize continuity and consistency of care, and nursing retention in accordance with the nursing envelope budget and Collective Bargaining Agreements, as applicable Ensures recertification of staff in all Transfers of Function and added Nursing Skills Provides leadership to the Nursing Leadership Team and Charge Nurses in all human resource functions Ensures that staff performance is evaluated during probation and annually thereafter with measurable performance goals Identifies nursing department learning needs and makes provisions for appropriate education Ensures the provision of mandatory annual educational in services for all nursing department employees Maintains up to date records on all nursing personnel Monitors absenteeism within the department in accordance with the Attendance Management Program Fosters positive working relationships with the union representatives Financial Accountability Participates in the development of the annual nursing budget Manages and monitors all nursing envelope expenditures, explains monthly variances and works with the administrator to identity corrective action Responsible for tracking High Intensity Needs / Lab costs and submission of the reconciliation to the MOHLTC within required deadlines Ensures there is effective and efficient procurement and supply control procedures within the nursing department Oversees the process to ensure that the Levels of Care Classification (CMI) is maximized Quality Management Provides leadership and direction to implement all aspects of the Quality Assurance and Risk management program as it pertains to nursing Establishes processes for the collection and analysis of clinical indicators within an established time line on a monthly basis Makes recommendations for corrective action to reach corporate mean for Page 3
clinical indicators Establishes processes and accountability for completion of monthly audits, analysis of results and development of corrective action plan Assigns responsibility for the management of all internal nursing systems and monitors effectiveness of same i.e. lab specimen collection, nursing supplies, equipment use and monitors effectiveness Holds staff meetings on a regular basis to ensure clear dissemination of information and expectations Establishes improvement plans as required to address quality care and service issues Care Coordination Maintains up to date job routines for all nursing personnel Participates in conferences with interdisciplinary team providing care to the residents Liaises with the medical advisor and in collaboration with the Administrator evaluates his/her performance and ability to meet the requirements of the role Supports evidence based nursing practice and research activities that will enhance resident care delivery Ensures quarterly summaries and MDS assessments are completed in PCC on time Family & Resident Relations Ensures that all staff foster positive customer service with residents and families in their day to day work Ensures ongoing communication with families to inform them of changes in health status, changes in policy and care conference dates Community Relations Promotes a positive working environment with the service providers working with the Home Establishes and maintains positive communication between the Home and community and community partnerships Occupational Health & Safety Observes work patterns and ensure safe working practices are adhered to at all times Implements and monitors zero lift policy Ensures that all nursing staff are properly trained and aware of emergency fire and disaster procedures Page 4
Ensures nursing staff perform pre start up inspections on all equipment requiring same i.e. lifts Conducts daily safety audits in the nursing department Ministry of Health Compliance Fosters good working relationships with relevant persons within the Ministry of Health Knows and interprets MOHLTC program standards and County of Grey standards Implements processes and systems to ensure compliance to all standards on an ongoing basis Ensures sustainability of all systems and processes in place to achieve compliance. Completes and executes compliance plans as needed Working Conditions Modern facility, temperature controlled, smoke free environment Combination of natural and fluorescent lighting Some disruption to lifestyle resulting from extra hours spent dealing with prospective clients, attending meetings and dealing with residents and families and responding to emergencies Sitting for meetings, computer work major job function Required to meet deadlines Contacts Internal Working Relationships Internal working relations with the Administrator, other Management staff, care givers and other employees, residents, Committee Members, Medical Director and attending physicians. External Working Relationships External working relations with residents families and next of kin, local Health Unit, Ministry of Health and officials, CCAC staff, care giving agencies, Physiotherapists, other long term care facilities, sales personnel, clergy, coroner, funeral homes and the general public. Knowledge and Skill Must have a current registration with the College of Nurses of Ontario Must have a Bachelor of Nursing Science or equivalent post diploma training Must have at least three years experience working as a Registered Nurse in a managerial or supervisory capacity in a health care setting with a minimum of one year in the in the long term care sector Must possess excellent leadership, team building and communication skills. Must have a sound clinical skills and knowledge of gerontological nursing practice Must be proficient in Microsoft Office Suite, internet use and computerized documentation Must be familiar with the Classification process and RAI-MDS Must know the MOHLTC program standards A current (within 6 months) Vulnerable Sector Check Impact of Error Improper supervision and management may result in very significant negative consequences to the well-being and quality of residential life. The lack of compliance to relevant Acts and Policies may lead to the citing of unmet standards resulting in a loss of public trust. Ineffective application of policies programs and service delivery may result in lost confidence by the residents, staff and the public relating to the Homes ability to provide appropriate care. Page 6
