It has inspired all of us to focus more on healthy eating and exercising. Our team has formed a lasting bond and has encouraged many students in our school to do the same while changing their habits in a healthy way. Fuel Up to Play 60 Rally for School Health Grant Components: Up to thirty schools awarded by random selection based on school demographics, geographic location, and performance on previous grants. *Multiple buildings in a district may apply, but only one building per district will be selected. Funds to support travel to Ford Field and related travel expenses for up to nine team members; 6 students and 3 adult Program Advisors. Funding amount is determined by distance from the school building to Ford Field in Detroit: - Within 150 miles = $500 - Greater than 150 miles = $1000 plus up to three hotel rooms with valet parking at the Antheneum Hotel in Detroit near Ford Field. A day-long, action packed Fuel Up to Play 60 Rally that will empower and teach students (and the adults who join them!) ways to begin/continue making changes at their school supporting healthy eating and physical activity. Promotional items (specifics to be determined) sent directly to your school to start your school year and build up excitement for other students and teachers to join in your school wellness efforts. The opportunity to apply for additional funds to help implement Fuel Up to Play 60 (up to $4,000 available per school building) is available on each school s Dashboard at Grant Requirements: If your school is selected as a recipient of a Rally for School Health Grant, you ll be required to sign a Memorandum of Understanding (MOU) with United Dairy Industry of Michigan, agreeing to complete the following grant requirements during the 2018-2019 school year: All nine team members (including six students) must be available to participate all day at the event (8:30am-3:00pm on either October 9 or October 16, 2018). Three adults and six students must make up your school team two of the three adults must be registered as Program Advisors at Key school leaders Principal, Food Service Director/Manager, and Physical Education teacher must approve of this grant application submission and fully support the proposed program and requirements. Grant Requirements Continued: Important Dates May 9, 2018 (Wednesday): Mandatory webinar/ conference call #1 for all grant awarded schools. At least one adult Program Advisor must be present. Agenda will cover grant requirements, what to expect from the Rally event and how to register your team. You can call in at 7 a.m. OR 3:30 p.m. for a 30-minute call. May 25, 2018 (Friday): Deadline for grant winners to submit ALL registration paperwork. November 9, 2018 (Friday): Must coordinate and execute a school-wide Fuel Up to Play 60 Kickoff activity/event on or prior to selected date. Fall 2018: Schools should identify, coordinate and begin implementation of at least one Healthy Eating Play and at least one Physical Activity Play that is sustained for at least four weeks. We encourage Plays to last through the end of the school year. April 1, 2019: Complete all Six Steps of Fuel Up to Play 60 and check them off on the Dashboard at www.fueluptoplay60.com to be nationally recognized as a Touchdown School and be eligible to win prizes. May 17, 2019: Final submission of three (3) success stories with pictures/videos on www.fueluptoplay60.com, one each for Kickoff, Healthy Eating Play and Physical Activity Play. Complete UDIM success story confirmation form. NEW: Must report pre- and post- school milk and yogurt sales and ADP (average daily participation) of breakfast and lunch at the end of the school year for 2017-2018 & 2018-2019. Will need to work with Food Service Director to obtain this. Technology Requirements: School/Program Advisor(s) must acknowledge that computers being used need to have updated PDF reader software. School team s Program Advisor(s) should be competent at opening, saving, and sending files (PDF s, Word documents, and pictures), or be willing to communicate with school s IT (information technology) person for assistance.
