A message from the Director Research and Innovation: What will it mean to you over the next 5 years? Professor Anoop Chauhan Director of Research and Innovation Portsmouth Hospitals NHS Trust We will bring increased opportunities for our patients, staff and our partners in healthcare You will be able to receive new technologies, experience new or improved services, be offered new treatments and have opportunities to take part in clinical research trials We will listen to your needs to improve health outcomes across Portsmouth and work with you to develop new innovative ideas We will create new or improve existing services for chronic conditions and acute care Research and Innovation is important because it allows us to meet and solve some of the challenges facing the NHS, and it is at the heart of everything we do at Portsmouth Hospitals NHS Trust. We have set a vision and delivery plan so that research and innovation is spread out at pace and scale to ensure we deliver the highest quality care. Our challenge: we know that with our local population growth, increased life expectancy with an increased prevalence of life style related diseases, has put huge pressure on healthcare providers to maintain high quality care. Research and Innovation will provide solutions to help deliver this care in different cost effective ways that maximises efficiency and achieves the best health outcomes. We have already made great strides in research in the last 5 years, and our new 5 Year Plan has been further developed in consultation with our patients and staff and outlines how research and innovation will be further developed at a significant rate over the next 5 years. To improve quality and productivity we need to continually develop new healthcare approaches. We will continue to encourage innovation and base more decisions on the evidence about what works best. Research is also vitally important to the growth of the UK s life sciences industry - we aim to make Portsmouth Hospital Trust one of the best places nationally and internationally for healthcare research. 5

Who we are Portsmouth Hospitals NHS Trust Portsmouth Hospitals NHS Trust provides a full range of acute secondary care services to our local population of around 650,000 people. The Trust s main hospital site is Queen Alexandra Hospital in Cosham. We employ over 7,000 staff and we are the second largest employer in the area. The Trust hosts the largest Ministry of Defence Hospital Unit (MDHU) in England. Our Emergency Department at Queen Alexandra Hospital is one of the busiest in the UK treating in excess of 132,000 patients each year. Our Trust vision is simple... Meet our Research and Innovation Team Our Research and Innovation Team Led by Professor Chauhan, the team consists of office staff, research nurses, midwives, doctors, clinical trial assistants, biomedical scientists and pharmacy personnel. Everybody who works in the Trust plays a part in healthcare innovation development and we actively encourage patients and carers to work with us in improving and transforming care. Our Vision, Mission and Plan for Research and Innovation Our vision is to be an internationally recognised centre of clinical research and innovation which will improve the health and wealth of our patients in Portsmouth. We will provide new, sustainable and effective research and innovation opportunities and services. We will continue to develop a thriving research and innovation culture with increased capability and opportunities for our patients and staff. 9

What we do We have created a thriving research culture over the last 5 years. We deliver clinical research and innovation to a high standard in collaboration with local and national organisations. We support, train and develop junior doctors across the Wessex Deanery who wish to develop their skills in research. To date we have trained over 30 junior doctors as part of our Research Fellowship Scheme. We will continue to provide clinical academic and career opportunities for nurses, midwives and allied health professionals. We have worked with many organisations and have increased our number of research funders and sponsors. We are now a provider of choice for many commercial companies. Through this we have increased our patient opportunities to participate in research. We work with patients and the public to develop new research studies, their input is crucial to the success of each new study and innovation developed. We encourage and provide a platform for our staff to think innovatively and suggest new ideas for healthcare improvements. 10

In delivering our innovation goals, we will: Reduce variations of care across Portsmouth Hospitals NHS Trust, and drive greater compliance with guidance from the National Institute for Health and Clinical Excellence (NICE) Work with industry to develop and publish better innovation uptake metrics, and more accessible evidence and information about new ideas Establish a more systematic delivery mechanism for diffusion and collaboration within the NHS by building strong crossboundary networks Align organisational, financial and personal incentives and investments to reward and encourage innovation Bring about a major shift in culture across Portsmouth Hospitals NHS Trust, and develop our people by hard wiring innovation into training and education for managers and clinicians Strengthen leadership in innovation at all levels across the Trust and set challenging and attainable innovation targets Identify and mandate the adoption of high impact innovations across the Trust 15

Impact - what we have achieved so far Developed a NEW integrated long term care model for patients with severe asthma Improved quality of life for our patients through developing new research studies and clinical target areas Developed NEW tests to monitor disease control Installed a NEW PET CT Scanner to improve Cancer diagnosis Research is being conducted across all our healthcare specialities to provide even more research opportunities to patients Over 3,000 patients are participating in clinical research studies annually The number of Clinical Academics has increased year on year and over 30 junior doctors have studied for higher degrees in the last 3 years We have developed new services and models of care across Portsmouth Developed NEW Innovations: VitalPAC technology has saved up to 400 lives a year at Portsmouth Hospital and the innovation has been adopted by other NHS organisations nationally We have developed services from research clinical support departments to ensure robust quality management systems, streamlined working as a research team and an improved the patient experience. Portsmouth Hospital NHS Trust were cited as a Research exemplar in the Future Hospital Report in 2012 Since 2012 we have increased our national and international research grants for research in respiratory, cancer, cardiac and gastroenterological diseases We have increased our research portfolio in new clinical areas 2014 HSJ AWARD WINNERS FOR Clinical Research Impact 17

Case Studies www.airsonett.com Case Study 1: Assessing a NEW Technology for Severe Asthma: The LASER Trial What is it and what does it do? The LASER Trial is assessing whether a NEW treatment device for patients with severe allergic asthma might help to reduce the frequency of asthma attacks. The treatment device, Airsonett, delivers Temperature-controlled Laminar Airflow (TLA.) It works by filtering out the allergy particles in the air of the patient s breathing zone while they sleep at night allowing the lungs to rest and recover from inflammation caused by exposure to allergy particles. The trial is funded be the NIHR HTA (12/33/28) What impact will it have on patients? We know that for many people with severe asthma their symptoms aren t well controlled by current treatments and they continue to have asthma attacks. The LASER Trial will determine if the TLA device can reduce the frequency of asthma attacks, improve asthma control and improve quality of life for these patients. What will the benefits be for the NHS? At the end of the trial, results will be compared to see if the NEW treatment device delivers the following results: www.lasertrial.co.uk Reduced frequency of asthma attacks Improvement in patient s asthma control and quality of life Reduction in cost of asthma treatment. Reduced frequency of emergency department visits and hospital admissions Reduced number of work/study days lost due to asthma. An innovative NEW treatment option for patients with severe allergic asthma. Case Study 2: NEW Treatment Technique for Diagnosing Oesophageal Cancer (Barrett s Oesophagus) What is it and what does it do? Portsmouth Hospitals NHS Trust is pioneering the use of vinegar (acetic acid spray) in detecting Barrett s Oesophagus, a pre-cancerous condition that causes changes to the cells of the lower part of the food pipe, and if undetected can lead to cancer. The research shows that acetic acid dye spray can identify early cancerous tumours in the majority of cases (95.5%). What impact will it have on patients? By identifying and diagnosing Barrett s Oesophagus earlier patients will not need to have as many endoscopy procedures (unless otherwise indicated), and detect cancer more easily. What will the benefits be for the NHS? The New treatment technique will: Reduce the number of biopsies taken Increase the number of clinicians trained to use the procedure Ensures that biopsies are taken from the affected areas only Improve the diagnosis of Barrett s Oesophagus Reduce complex treatments as identified with Barrett s Oesophagus Provide earlier diagnosis for patients and reduce the risk of further operations Offer a NEW innovation treatment for early detection of Barrett s Oesophagus Detect oesophageal cancer more effectively 18

Case Studies Case Study 3: NEW Technologies: VitalPAC What is it and what does it do? Portsmouth Hospital NHS Trust co-innovated and pioneered a new technology device called VitalPAC, an early warning system (EWS) in partnership with The Learning Clinic. With VitalPAC, nurses record patients blood pressure, pulse, oxygen levels and other indicators on the handheld devices. Specialist software automatically calculates if the patient is deteriorating. If so, the nurse is warned to monitor the patient more frequently and in some cases, to alert a doctor or rapid response team. What impact will it have on patients? The impact on clinical care is immediate and lasting. Death rates at Queen Alexandra Hospital (QAH) fell by more than 15% after VitalPAC was introduced, which equates to more than 400 lives saved in a single year. What will the benefits be for the NHS? This work forms a world-class clinical information system focused on processes of care. The hand held device is now nationally used by other hospitals. The device: Reduces mortality by up to 15% Reduces morbidity Enables clinicians to respond rapidly if a patient s condition deteriorates Improves the reliability and recording of accurate data Improves patient outcomes Delivers New Technology which is nationally recognised New innovative methods for clinical use are currently being developed www.thelearningclinic.co.uk This system captures key clinical information about patients at the bed side which enables the clinical team to support, control and standardise care Dr Paul E Schmidt Consultant in Acute Medicine, Portsmouth Hospitals NHS Trust www.activ8rlives.com Sputum sample analysis Case Study 4: Development of a NEW self-management tool for early identification of respiratory infections The SENSOR Trial What is it and what does it do? Working with Aseptika, the SENSOR study is evaluating whether a NEW sputum test, alongside daily health assessments, can predict exacerbations in patients with chronic lung diseases giving an early warning of deterioration and allowing pre-emptive treatment to avoid the need for hospital admissions. Patients taking part in the study are provided with an ipad and selfmonitoring tools. What impact will it have on patients? If the sputum test is shown to be effective in predicting exacerbations, it may enable early, selfmanagement at home and avoid the need for hospital admissions. Patients taking part in the trial have reported that they feel more empowered in self-managing their care and feel reassured by the daily monitoring. Patient self-monitoring system What will the benefits be for the NHS? At the end of the trial, results will be assessed for delivery of the following benefits: Early Identification of respiratory infections Reduced cost of treating Improved quality of life for the patient exacerbations Reduced length and severity of exacerbations Improved patient selfmanagement Reduced frequency of acute admissions at home 19

Case Studies Case Study 6: Development of a NEW mobile app to support pregnant mothers make an informed choice on the place of birth What is it and what does it do? In a project funded by The Health Foundation, our maternity unit developed an app, My Birthplace, to support pregnant women identify their preference for place of birth. What impact will it have on patients? It provides women and midwives with evidence based information to support place of birth decision making. What will the benefits be for the NHS? The mobile app: Is a New innovation first of its kind in the UK Integrates research into a tool for pregnant mothers and midwives to discuss preferences for place of birth Encourages shared decision making between women and midwives Empowers pregnant women to make the right choice for them www.health.org.uk 20
