UNDERSTAND THE CARE NEEDS OF YOUR LOVED ONE When you think about home care, you may not think about how a professional care team can help your loved one live the safest, healthiest, and happiest life possible at home. We ve organized these questions to help you understand your loved one s companion and personal care needs. One tip for approaching this is to think about how well your loved one navigates their typical daily routine. This guide is not meant to be an official diagnosis or prescription. Understanding a loved one s true care needs comes through discussion and planning among family members and medical professionals. Your local BrightStar Care Professional Care Team led by a Registered Nurse is here to help you on your journey. HELPFUL TIP Consider asking another sibling or care partner to fill out this assessment independently and then the two of you compare what you each came up with. As you go through the questions, consider your loved one s level of independence for each task. If you answer NO to any of these questions, then your loved one may benefit from companion care services. The number of times you check NO for each question may indicate the amount of support needed. Companion Care Needs For this first set of questions, we ll explore their lifestyle and household maintenance needs NO If the phone rings, can they hear the ring? If the phone rings, can they get up and answer it in a safe and timely manner? INTERIOR: Do they clean the kitchen, living room, bedrooms, and bathrooms? EXTERIOR: Do they mow the lawn, shovel (if needed), garden, etc.? Are they doing their own laundry? Carrying it safely into the laundry room, transferring it from washer and dryer, folding it, and putting it away. Do they shop for their own groceries Drive to the grocery store, pick out healthy foods, pay for the food, and transfer the groceries from the car to the kitchen? Do they stay on top of their finances? Pay mortgage; phone, cable, and energy bills, etc. Do they drive to errands like going to the bank, shopping for household items, etc.? Think about your loved one s social and emotional wellbeing NO Are they attending social and family activities they enjoy, like book club, bridge club, religious services, reunions, birthdays, etc.? Do they talk and socialize with their friends, neighbors, or relatives regularly? BRIGHTSTAR CARE HOME CARE PLANNING GUIDE Care Needs

Personal Care Needs Again, as you go through the questions, consider your loved one s level of independence for each task. If you answer NO to any of these questions, then your loved one may benefit from personal care services. The number of times you check NO for each question may indicate the amount of support needed. NO When your loved one wakes up, can they get out of bed easily? Can they walk to the restroom? Do they bathe regularly and completely? Can they get into the shower or bath safely? Do they wash their body? Do they groom themselves and maintain good overall hygiene? Brush teeth, shave, comb hair, etc. Do they put on clean clothes? Do they toss their dirty clothes in the laundry? Can they walk from the bedroom or bathroom to the kitchen without risk of falling? Think about their personal health and wellness NO Does your loved one plan and prepare their own nutritious meals? Does your loved one eat nutritious meals? Can they prepare meals to meet dietary requirements, if necessary? If they require medical equipment (e.g. oxygen), can they manage it independently? Do they take medications at the right time in the right dosage? Do they get at least 30 minutes of exercise every day? Do they drive to doctor s appointments? Other Important Care Considerations Have you observed your loved one experience any of the following? Check only one box, if applicable. NO Do you fear that they are a risk for wandering out of the house without telling anyone where they are going? Is the family or care support system available, willing, and able to provide the needed level of care? Next Steps Once you ve completed this exercise, you are now on the right path to talking with your loved ones, listening to their wishes, and together exploring the right care options that will help them live at home safely and happily. The next section of this guide shares some tips on having that conversation with mom and dad. MOM OR DAD NEEDS CARE, NOW WHAT? After you complete the Care Needs Assessment and identify specific ways your loved one could benefit from professional assistance, the next step is to talk with them about their wishes and discuss care options. These conversations aren t always easy. There are better ways than others to talk about sensitive topics. Do it the wrong way, and you risk mom or dad pushing away from future conversations and making finding a healthy solution even more challenging. To help you and your loved one enjoy a compassionate and productive conversation, BrightStar Care prepared a list of Dos and Don ts: DO Prepare questions in advance Have the conversation in person Give your family member time to get comfortable Listen with your full attention Sit in a relaxed position, facing your family member. Relax your shoulders and breathe deeply. This shows they have your attention. Ask questions Let them talk Be patient. Indicate ease and openness by keeping your hands folded on your lap Keep a notebook to record details from the conversations and any new ideas or comments that you discuss DON T Rush into the conversation, try to tackle it on the spur of the moment Try to do this over the phone, video-chat, or email Rush. Many adult children tread lightly when it comes to the health of an aging loved one, especially when it could threaten independence. And, sadly, it often takes an incident of some sort before families feel that enough is enough and have the confidence to face a health issue head-on. AARP offers a few additional strategies that families can try to approach these sensitive situations as delicately and respectfully as possible. Empowerment It is crucial for an adult child to show his or her parent that they are allies, not adversaries, and that the child fully supports the parent s desire to live independently as long as possible. But the child should suggest that accepting help would allow the parent to do what he or she wants and maintain self-sufficiency. If the parent would receive help for certain activities, then it could conserve energy to pursue other interests. When aging parents allow their kids to be their caregivers, they are intentionally giving them that opportunity to learn and grow. You can explain how much it would mean to you if the parent would receive help. Role Model Reminder Receiving care graciously is equivalent to getting old gracefully: As youngsters, adult children learned from observing their parents about how to prepare for and handle the challenges of the prime of life. As middleagers, they learn from observing their parents about preparing for and handling the many losses at the end of life. If he can display calm acceptance and resolve in receiving care, it would provide them with a model of graciousness in the face of physical and cognitive decline. If he persists in practicing denial and refusal, the lesson they ll learn is that old age is to be bitterly fought, rather than adapted to and embraced. INTERVIEWING THE AGENCIES To get all that you deserve and are expecting in a home care agency, make sure you ask the right questions. Below is a list of questions to ask, as well as a chart for you to use to compare the different companies you speak with during your search. COMPANY #2 COMPANY #3 Do you provide insurance for accidents, if they occur, in the home? Are you licensed for medical care and non-medical (companion) care? Do you have an RN to conduct an initial assessment before starting each engagement and to follow up with supervisory visits to ensure the plan of care is being carried out to your satisfaction? Are your caregivers trained and competency tested? Are all employees drug tested, background checked, licensed, bonded and insured? Do you take care of paying the employees, including taxes and withholding? Will you send another worker of equal skill and experience levels if my caregiver has an emergency? Can I interview the employee(s) before I commit to using your agency? * Does your agency have a Guaranteed Compatibility system in place if there is a conflict of personalities between me or my loved one and the caregiver? Can you accommodate any length of visit request, even as short as 1 hour per day? What are your rates? Can I get someone who has been trained in a particular condition?
