Purpose of the role : To develop and provide a responsive service to people referred for primary mental health care with mild to complex issues To be pivotal in the ongoing development of an integrated service between primary and secondary mental health care To coordinate and support all primary care liaison across the CY&F MHS portfolio KEY TASKS To develop key stakeholder relationships that will assist with the development of appropriate referral pathways and provide leadership in regards to implementation. To provide Clinical Leadership in relation to the Primary Care Liaison role Service Improvement Projects: Undertake key service & improvement projects as identified as part of Primary Care Liaison function EXPECTED OUTCOMES Identify and consult with primary care stakeholder groups To ensure that clear pathways are developed and implemented in partnership with the Schools/ NGO/ PHO/ GP Practices and DHB CYF MH services Provide coaching and skill development of DHB staff in respect of optimal relationships with primary care staff Work with consumers to ensure that pathways are appropriate in regards to a timely response to identified consumer need Provide a clinical leadership role as part of best practice initiatives/service development Actively promote clinician involvement in best practice initiatives Actively promote clinical input into policy and procedure development Provide leadership in supporting a culture of evaluation and the ongoing quality improvement of best practice Provide leadership in working with service users with a range of health needs Provide senior leadership into the governance group and the wider service and, as appropriate, consultation to a broad range of health welfare and other community organisations Initiate/participate in and evaluation activities as appropriate. Improvement Projects are consistently implemented in line with Clinical Governance strategic direction, values and goals. Robust project management and process improvement methodologies are used and clear work plans, deliverables and measures. PERSON SPECIFICATION POSITION TITLE: Minimum Qualification A recognized allied health or nursing qualification e.g. social work/ Mental Health nursing/ OT Full professional membership Preferred Additional qualifications in project management or health management Experience Minimum of 4 years working in child, youth and family mental health setting and/or community based services/ PHO s Previous project management experience Experience of working across multi-service organisations Experience of working with Maori, Pacific and other cultures Skills/Knowledge/Behaviour Ability to establish and maintain effective relationships between DHB staff and clinicians, GP practices and community agencies in primary care. Demonstrates ability to work sensitively with all cultures including ethnicity, age, disability, sexuality, gender as outlined in WDHB Mission Statement. Sound professional knowledge base.
