A tertiary qualification in Exercise Physiology and accreditation as an Exercise Physiologist with ESSA 3. Demonstrated experience in providing Exercise Physiology Services in a Rehabilitation and Aged Care setting 4. Minimum one year of post graduate experience in Exercise Physiology 5. Demonstrated experience in providing aquatic based exercises and programs in a Rehabilitation and Aged Care setting 6. A current drivers licence, safe driving record and access to own vehicle 7. 2. Context and Scope HammondCare is an independent Christian charity that exists to improve quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence based best practice in its services and we provide these services through home care, subacute hospitals and residential aged care. Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances. Summary of the Role The role of the Exercise Physiologist is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Exercise Physiology service is provided to clients through HammondCare s Centre for Positive Ageing, along with appropriate support and advice to their carers. The Exercise Physiologist will participate as a member of the multi-disciplinary team to ensure a client centred, evidence based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and Exercise and Sports Science Australia. The essence of this role: Participate as a member of the HammondCare team in the application of the organisation s Mission and values to the pursuit of its objectives in a client centred, continuous quality improvement work environment Provision of effective and efficient Exercise Physiology service to clients and appropriate support and advice to their family, carers and other HammondCare staff Assist in the development and delivery of short term restorative care programs for clients in accordance with HammondCare requirements and the Australian Government Department of Health Short-Term Restorative Care Programme Participate as a member of the multi-disciplinary team in assessing clients level of functional and care needs, setting care goals and providing appropriate treatment 3. Responsibilities Capability Outcomes communication with medical, nursing, allied health and other staff and voluntary workers Document client s progress as required in accordance with industry guidelines Before using equipment in client care, check contra-indications, conduct necessary tests and issue appropriate warnings Assess the standard of interventions by utilising outcome measures Respect and maintain clients privacy Teach carers and co-workers the skills they need to assist clients Provide individual and group based exercise therapy to HammondCare clients in both land and aquatic based settings at HammondCare s Hammondville and Miranda sites Provide exercise physiology services through HammondCare s Restorative Care Service within the Sydney South East and Sydney South West regions. Deliver education and exercise sessions, and health promotion initiatives to HammondCare staff and clients to support effective care provision Has effective working relationships with medical, nursing, allied health and other staff and voluntary workers Actively participates in multidisciplinary team meetings for designated caseload Completes documentation of comprehensive Exercise Physiology assessments, program and interventions in client medical files in accordance with HammondCare and department policies Outcomes of therapy are evaluated and documented with relevant handover given to other services as necessary Enhanced client care through implementation of exercise physiology programs 2.0 Operational Management Activities Participate in development and promotion of Exercise Physiology services Actively participate in the implementation of an effective and appropriate quality Exercise Physiology service Participate in regular evaluation of Exercise Physiology practice Exhibit a commitment to quality service and participate in activities to enhance continuous quality improvement Attend and contribute to team, case, department and other meetings as needed Read and understand the Policy and Procedures Manual(s) for the Department and HammondCare, and work within the principles contained therein Behaviour and performance are consistent with professional expectations, HammondCare s MIA and Capability Framework Attends and participates in relevant meetings Undertakes department support and development activities as directed by supervisor or Manager and completes in agreed time frames
