EFMP Enrollment EFMP provides assignment considerations for sponsors with family members identified as having special conditions Department of Defense Instruction (DoDI) 1315.19, The Exceptional Family Member Program, Section 3, dated 19 Apr 2017, outlines conditions meeting criteria Medical Needs Mental Health Needs Educational Needs Adaptive Service Needs Enrollment is mandatory for family members who meet criteria 11

Special Needs Coordinator Each Air Force Installation has a Special Needs Coordinator (SNC) position, located within the MTF The SNC at each Installation is responsible for: Identification of EFMP criteria Enrollment of new EFMP cases Management of existing EFMP cases Oversight of EFMP clearance process during PCS Review of EFMP documents for EFMP Reassignment requests The SNC notifies the MPS when special needs are identified by requesting an Assignment Limitation Code-Q (Q-code) on the member s personnel file The SNC works with the EFMP-Family Support office to refer families in need of supportive services 14

Expedited Locations Qualifiers: Expedited FMRC process is used when a sponsor has an assignment to one of the following locations: Hill AFB; JB Anacostia-Bolling (includes the Pentagon); JB Andrews; JB Langley-Eustis; JBSA-Lackland (includes Fort Sam Houston & Camp Bullis); JBSA- Randolph; Los Angeles AFB; Luke AFB; MacDill AFB; Wright-Patterson AFB Must have a RIP with a PAS code to one of the above Disqualifiers: Assignment to a GSU of any above listed locations Assignment is for training with an operational follow-on Family member requires transplant services 15

Expedited EFMP Clearance Members with a projected assignment to the Expedited bases may participate in an abbreviated EFMP clearance Family will not be asked for the following: Civilian medical records Medical/Educational clearance forms Face-to-face interview All family members are locally cleared at the losing base 3 day turn-around for the sponsor (AF1466 completion) Enrollment and Clearance are not the same If criteria are discovered at the time of clearance, a separate enrollment process will take place Does not effect clearance times 16

EFMP Assignments Policy EFMP enrollment ( Q code) currently not considered during formal assignment selection process for Airmen Airmen are selected in their own right, based on eligibility and qualification factors, ensuring a fair and equitable assignment selection process and guaranteeing career development opportunities are not limited The Family Member Relocation Clearance (FMRC) process (EFMP-Medical) occurs after formal assignment selection to ensure the gaining accompanied location is able to provide required specialty services, at the frequencies required, through the Military Treatment Facility (MTF), TRICARE support network, or a combination of the two EFMP Where Mission and Compassion Meet

Eligibility for Reassignment or Deferment EFMP Eligibility Criteria (at least one criterion must be met): Care unavailable at current or projected assignment location (reassignment or deferment) Newly diagnosed condition identified & Airman s presence considered essential (deferment) Family member receives negative travel recommendation to projected assignment location (diversion or cancellation) EFMP Where Mission and Compassion Meet

Reassignment / Deferment Request Processing Processing time is an estimated 4-6 weeks for requests to process in their entirety Airmen can contact TFSC for status update Commanders and First Sergeants can contact DP2LWA Potential bases WILL NOT be released to member or leadership; leads to processing delays and false sense of direction to family members New requirement is all EFMP-enrolled Airmen MUST initiate Family Member Relocation Clearance (FMRC) process with SNC within 30 days of assignment notification DD Form 2972 & DD Form 2792-1 are valid for 12 months AF Form 1466 valid for 6 months Administrative action may be taken for failure to comply EFMP Where Mission and Compassion Meet
