Sustainability NorthWestern Mental Health (NWMH) provides a comprehensive, integrated range of services to people with a mental illness in North-Western Melbourne. A multi-disciplinary workforce provides services through four Area Adult Mental Health Services (Mid-West, Inner West, North West and Northern), an Aged Persons Mental Health Program and Orygen Youth Health. NWMH is a clinical Division of Melbourne Health, and operates in partnership with two other metropolitan health services; Northern Health and Western Health. Orygen Youth Health (OYH) provides specialist mental health services for young people aged between 15 and 25 years of age who reside in the western and northwestern regions of Melbourne. Clinical services are provided to young people with mental health disorders within the community, and within an inpatient setting for episodes of acute disorder. The clinical program offers specialist interventions for a range of disorders including psychosis and mood disorders, co-morbid substance use and personality disorders. Services include crisis intervention and assessment, inpatient care, individual case management, group and family therapy interventions, intensive outreach services, community consultation and education. OYH services a catchment area of more than one million people and provides treatment to approximately 900 young people each year. Position Summary for Clinical Case Manager, HYPE, Region A/B The Helping Young People Early (HYPE) program was established in 1999 and provides an evidence-based early intervention service for borderline personality disorder (BPD) in youth. The program offers treatment to young people diagnosed with 3 or more DSM-V BPD criteria, with no specific exclusions for co-occurring psychopathology. Treatment comprises assertive case management, general psychiatric care, and up to 16 sessions of individual psychotherapy using cognitive analytic therapy. This role provides specialist mental health treatment to young people aged 15-24 who are eligible for treatment in the HYPE program. The HYPE clinical case manager/therapist works within a multidisciplinary team environment and is responsible for the provision of the HYPE intervention, which includes mental health assessment, assertive case management and treatment (including outreach work), individual psychotherapy (using a cognitive analytic therapy framework), development of an individual service plan with the client and delivery of individual, family and systemic treatment interventions. The clinical case manager/therapist is responsible for management of a clinical caseload and consultation and liaison with other services relevant to the client. Co-provision of secondary consultation to services external to Orygen Youth Health is also part of this role. Applicants are expected to have experience in the provision of mental health services to young people. Applicants who do not have experience and training in cognitive analytic therapy will receive training and supervision in this role. Duties: Work within an integrated service delivery model which incorporates case management and the delivery of treatments (including cognitive analytic therapy), in accordance with service and/or research protocols;

Carry out effective biopsychosocial assessment, mental state examination, crisis and risk assessments and develop, implement and evaluate comprehensive individual service plans for each client and their family/support; Deliver clinical interventions to young people with full- or sub-syndromal borderline personality disorder and other severe mental health problems; Provide effective assertive case management to young people with borderline personality disorder and other mental health problems, their families/carers/support workers/significant others; Liaise with other psychiatric, welfare and educational service providers to ensure continuity of care for clients; To provide duty cover, crisis response and community treatment to current clients of OYH; Maintain appropriate case notes, records and statistics; Participate in the planning, development and implementation of specialist services; Other duties as directed that may arise. To reflect the ongoing commitment of OYH to improve service accessibility, the service operates over a number of sites. Staff may be required to work from any of the OYH sites, which at this stage include Parkville, Footscray, Sunshine, Melton, and Wyndham. Staff will also be required to outreach to other community settings such as clients homes. Key Accountabilities Manage and undertake mental health and risk assessment, psychological treatment, crisis intervention, family work and systems containment, for the client/carers/system Provide high-quality clinical care based on clinical experience working with young people with serious mental illness Ensure high standards of clinical care that are congruent with OYH aims client focused, accessible, responsive and consistent with best practice Capacity to provide, or develop skills in, integrated assessment, treatment and care for dual diagnosis clients Timely completion of mental health assessments including formulation, individual service plans, crisis management plans, outcome measures, documentation of client s progress in client files, discharge summaries and contact sheets Provision of case management or family work to selected clients and associated documentation Attendance at clinical review meetings Manage clinical risk and actively work toward implementing risk reduction strategies Comply with Safety & Service Improvement / Quality Management policies and procedures Support and participate in the Health Services Occupational Health & Safety (OHS) Policies and Procedures and implement (where appropriate) at unit/service area level including reporting any incidents of unsafe work practices, sites/areas or equipment Establish and maintain effective interfaces and partnerships with key agencies and community groups through consultation, liaison, education and collaborative projects: including Drug and Alcohol services, Youth Justice and primary care providers etc. Participate in ongoing service improvement initiatives and activities. Key Selection Criteria Formal Qualifications Registered Nurse: Qualification: Bachelor Degree in Psychiatric/Mental Health Nursing or equivalent OR Bachelor Degree in Nursing or equivalent plus an approved postgraduate qualification in Psychiatric/Mental Health Nursing Registration: as a Registered Nurse with the Nursing and Midwifery Board of Australia Enrolled Nurse: Qualification: Certificate IV in Health (Nursing) or Diploma in Nursing.
