We understand that families reach a point when they need to explore other care options for their medically complex children. They seek a place that offers state-of-the-art medical treatment, educational opportunities, person-centered living, and community involvement. That place is Seven Hills Pediatric Center. Seven Hills Pediatric Center Making An Affiliate the of Right the Choice Seven Hills for Your Foundation Child

Seven Hills Pediatric Center Seven Hills Pediatric Center (SHPC) is a pediatric skilled nursing community located on six wooded acres in beautiful Groton, Massachusetts. The five-star facility provides long- and shortterm care to children and young adults, from infancy to age 22, who are profoundly developmentally delayed and have complex medical needs, or who need transitional care after an extended hospitalization. Our children and young adults come to us in many ways as a result of complications during labor and delivery, accidents, and trauma, among others. All of these children have something in common: families who love them and want them to have the very best of care. Our medical, educational, and community-based supports all revolve around our person-centered living philosophy that it is a privilege to care for each and every child entrusted to us, and we celebrate their individuality in the pursuit of personal happiness, comfort, interests and potential. SHPC understands that placing a child in a long-term care facility is very difficult. Families often make the decision to do so when 24-hour care in the home becomes too great and requires sophisticated, skilled nursing. Seven Hills Pediatric Center can help make that decision easier. We are here to offer your child a home one that welcomes you and your family as members of our family in a community that offers experiences you never thought possible. HOME A Place Your Child Can Comfortably Call

Care Options Long-Term Care Children enrolled in our long-term care program receive all the necessary medical, nursing, therapy, and leisure services to enhance their quality of life. Cognitively, residents are under the age of 12 months and non-ambulatory. All of the children attend a private, special education school, go on field trips and participate in community activities. Short-Term Respite Care Families caring for medically fragile children face challenges such as unforeseen hospitalizations, trips, family activities and other events, which make a short-term stay at a facility necessary. Seven Hills Pediatric Center can provide that care. The staff is trained on the newest technologies and will provide tracheostomy care, gastrointestinal feeding, or IV therapies as well as care for children who are ventilator-dependent. Short-Term Post-Hospitalization Short-term care is available for children, regardless of cognitive level, who require 24-hour medical support and rehabilitation after a hospitalization. Our nursing staff provides hands-on training in wound dressing, intravenous therapy, gastrointestinal tube feeding, medication management and the proper usage of advanced technical equipment to families who need to administer treatments at home. An Individual Transition Plan is developed with each family to move the child from hospital to home as quickly and comfortably as possible. Admissions decisions for all SHPC care options are approved through the Medical Review Team (MRT). Families are welcome any time! Seven Hills Pediatric Center is open 24 hours a day for family visits. Each room is personalized for each child, with photos and favorite toys. Equipment is discreetly out of view. Suction, oxygen, and ventilators are accessible at the bedside cabinet. State-of-the-Art Medical Treatment is Combined with Compassionate Care Centered Around YOUR Child.

Leading Medical Treatment The Seven Hills Pediatric Center s medical staff, from the physicians to the nurses and aides, have one goal to provide the absolute highest level of care to our residents. Many of our residents require oxygen, feeding tubes, tracheostomies, and other complex medical equipment. We work diligently with our partner, Children s Hospital Boston, to ensure they receive the most advanced care available.

In the Community The children and young adults at Seven Hills Pediatric Center live full, active lives, and have become an integral part of the community. They attend sporting events, concerts, go shopping, and participate in local parades and celebrations. If needed, our children travel with their medical equipment (oxygen, pulse oximetry) and skilled nursing staff. Residents over the age of 22 also have the option of attending day habilitation programs outside of Seven Hills Pediatric Center to enjoy community experiences. Our residents develop friendships within the community, and community members look forward to their visits. The Children Enjoy an Active Schedule Filled with Outings, Celebrations, and Community Events.

Seven Hills Academy at Groton The Seven Hills Academy at Groton is committed to meeting the developmental learning needs of the students and strives to help each child attain his or her maximum potential. To achieve this goal, the school provides educational, habilitative, and health services that are based on each child s needs. The philosophy of the academy is that students can grow, develop, learn, and contribute to their community.

Individualized Attention Working together with staff, families, local public schools, and the community at large, the academy develops each student s communication, social, and self-help skills. Assistive technology, adaptive devices, and specifically designed educational materials support the needs of each child. Our interdisciplinary staff provides ongoing and diverse multisensory experiences embedded in all school activities. Assistive technology as well as wheelchairs, braces, and hand splints are all customized to fit as the children grow. Seven Hills Academy at Groton is approved by the Department of Elementary and Secondary Education (DESE) as a private, special education school for students ages 3-22. Services are based on the goals and objectives noted in each student s Individualized Education Program (IEP). Students receive occupational and physical therapy in a state-of-the-art setting. Speech Therapists provide support, and school nurses are available at all times for any emergent health issues. Unique Learning Needs are Met within our Multi-Disciplinary Learning Environment.
