Introduction of Offerors Strategic Partnership: Large networks of clinically integrated physicians Chinese Community Accountable Care Organization 4/1/2012 - Approved as one of the first ACO approved under the Medicare Shared Savings Program 1 of 7 ACOs nationally to receive interim payment of shared savings (out of 114 ACOs approved in 2012) > 12,000 assigned Medicare Beneficiaries Eastern Chinese American Physician IPA Established in 2004 Over 210 physician members 2

Introduction of Offerors CCACO and ECAP Membership Profile 50% PCPs to 50% Specialists Specialty Services Include but not limited to: Cardiology, Oncology/Radiation-Oncology, OB/GYN, Ophthalmology GI, Endocrinology, Rheumatology, ENT, Surgery, Podiatry, etc. CCACO/ECAP physicians maintain independent hospital affiliations/credentials include: LMC, Downstate, LICH, NYU, Maimonides Medical Center, BI, Mount Sinai, Columbia Presbyterian. Commitment to Quality (NCQA Recognition, Meaningful Use, PCMH0 Diabetes Recognition Program Nearly 70 physicians achieved NCQA Diabetes Recognition Program by via a grant funded by the New York State Health Foundation Patient Centered Medical Home (PMCH) Certification 30 practices are PCMH certified, Level 1 - Level 3 Additional 10 in process of obtaining certification 3

II. Sustainability Plan for a Viable LICH: Hospital Operational Vision 4

LICH Sustainability Plan Hospital Operational Vision Establish a not for profit Article 28 hospital with local community physician leadership ( debt free hospital facility) Provide high quality, low cost care to Brooklyn s residents via an efficiently operated, right-sized community hospital. Will consult with stakeholders to provide full service community hospital. 5

SUNY RFP Long Island College Hospital Meeting the 5 Key Objectives 1. Assist Downstate in exiting the provision of clinical services at the LICH campus. 2. Operation of a health care facility or health services on all or part of the LICH campus, consistent with the health care needs of the community. 3. Continuation of an academic affiliation with Downstate 4. Sale of the Property at no less than the current market values and satisfaction of liabilities attendant to such Property. A proposal to purchase less than all the Property will be considered provided it meets the objective of satisfying the liabilities attendant to all the Property. 5. Encourage collaborations among interested parties that will make use of the entire LICH campus to benefit the community, including but not limited to the provision of health care services CCACO/ECAP LICH Vision = 100% Alignment with 5 Key RFP Objectives 13
