2. Context and Scope HammondCare is an independent Christian charity specialising in palliative care, dementia care, restorative care, and specialist mental health for older persons. The organisation provides these services through sub-acute hospitals, residential care and innovative community services in NSW. We are an innovative organisation, well regarded within the health and aged care sectors, especially in regard to the quality of our care services and our external training services. HammondCare works with its staff to ensure they have support, opportunities, training, responsibility and reward, good work/life balance and satisfying and creative roles. The HammondCare Consortium comprising HammondCare, St Vincent s & Mater Health Service, and Calvary Health Care Sydney Ltd, in collaboration with participating Local Health Districts (LHDs) and specialist palliative care services, proposes to address the end-of-life care gap with three strategies: Supportive Palliative Care Packages, State-Wide Education and Evaluation. The HammondCare Consortium Community Palliative Care service provides a flexible, rapid response care delivery service in the area of Palliative Care providing skilled and experienced care workers through brokerage arrangements to internal and external service providers. The HammondAtHome (HAH) Palliative Care Home Support Program will hold and administer the Palliative Care Packages centrally. The Program and Package Coordinator of the Palliative Care Home Support Program assists in the coordination of the administration of the Palliative Care Service and assists in the coordination of packages of care, working within the philosophy of care and Mission in Action of HammondCare to ensure that the brokerage services are delivered in a style that aims to maximise client satisfaction and quality of life, and meets the needs of the designated client group. The service also needs to be carried out in a manner that meets the requirements of the service provider, and is consistent with relevant internal or external policies and procedures, funding guidelines and standards, and all relevant government legislative requirements. 2. Key responsibilities 2.1 Assist in the provision of strong and effective administrative oversight and coordination of the Palliative Care Home Support Program operations. Office administration responsibilities, including the ordering & maintenance of supplies. Assist the Palliative Care Home Support Program with phone support, being a point of contact for enquiries from the Local Health Districts and for referrals. Administration and maintenance of the package database, detailing overall package availability per participating LHD, utilisation, number of packages available, total hours of booked service delivery per client and any residual hours available. Preparation of regular and ad hoc reports, detailing availability and utilisation of packages, by LHD. Assist with the organisation of Steering Committee meetings and venues. Assist in the preparation of agendas and meeting minutes. Assist in the processing of invoices as per policy, and completion of expense and credit card processes. Assist with organisation of travel and accommodation. Assist with the development and preparation of forms and documents. Assist in the preparation of reports and the collation of work activity. 2.2 Tasks to support recruitment and the delivery of training including: Coordination of recruitment paperwork and processes.
Liaison with HammondCare s CTC for organisation of Care Worker Education. Liaison with HammondCare s CTC for organisation of health professionals education. Receive and process course enrolments. Receive and answer enquiries regarding future courses. Book and set up training venues for programs. Organise catering. Prepare training materials and distribute as directed. Liaise with relevant staff to ensure the smooth roll out of each training session. 2.3 Tasks to support allocation of palliative care packages including: Coordination of referrals and referral processes. Liaison with LHDs and Care Worker agencies for organisation of care delivery. Receipt and processing of package requests. Receive and respond to enquiries regarding package availability. Preparation of reports for participating LHDs and for the Program Evaluation. Liaise with all relevant staff to ensure the smooth rollout of each package of care. 2.4 Data and ICT support Recording of all equipment and addressing any operational issues with the ICT team. Competence in the set up and use of equipment, ensuring adherence to relevant protocols. Data collection and database management. Competence in the set up and use of various software programs, including Microsoft Excel. Please note that responsibilities may vary at the discretion of the Head of HammondAtHome Northern Sydney. This is particularly important for a developing service with emerging needs. The employee will be notified of changes in their responsibilities; however, they will not be required to sign an updated job description unless their role is significantly revised. 3.
