The applicant must submit a short essay demonstrating his/her service to the Kings Park Community along with the completed application form. The applicant must submit a short essay describing how the events of September 11, 2001 impacted their life, the life of a family member and/or their community. The applicant must submit a short essay describing a circumstance when you resisted peer pressure in order to avoid a potentially dangerous situation. Alfina Colarusso Scholarship Application #8 Award: $500 Awarded to a graduating Kings Park High School applicant who has been working up to potential and is planning to further his/her education in the liberal arts. Cousins Courage Memorial Scholarship Application #9 Award: 2 @ $750 each Awarded to a graduating Kings Park High School applicant who plans on furthering his/her education and who demonstrates selfless dedication to public/community service. Kaleigh Fallon Memorial Scholarship Application #10 Awards: 2 @ $1,500 each Awarded to two graduating Kings Park High School applicants who plan on furthering their education and have overcome either academic or family hardships. Applicant must have been involved in extra-curricular activities during their four years in the high school. Financial need is strongly considered. Fort Salonga Association Scholarship Award Application #12 Award: $1,000 A brief essay must be submitted, along with the application, and a copy of your KPHS transcript. The essay must describe yourself, your interests and a summary of your educational and career goals. This scholarship is open to college bound Fort Salonga residents whose parents are dues paying members in the Fort Salonga Association. Please include a record of your community service and/or volunteerism. Fort Salonga Association - Market IGA Richard G. Costello Memorial Scholarship Application #13 Award: $1,000 A brief essay must be submitted, along with the application, and a copy of your school transcript. The essay must describe yourself, your interests and a summary of your educational and career goals. This scholarship is open to all Kings Park High School students that are college bound. Fort Salonga Association - Charles and Helen Reichert Family Foundation Scholarship Application # 14 Award: $1,000 A brief essay must be submitted, along with the application, and a copy of your school transcript. The essay must describe yourself, your interests and a summary of your educational and career goals. This scholarship is open to all Kings Park High School students that are college bound. Also, please include a record of your community service and/or volunteerism. Financial need is strongly considered. Fort Salonga Elementary School PTA Alumni Scholarship Application #15 Awards: 2 @ $500 each Awarded to a former student of Fort Salonga Elementary School. Please submit an essay describing (1) a situation of adversity you ve successfully dealt with, OR (2) an obstacle you ve overcome. Explain the character values you possess which you feel are significant in achieving this, as well as future success. Also include a brief summary of your background, educational plans and hopes for the future. Attach the essay and résumé (if desired) to the application form. Michael & Michele Hanley Scholarship Application #16 Award: $500 Awarded to a graduating senior planning to major in business at an accredited two or four year college. The student must have maintained an average of 85% or better. Extra consideration is given to applicants with work experience, participation in sports, and/or a love for numbers. Edward P. Hogan Memorial Scholarship No Application Award: $500 Awarded to a graduating Kings Park High School senior who is a community service minded, average student, planning on a political science or social services oriented career. Applicant must submit a five hundred word essay on why they have chosen to pursue a career in political science or social services. KPHS Scholarship Packet 1-16-18-lh 4

Madeline C. Jones Memorial Scholarship Application #17 Award: $500 Awarded to a graduating KPHS senior who has been accepted to an accredited two or four year college/technical/business school in the fall. The student must be considered respectful, friendly & responsible by his teachers & peers, and able to provide supporting endorsement to that effect. Eligibility is limited to a child of a KPCSD employee. Kings Park Building Administrators Association (BAA) Scholarship Application #18 (Brief Essay) Award: $500 Awarded to a graduating KPHS senior who has been a student in the Kings Park Central School District since Kindergarten. The applicant must submit a brief essay (250-500 words) describing themselves and a leadership role they have held within the school or in the community. Extra consideration will be given to a child of a KP BAA member and/or KPCSD employee. Kings Park Chamber of Commerce Scholarship Application #19 Awards: 2 @ $1,000 each Awarded to an applicant who is entering an approved, four year college, based on academics and financial need. Your transcript and a brief essay must be included with the application. The essay must indicate your chosen field of study and an explanation of what events led you to this decision. Kings Park Civic Association -- The Greg Szurnicki Memorial Scholarship Application #20 Award: $500 Awarded to an applicant who has maintained an 85% average with a good attendance record while attending Kings Park High School. Preference given to students involved with school and community service and other extracurricular activities. Kings Park Classroom Teachers Association George L. Coupian & Nicholas Loukides Memorial Scholarships Application #21 Awards: $250/year for 4 years Awarded to an applicant who is entering an approved, four year college, based on academics and financial need. Kings Park Council of Schools Scholarship Application #22 Award: $500 Applicant must have attended Kings Park High School. The applicant must be planning on attending a school of higher education. Kings Park Educational Unit CSEA Scholarship Application #23 Award: $500 Awarded to a graduating Kings Park High School applicant who has been accepted to a two year or four year college/nursing/technical school. Kings Park Fire Department Ladies Auxiliary Scholarship Application #24 Award: $1,000 Awarded to a graduating Kings Park High School applicant who will be attending a two or four year college/nursing/technical school, demonstrates financial need and has been a resident of the fire district for three years. Kings Park Fire Department Edward Mankuski Memorial Scholarship Application #25 Award: $2,000 Awarded to a graduating Kings Park High School applicant who will be attending a two or four year college/nursing/technical school, demonstrates financial need and has been a resident of the fire district for three years. Kings Park Fire Department Warren Memorial Scholarship Application #26 Award: $1,000 Awarded to a graduating Kings Park High School applicant who will be attending a two or four year college, demonstrates good writing skills and has been a resident of the fire district for three years. Kings Park High School Music Boosters Scholarship Application #27 Award: $TBD Awarded to graduating seniors who have been involved in the Kings Park High School Music Program. Kings Park High School PFA Scholarship Application #28 Award: $500 Awarded to a four year college bound applicant who has a cumulative, weighted average of 80 percent or more and who has participated in extra-curricular activities (clubs, organizations, school team/non-school team sports, employment) and/or community service activities (Boys Scout/ Girl Scout projects, religious volunteer, charitable organization volunteer, etc.). Applicant s family must have paid membership to Kings Park High School Parent Faculty Association for 2017/2018 school year. Kings Park Homemakers Award No Application Award: $250 (Letter) Awarded to a graduating Kings Park High School student. Applicant must submit a one page letter, stating goals and ambitions. Financial need is also a consideration when recipient is being selected. Please include your name, address and phone number. Kings Park Retired Teachers Association Scholarship Application #29 Awards: 2 @ $1,000 each Applicant must be a graduating Kings Park High School student planning on pursuing his/her post-secondary goals in college or vocational school. Applicant must submit completed application form, a copy of current KPHS transcript, letter of recommendation from teacher or Guidance Counselor, and a 250 word essay. Kings Park Saint Patrick s Day Parade Committee Scholarship Application #30 Award: $TBD Awarded to a Kings Park High School graduating senior of Irish Descent. Extra consideration is given to applicants with a resume showing extra-curricular activities in school or community services and/or Irish cultural activities. KPHS Scholarship Packet 1-16-18-lh 7

Kings Park Soccer Club Scholarship Application #31 Awards: 2 @ $1,000 each Awarded to a dedicated male or female soccer player who is currently playing or who has played soccer, coached, refereed or helped the Kings Park Soccer Club in any capacity. They must plan on attending a school of higher education. Candidates should submit in their qualifications list: the capacities in which they helped the Kings Park Soccer Club; any community or social work that they have been involved in; information regarding the candidate s school achievements; sports and scholastics. No late requests will be considered. Kings Park Soccer Club Rudy Mastrocinque Scholarship for Community Service Application #32 Award: $2,000 The criteria for consideration are as follows: 1. This scholarship is to be applied for by recommendation from a non-family member. All recommendations must be received in writing and should explain why the student deserves this award and lists any other qualifications that relate to the criteria below. In addition, the person submitting a recommendation should include their name, address and phone number as well as contact information for the student. 2 The student must be planning to attend a school of higher education. 3. This scholarship is to award the student s ongoing community services and/or an act of community service that distinguishes him or her from the rest of the student body. 4. The student should show involvement in a variety of extra-curricular activities; such as athletics, school clubs, music, community clubs and/or other activities. 5. The student, at some time, must have been involved with soccer either with a club or school team in Kings Park. 6. No late requests will be considered. Kings Park Soccer Club Jim Spence Leadership Award Application #33 Award: $500 Awarded to a dedicated male or female soccer player who is currently playing or who has played soccer, coached, refereed or helped the Kings Park Soccer Club in any capacity. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Applicant must also submit proof that he has volunteered directly for KPY. Kings Park Youth Sean Weisse Memorial Boys Basketball Scholarship Application #35 Award: $500 Awarded to KPHS graduating male applicants who participated in KPY basketball and have played basketball at the high school level. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Applicant must also submit proof that he has volunteered directly for KPY. Kings Park Youth Cheerleading Scholarship Application #36 Award: $500 Awarded to KPHS graduating applicants who participated in KPY cheerleading and have participated in cheerleading at the high school level. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Applicant must also submit proof that she has volunteered directly for KPY. Kings Park Youth Joann Ryan Football Scholarship Application #37 Award: $500 Awarded to KPHS graduating applicants who participated in KPY football and have played football at the high school level. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Kings Park Youth Sean Weisse Memorial Girls Basketball Scholarship Application #38 Award: $500 Awarded to KPHS graduating female applicants who participated in KPY basketball and have played basketball at the high school level. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Applicant must also submit proof that she has volunteered directly for KPY. Kings Park Youth Softball Scholarship Application #39 Award: $500 Awarded to KPHS graduating applicants who participated in KPY softball and have played softball at the high school level. A 150 word essay, describing a KPY coach and a specific KPY event that is most important to the applicant, should be submitted with the application. Applicant must also submit proof that she has volunteered directly for KPY. Knights of Columbus Peter Brust Memorial Scholarship Father Seyfried Council #821 Application #40 Award: $500 Awarded to applicant who resides in the Kings Park Central School District, St. Joseph s Parish or is the child or grandchild of a Council #821 member; is planning to attend a college or trade school; who has a minimum of an 80 GPA; and attaches a brief essay reflecting their involvement in the community. Sean Knott Memorial Scholarship Application #41 Award: $TBD Awarded to a graduating Kings Park High School applicant planning on attending a military academy in the fall following graduation. Katherine Martell Memorial Scholarship for Nursing offered by the Catholic Daughters of the Americas Court Ave Maria Application #42 Award: $1000 Applicant must be a Graduating Senior of Kings Park High School OR a child or grandchild of a CDA Court Ave Maria member in good standing OR a member in good standing of JCDA Court Ave Maria. The applicant must have been accepted and enrolled in an accredited Nursing School, & complete the application form along with their essay. Athletes with a college sport scholarship already awarded or students attending a military academy are not eligible. Financial need is strongly considered. T. Nally Memorial Scholarship for Community Service Application #44 Award: $250 Awarded to a graduating Kings Park High School applicant who has been working up to potential and is planning on furthering his/her education. Applicant must have been involved in extra-curricular activities and community service during their four years in high school. National P.T.A. Scholarship Award Application #45 Awards: 4 @ $500 each Applicant must be a Special Education student, who will be attending a two year or four year college or vocational school. Naval Academy Parents Club of Long Island - Kent-Sisler Memorial Scholarship Application #46 Awards: 2 @ $ 250 each Applicant must be a graduating KPHS senior who is attending a military academy or accepting a ROTC position at a university or college OR enlisting in an armed force service in the fall following graduation. He/she must have been involved in extracurricular school activities and/or community service. Along with application, please include a list of colleges or universities to which you have applied, two letters of recommendation from extra-curricular activities, a signed student activity sheet and a 250 word essay. Peter Nelson Memorial Scholarship Application #47 Award: $500 Applicant must plan on attending a four year college in the fall, have been on at least one athletic team in Kings Park High School for 2 or more years and have a minimum average of 80%. Jeffrey Nordt Memorial Scholarship Application #48 Awards: 2 @ $500 each or 1 @ $1,000 Applicant must be a graduating Kings Park High School student who has a B-B+ average (no honor students, please) and must be in financial need. Father John Papallo Lodge #2684 Order of Sons of Italy in America Scholarship Application #49 Award: $500 Mail application & supporting materials by APRIL 30, 2018 to: RON & JANETTE ROTH 81 CEDAR ROAD KINGS PARK, NY 11754 The applicant must be a graduating high school student, who is a child or grandchild of a current Lodge member. Applicants must have achieved an average of 85% or better (or a grade point average of 3.0) and must be planning to attend a full time program at a college or other post secondary program. A brief essay (approx. 250 words) must be submitted, along with the application. The essay must summarize the applicant s career aspirations, community contributions and how the student s Italian American heritage has been an influence in their life. The student must also submit an official transcript of their grades, including the first half of twelfth grade and two letters of recommendation from teachers, guidance counselors, or other school personnel. Rogers Middle School; who has a minimum grade point average of 80%; and who has been accepted to a two or four year college program. Please include transcript; attach two letters of character reference (academic work or outside affiliation, such as church, civic activities, etc.); attach a five hundred word essay on How a Teacher Can Make a Difference or What is an Educator? The essay should be on 8.5 x 11 paper, with applicant s name on each page. St. Joseph CYO Girls Basketball Scholarship Application #55 Award: $TBD Awarded to a graduating female senior who participated in St. Joseph CYO Girls Basketball and played basketball at the high school level. A 150 word essay describing your experiences and involvement in CYO basketball and what you have learned through CYO youth basketball. St. Joseph CYO Girls Volleyball Scholarship Application #56 Award: $TBD Awarded to a graduating female senior who participated in St. Joseph CYO Girls Volleyball and played volleyball at the high school level. A 150 word essay describing your experiences and involvement in CYO volleyball and what you have learned through CYO youth volleyball. San Remo Civic Association Scholarship Application #57 Award: $700 Essay Recipient must be a current resident of San Remo and must be a child, grandchild or legal ward of current member of the San Remo Civic Association (the current member must be in good standing for two consecutive years prior to application for the scholarship). The applicant must be a graduating senior planning on furthering their education in a given profession or trade. Other requirements include participation in community services and maintaining a grade point average of 90 or above. Applicant must include a brief essay describing their service to the school and community and educational plans for the future, along with their application. Victor C. Simoes Scholarship for the Future Application #59 Award: $150 Essay Open to graduating KPHS students. Submit a one page essay explaining your goals for college. Wendy Kathleen Smith Memorial Scholarship Award Application #60 Award: $1,000 Essay Applicants must be a college bound Kings Park High School senior with a C average or better. Transcript must be included with application, as well as an essay, telling about yourself and your aspirations and goals for life. Art Snelders Memorial Scholarship Application #61 Award: $500 Awarded to a graduating Kings Park High School applicant who demonstrates integrity, commitment and respect and is planning on furthering his/her education. Michael & Christine Hall Scholarship Application #65 Award: 2 @ $250 each Awarded to a graduating Kings Park High School student who has been accepted to an accredited two or four year college in the fall. Transcript and essay must be included with the application. The essay must contain your chosen field of study with an explanation as to how you arrived at this decision. It must also include one way in which you have given back to the community. Involvement with school and community service is tremendously important and will be heavily considered.
