Reminder About Changes in Designated Alternates Reminder: For Medical Staff Issues Involving Patient Care Call The Performance Improvement Hotline KMCWC Medical Staff Services Centralized Credentialing Verification Office Department Meetings Welcome New Members and Allied Health Professionals Informed Consent Form: One Form for All HPH Facilities The Medical Executive Committee has approved a new Informed Consent document that includes the following enhancements: This standardized form is currently being utilized throughout Hawaii Pacific Health hospitals. A witness signature is no longer required, and the witness signature line has been removed. This has been approved by legal counsel and meets all regulatory and legal requirements. Print is larger so it is easier for our patients to read. The templating of this form will eventually ease the transition to an e-signature process, mostly eliminating the need for hard copy consent documents, and will be available in Epic at that time. Please be aware that The Joint Commission and Centers for Medicare and Medicaid

Services (CMS) Conditions of Participation (CoP) still require that both the physician and patient sign, date and time this document when executed. A fillable PDF version of the form is now available on the Kapiolani Medical Center for Women & Children intranet site under Medical Staff Services, Informed Consent, and throughout the hospital at your request. Additionally, we will be able to work with you to customize the content of the document specific to your procedures. Construction Update Pacific Business News recently took an inside tour of Kapiolani Medical Center for Women & Children s new building, which will be home to an expanded neonatal intensive care unit, a pediatric intensive care unit, an adult medical surgical unit, an outpatient clinic, a rehabilitation services department, as well as an employee fitness center and conference rooms. HPH Hospital Emergency Codes A new safety code was introduced this year Code White: Active Shooter. This applies to situations in which there is an immediate threat to life (whether by gun or other lethal weapon) when an individual(s) is actively trying to kill people in a confined space. The code was established to protect medical staff, employees, patients and visitors. Prevention is possible. By being observant and working together, it may be possible to prevent or mitigate an intended incident. Medical staff and employees are our eyes and ears, located throughout our facilities, so if you see something suspicious, report it immediately! Oftentimes attackers will scout out an area beforehand, so be sure to always report suspicious or concerning behavior. Knowing what to do is important. We need to be prepared and know what to do in the event of a Code White incident. To review the new Code White safety code, or to review all other HPH hospital emergency codes, click here. Reminder About Changes in Designated Alternates Please notify the Medical Staff Services Office at 983-8564 as soon as possible for changes to your designated alternates. The Medical Staff Rules and Regulations requires that all providers with admitting privileges shall designate in writing two (2) or more alternates with comparable admitting privileges at this medical center. This

information is provided to all patient care units in the event that you are not available to attend to your patients. Please also note that providers are responsible to inform their designated alternates when they are not available to provide services (when provider is on vacation, sick leave, etc.). There have been instances when designated alternates did not assume their designated alternate responsibility of providing coverage during the provider's period of absence. Reminder: For Medical Staff Issues Involving Patient Care Call The Performance Improvement Hotline The Performance Improvement Hotline provides physicians, patients and hospital staff a route to refer questions involving the clinical care of patients in a confidential and timely manner, and to eliminate documentation on Occurrence Reports that are not protected under the peer review process. Messages are recorded and are referred to the respective department chairperson or designated alternate to determine appropriate follow-up with physicians. The number for the Performance Improvement Hotline is 983-8211 and is confidential. For concerns about medical management requiring immediate intervention, please utilize the Chain of Command. Note: The Performance Improvement Hotline is totally separate from the Corporate Compliance Hotline. The Corporate Compliance Hotline (1-888-274-3832) is for employees, physicians on Medical Staff, vendors, agents and partners of Hawaii Pacific Health to report any identified issues or questions associated with potential violations of laws, rules, regulations, policies and procedures.
