2 Darzi Fellowships in Clinical Leadership Charles Adler I come to the Darzi fellowship with a keen interest in improving the experience of patients navigating their way through different and often poorly-connected services. As a Paramedic, I have a developed an understanding of dealing with crisis and an interest in working back from the patient to address root causes. I am an advocate for increasing interoperability between services using IT. My background includes operational experience with the British Army as an infanteer and interpreter, including the mentoring and development of the Afghan National Police and National Directorate of Security. My Darzi Project: The delivery of a clinical and community wide urinary catheter learning and training project Coral Akenzua I am a paediatric doctor with a passion for change work and quality improvement. I have participated in several projects at work to help improve efficiency which I have really enjoyed. My project as a Darzi fellow will allow me exercise and improve on these skills more formally. My project about facilitating a pathway that allows patients with COPD to be managed as far as possible in the community. It aims to develop a means whereby where by these patients can utilise community services, new roles and technology to support the acute services. My Darzi Project: Hospital to home Nicola Aloysius My Darzi Fellowship is based in Surrey Heartlands STP Academy. I will be working within this inter speciality network to support the delivery of innovation and transformation across the STP clinical work streams, identifying and spreading best practice and removing unwarranted variation in clinical care. I am a General Practitioner by background and bring my experience of working as a Respiratory Clinical Lead to improve outcomes and reduce non - elective admissions to this role. I am passionate about enabling all service users to receive excellent care irrespective of their background and believe the key to this is focusing on greater clinician engagement to lead change. 3 Antony Aziz I m a gastroenterology ST3 trainee in KSS and am looking forward to working with the IBD service at East Surrey Hospital as part of the Darzi Fellowship to evaluate the impact of non-face-to-face IBD services on chronic disease management. I will be looking to support a rollout of this initiative to other specialties and sites across Surrey. My Darzi Project: Evaluation of the redesigned patient-centred East Surrey IBD care model with planned implementation across a range of specialties. Helen Bell I am a research based pharmacy technician based at The Centre for Medication Safety and Service Quality at Imperial College Heathcare NHS Trust. I will be working on a project titled Community Engagement and Involvement in the design and delivery of the Multispecialty Community Provider (MCP) based at First Community Health and Care (FCHC) in Redhill. The fellowship will be to create and deliver year one of FCHCs approach to community engagement and involvement in the design and delivery of the MCP. Rachel Coyle I am a public health registrar in my second year of training. As a registrar I have worked a variety of settings including local government and health protection. Most recently I have been carrying out research on sexual health and sexual behaviour at University College London. Prior to starting public health training I worked in sexual health and HIV medicine, and in various general medical specialties. Outside of work I am a trustee of and volunteer for a small charity supporting people living with HIV in London. 4 Kent, Surrey and Sussex Darzi Fellows Shirmilla Datta My project is the development of a quality improvement network in EoLC in SECAmb, improving on work already completed in the last year in order to ensure sustainability and develop a community of practice. I am a specialist paramedic who has already developed a good foundation to roll out a program of education and integration of services within the ambulance service. The aim of the project is to improve the standard of care that Ambulance clinicians are able to provide to their patients by improving knowledge and understanding, and working closely with community teams. My Darzi Project: End of Life Care in SECAmb Daniel Dodd I am a specialist paramedic in urgent and emergency care with an interest in quality improvement and system design. Over the last few years I have been working on sepsis care in the out of hospital environment and I am a member of the KSS patient safety collaborative for sepsis. Last year I was lucky enough to enrol onto the IHI patient safety collaborative. My project at Ashford & St Peters NHS Foundation Trust is a clinically led diagnostic project to understand the cultural, professional, hierarchical and organisational barriers which prevent the health & social care system from working in a one team integrated approach; and be part of developing a workforce model which would operate in this way. 5 Lisa Ekinsmyth I am a committed nurse with 5 years experience working as a clinical Matron followed by 3 years working in quality improvement roles across WSHFT. I have a strong background in acute care but also good awareness of the challenges we face across the health economy. I will be working at WSHFT on the safer care project. The philosophy for this project is to enable frontline staff to identify and overcome the barriers to safe care and support standardisation of successful approaches such that they become business as usual rather than to direct staff about expected actions and behaviours. I feel very excited about the potential to work with our community colleagues using the broad expertise of the multi-disciplinary team but also to potentially co design a project with patients and their carers My Darzi Project: Western Sussex Hospitals NHS Foundation Trust Safer care project(falls and pressure damage reduction) Mark Gregson I have been working as a Paramedic for 8 years. More recently a specialist in emergency and urgent care and in 2014 I started working in the education of pre-registration student paramedics. I am a committee member of the UK Educational Advisory Committee for the College of Paramedics, and part of a working group developing a UK Practice Educator handbook. I live in London, and enjoy cycling, keeping fit and visiting friends regularly. Fatin Fizz Izagaren I am currently an ST3 Paediatric Trainee as part of the KSS Deanery. I competed my Medical degree at King s College London in 2012 after completing a degree in Biology. I am very excited for my upcoming year as a Darzi Fellow! My Darzi role will be Children s Urgent Care Transformation Lead for Surrey Heartlands with the aim of developing a sustainable model for an improved delivery and quality of children s acute care services. Ultimately, the aim is to hopefully reduce the current high usage of Children s A&E though parent/ community education and empowering healthcare professionals whilst creating community based alternatives to A&E. 6 Darzi Fellowships in Clinical Leadership Claire Jordan I have a background in Pharmacy and then subsequently retrained as a counsellor and then a Cognitive Behavioural therapist. I like to problem solve and am good with numbers as well as people. I am told this is an unusual skills mix. I am looking forward to working on a project to help look at sustainable care pathways and be the clinical link between trust policies and new directions and front line services in regards to sustainability issues. The long term aim is that this will reduce carbon footprint, save resources: time, finances and waste. Work already done in this area has saved the trust money and improved staff wellbeing. My Darzi Project: Creating a more sustainable model of community care Bonnie Koo I have spent 5 years in the NHS pharmacy, now specialising in surgery and critical care during which I have had the opportunity to carry out a change management project. I analysed and improved the process of discharge medication supply turnaround time by 40% after 2 months. Since then I have been very much focused on bringing about a positive, enduring change to the greatest number as possible. But the change I am particularly passionate about is patient empowerment and inducing a behavioural change in patients to encourage self-management. The project that I have chosen focuses on patient-centred evaluation of medical interventions and liaising communication between pharmaceutical companies and patients to establish a mutually beneficial relationship. 7 Helene McCarthy I am a histopathology trainee with a strong interest in autopsy pathology. I studied medicine in Dublin and have worked in Galway, London and Brighton. Whilst my current clinical role does not involve direct patient contact, I feel strongly about social issues such as isolation and mental health. My work with the KSS Armed Forces community will mainly involve raising awareness of this group and helping employers across the region understand how armed service personnel, with their unique skills and experience, can be a valuable asset to their organisations My Darzi Project: KSS Armed Forces Community as an Asset Sally Morgan I have an extensive experience in working across traditional organisational boundaries and in working with service users and carers to ensure that their needs are at the centre of clinical work. I have worked with children and adults with learning disabilities so am looking forward to working in COPD and excited about bringing my experience to a new area and learning about different organisations and systems. My Darzi Project: KSS Respiratory Network Improving outcomes in COPD, Pulmonary Rehab and Oxygen Prescribing Uduakobong Ndiyo I am Uduakobong Ndiyo, currently undertaking a post graduate training in Paediatrics. I am in ST4 within KSS deanery and a Darzi Fellow 2017 session and my project is at Darrent Valley Hospital. I am fully registered with the General medical council with a licence to practice with a satisfactory revalidation 2014 and ARCP outcomes. In the next one year I have decided to tow a new part by looking into our NHS and being determined to make a change hence my application to become a Darzi Fellow. I am open to a challenging and rewarding year both in the University and in my personal project. 8 Darzi Fellowships in Clinical Leadership Cheta Okonkwo I am 5 years qualified as a pharmacist, have experience in both hospital and community pharmacy. In work, I enjoy networking, being challenged, have opportunities to lead and interested in change management projects. My project is based on a collaborative approach to enhance clinical outcomes and quality of life for adults with Type 1 diabetes by improving access and use of medical technology. Looking forward to getting stuck in! My Darzi Project: A collaborative approach to enhancing clinical outcomes and quality of life for adults with Type 1 diabetes by improving access and use of medical technology. Joanne Olagboyega I qualified as a midwife in 2010 and have undertaken various roles within the profession both within the acute service and the community setting. I undertook the Trust Clinical Leadership Programme in 2013 and following completion of this I was appointed as the clinical governance midwife. My responsibilities included incident investigation, root cause analysis, writing complaint responses, national reporting, audit and guidelines. I have just completed a secondment to the role of Community Midwifery Matron where I was responsible for over 100 midwives delivering care to approximately 7500 women a year. I will remain in East Kent to undertake my Darzi project which will mainly involve innovation in the maternity service related to digital technology. My Darzi Project: Overcoming the depersonalisation of digital services Jennifer Palfrey I am a palliative medicine trainee in London and am passionate about improving end of life care, educating both the public and healthcare professionals about dying and trying to reduce the taboo that has come to surround dying. I have worked in the acute sector; hospices and the community so have experience of different healthcare settings and understand how they relate to one another. My project is at Princess Alice Hospice, Esher and will focus on improving community end of life care. I will aim to use technology to aid communication and will look at ways to increase carer resilience. 9 Charlotte Pavitt I am a senior Public Health Registrar. A non-medic by background, I have always worked in Public Health at local and National level in health improvement and health protection. My project aims to deliver Social Prescribing at Scale across a number of localities in Surrey. The Fellowship really excites me as I see it as an ideal opportunity to bring together my Public Health training with clinical leadership. To me it seems that a focus upstream on prevention coupled with strong leadership to influence change and innovation is a welcome combination for the future of the NHS. The project will involve the design and implementation of a social prescribing system for the local population, providing GPs and other primary care staff with a non-medical referral option that will operate alongside existing treatments to improve health and wellbeing. My Darzi Project: Social Prescribing at Scale Harriet Peters I am a Pharmacist who currently specialises in Community Services and Palliative Care. I am interested in education and training, so I am excited to be working on a project at Health Education England, Kent, Surrey & Sussex to develop multiprofessional training opportunities. I will review the current provision; look at commonalities of training across healthcare professions and work to increase the opportunities for joint learning. I will also be reviewing the current communication arrangements between HE KSS and trainees and look to develop new ways of informing and engaging trainees. 10 Darzi Fellowships in Clinical Leadership Sangeetha Sornalingam I am a GP in Brighton with an interest in medical education. During my Darzi fellowship year I will be working with a not for profit social enterprise called Here. I will be exploring how we can make general practice work better for patients. I will do this through learning about and implementing a systems thinking approach to general practice service design. I will test new ways of working in general practice, which aim to improve efficiency, whilst ensuring that the focus is always on what matters to the patient community. With my educational background I am also planning on working on a smaller project with Here that aims to enhance the undergraduate experience of general practice. I am looking forward to the Darzi and working with such a forward thinking organisation. My Darzi Project: Making a difference to the lives of a community of 7000 people. Anand Sundaralingam I am a Respiratory registrar with an interest in leadership and management, health policy and economics and finally innovation and technology. I believe the project encompasses all these aspects that I am passionate about. I will be working on a Digital Health Testbed. The project will look at developing a living lab (real people in their home environment) provided digital technologies by a network of tech champions with the chance to test and evaluate these technologies. My Darzi Project: Digital Health Testbed Sarah Trute I am a quality improvement nurse - learning disabilities and will be undertaking my Darzi fellowship with Health Education England. 11 Fiona Whitaker My background in acute nursing of both adults and children in the NHS and in the private sector and health visiting in the community has allowed me to gain an excellent overview of healthcare. I would like to use my knowledge to help transform care, so that clients receive appropriate, streamlined care wherever they live. This project focuses on the re-design of county wide services to provide an integrated networked model of care that promotes a single point of access for people experiencing a mental health crisis. Currently there are substantial variations in service provision; due to the fact the organisation covers 3 STP s. Outcomes will include: reduced A&E attendances and streamlined early intervention and preventative approaches. My Darzi Project: Managing Crisis Well in Mental Health Stuart Yeomans I have worked in the NHS as a Physiotherapist for eight years and, as a Team Lead in Musculoskeletal Outpatients, saw the appeal of the Fellowship as a great opportunity for myself and my Trust to benefit from a unique learning environment in a time of great change for the NHS. My project at Sussex Partnership NHSFT involves generating solutions to enable those with serious mental health conditions to access the physical health checks and treatment required to increase their length and quality of life. I am looking forward to working closely with the regional Trusts and STP s to address this issue over the coming year.
