JOB TITLE GRADE Band 3 HOURS OF DUTY REPORTS TO SAINT FRANCIS HOSPICE JOB DESCRIPTION Pemberton Place Support Assistant 37.5 hrs /or part time pro rata Pemberton Place Co-ordinator KEY RELATIONSHIPS Pemberton Place Co-ordinator, Pemberton Place Administrator,, Individuals attending Pemberton Place Carers, Internal and External Care Providers and Volunteers Head of Community Services RESPONSIBLE FOR N/A JOB SUMMARY To work within Pemberton Place, ensuring that holistic, high quality specialist individual care is maintained. Assist, support and work in collaboration with the facilitators of group sessions Be an advocate for the Pemberton Place model of access to services and outpatient sessions Work flexibly as indicated by Pemberton Place Co-ordinator to allow for development of groups and sessions To be aware of and involved with department processes as required, the organisation and coordination of transport requests on a flexible day to day basis for individuals to and from the hospice and to hospital for various appointments, arrange for specimen collection and delivery whilst having regard to the cost effective use of volunteer drivers and other services use of provide transport. MAIN DUTIES AND RESPONSIBILITIES TEAM WORKING To participate fully in the hub environment of group and outpatient sessions that will be held in Pemberton Place. To work as part of the Pemberton Place Service both under supervision and alone. To work as part of the multi-professional team. To participate as a member of Pemberton Place attending daily meetings and any other meetings as required. To have visual access to daily electronic calendar as resource and have awareness regarding the process for inputting room booking requests as directed by Pemberton Place Coordinator To interact and liaise daily with OrangeLine and work together for the benefit of the individuals attending the Hub space To have an understanding of the importance of multi-professional team-working and the ability to promote it within the organisation. To have an understanding and awareness of behaviours and interpersonal skills that are vital to effective team working and practice them at all times. To participate in change and development of practice within Pemberton Place To give basic administrative support to the service in the absence of Pemberton Place Administrator

To assess appropriateness, organise and co-ordinate transport for patients to and from the hospice, and to hospital for various appointments, having regard to the costeffective use of volunteer drivers. To contact the appropriate ambulance providers to arrange transport for patients for admission or discharge to the hospice or outpatients appointments in Pemberton Place. To aim to resolve necessary transport difficulties including changes in arrangements. To contact patients or relatives to confirm arrangements for transport. To arrange on infrequent occasions for the emergency collection of drugs from the pharmacy for the inpatient unit as requested To arrange on infrequent occasions individuals discharged from IPU may need necessary delivery of pharmacy medications to patients home To keep the appropriate managers/staff informed of arrangements. To maintain adequate documentation records using icare where appropriate and completion of data forms to record transport use and rationale of journey To access, input and print off reports as required patient details using various recording systems including, icare, Cyberlab and Health Analytics etc Use training opportunities available to increase IT skills and use within the role. CLINICAL PRACTICE: To assist patients and carers with their holistic care needs. To consult and liaise with both the nursing and multi-disciplinary teams to ensure continuity of holistic patient care. To implement nursing procedures correctly and ensure that patient documentation reflects a holistic account of patient interventions. To be a reflective practitioner who demonstrates flexibility and attend mandatory reflective practice sessions. To ensure documentation regarding patients is fully updated appropriately and timely To ensure all policies and procedures adhered to if on the rare occasion to escort patients off site on volunteer transport both to own home and on hospital admissions. Participate and contribute to hospice Widening Access groups and Special Interest groups as appropriate COMMUNICATION To maintain effective communication with patients, relatives/carers and other visitors including external providers of care To participate in discussion and communication within the team when planning and evaluating patient care needs. To participate in multidisciplinary team discussion of patient and carers needs. EDUCATION OF SELF AND OTHERS To maintain an awareness of developments in nursing and palliative care. To take responsibility for own personal development which will be monitored in one-to-one sessions through the hospice appraisal system ADDITIONAL REQUIREMENTS OF THE POSTHOLDER To adhere to the policies and conditions of service of Saint Francis Hospice

ADDITIONAL REQUIREMENTS OF POSTHOLDER SENSITIVITY AND PROFESSIONALISM: As an employee of Saint Francis Hospice you should expect that it is likely that you will come into contact with patients, their families and members of the public who are at the end of life or recently bereaved. All employees are expected to respond to people recounting and expressing their emotions about such events with sensitivity and understanding, and to know enough about the services that the Hospice provides to be able to signpost people who may benefit from our services to the relevant contacts. CONFIDENTIALITY: The post holder must maintain confidentiality, security and integrity of information relating to patients, staff and other hospice matter during the course of duty. Any breach of Confidentiality may become a disciplinary issue. DATA PROTECTION: It is the responsibility of all staff to be aware of their obligations in respect of the Data Protection Act 1998. HOSPICE IDENTITY BADGES: Hospice ID badges must be worn at all times whilst on duty. SMOKING: The hospice acknowledges its responsibility to provide a safe, smoke free environment, for its employees, service users and visitors. Smoking is therefore not permitted at any point whilst on duty, in accordance with the guidelines set down within the Hospice No Smoking Policy. REGISTRATION COMPLIANCE/CODE OF CONDUCT: All employees who are required to register with a professional body to enable them to practice within their profession are required to comply with their code of conduct and requirements of their professional registration. WORKING TIME REGULATIONS: that you should not work more than an average of 48 hours in each week. For example, in a 26 week period you should work no more than 1248 hours.
