CCI Overview WHAT IS CAL MEDICONNECT? Cal MediCnnect is a health plan that cmbines all yur Medicare and Medi-Cal benefits. Cal MediCnnect health plans will fcus n crdinating all yur health care needs, including medical care, behaviral health care, and hme and cmmunity-based services. These health plans als prvide additinal benefits such as transprtatin and visin care. WHO IS ELIGIBLE? Yu must have bth Medicare and Medi-Cal t enrll in a Cal MediCnnect health plan. Cal MediCnnect is currently available in seven cunties: Ls Angeles, Orange, Riverside, San Bernardin, Santa Clara, San Dieg, and San Mate. T enrll in a plan tday, call Health Care Optins at 1-844-580-7272, Mnday Friday frm 8:00 am t 5:00 pm. Fr TTY users, call 1-800-430-7077. A representative in yur language can help yu enrll r change yur health plan. WHAT ARE THE BENEFITS? Cal MediCnnect health plans prvide the same medical services that yu currently receive, but have additinal benefits as well: Only ne phne number t call Only card t carry. When yu have questins r need help, yu nly need t call ne phne number. And yu nly need yur Cal MediCnnect health plan card fr services. Yur health plan will assign yu a persnal care crdinatr (a nurse r ther health care prfessinal) t answer questins and make sure that yu get the right services frm the right prviders. They can even help yu schedule appintments, arrange transprtatin, and check n prescriptins. Yur health plan will make sure that yur dctrs, specialists, pharmacists, caregivers, case managers, and ther prviders all wrk tgether t help yu take care f yur health. Yur health plan will cver and crdinate yur health needs, including: prescriptin drugs, visin care, and sme transprtatin t medical services. 2

CCI Overview Transprtatin: 30 ne-way health-related trips per year, such as medical appintments, picking up prescriptins, physical therapy sessins, and mre. Visin: Free annual eye exam and $100 fr eyeglasses r cntact lenses every tw years. Yur team is put tgether based n yur needs and always includes yur care crdinatr. Yur care team can als include: specialists, family members, care givers, In-Hme Supprtive Services (IHSS) case wrkers and prviders and thers. Yu and yur care team create a CARE PLAN just fr yu. Yur care plan helps yur prviders understand yur health gals, 5 needs, and preferences. It highlights the services yu want and need, and helps yur care team cnnect yu t thse services.

Prviders CAN I KEEP MY DOCTOR? If yu already have dctrs that yu knw and trust, yu can call the Cal MediCnnect health plans in yur cunty t ask if yur dctrs are a part f their netwrk. Yu can als ask yur dctrs if they have jined a Cal MediCnnect health plan netwrk. If yu jin Cal MediCnnect, yu must use dctrs wh are a part f yur Cal MediCnnect health plan s netwrk. Fr help finding ut if yur dctrs are in a Cal MediCnnect health plan s netwrk: call Health Insurance Cunseling & Advcacy Prgram (HICAP) at 1-800-434-0222. If yur dctr is part f a Cal MediCnnect health plan's netwrk If yur dctr is NOT part f a Cal MediCnnect health plan's netwrk Make sure yu enrll int the same Cal MediCnnect health plan that yur dctr is cntracted with. Wrk with a Care Crdinatr t transfer yur care t a differnt dctr in the health plan s netwrk. Ask yur dctr t jin a Cal MediCnnect health plan. In sme cases, yu can cntinue seeing yur dctr fr up t 12 mnths fllwing yur enrllment in a Cal MediCnnect health plan. This is called Cntinuity f Care which is explained, in detail, n page #.? Chsing Yur Dctr Yur health plan can help yu chse dctrs that yu trust and meet yur needs. As a Cal MediCnnect health plan member, yu will have the ptin f chsing a primary care prvider, r yu will have ne assigned t yu wh will wrk with yu t get yu necessary check-ups and screenings, keep yur health recrds, help yu manage chrnic cnditins, and cnnect yu with ther prviders as needed. If yu d nt like yur assigned primary care physician, yu can wrk with yur health plan t find anther ne. Yur primary care prvider will cnnect yur with specialists, such as cardilgists, nclgists, psychlgists, allergists, pdiatrists, and rthpedists t treat specific cnditins. Changing Yur Dctr Yu shuld always have dctrs wh yu can wrk with, trust, and wh meet yur needs. If yu are nt happy with yur first visit, yu can call yur 6 dctr s ffice and share yur cncerns. If yu d nt want t try a secnd appintment with that dctr, yu may ask t see anther dctr in the same ffice, r wrk with yur health plan t find a new dctr.

Cntinuity f Care When yu enrll in a Cal MediCnnect health plan yur care shuld nt be disrupted. Yur Cal MediCnnect health plan will ensure that the transitin is smth and yur health needs are met every step f the way. Call yur new plan t let them knw abut any treatment r services yu have scheduled. HOW TO KEEP YOUR DOCTOR: CONTINUITY OF CARE If yur current dctr is NOT part f yur Cal MediCnnect health plan, yu have the right t: Cntinue receiving care frm yur dctr fr 6 mnths fr Medicare services and 12 mnths fr Medi-Cal services - as lng as yu have an existing relatinship with the dctr. Yu must have an existing relatinship with a primary care dctr. Yu have t have had at least 1 visit in the last 12 mnths with yur specialist and yu have t have had at least 2 visits in the last 12 mnths with yur primary care dctr. Additinally, in rder t receive cntinuity f care, yur dctr must: Be willing t wrk with the plan and accept payment frm the plan. Nt be excluded frm the plan s netwrk fr quality r ther cncerns. Yu must transitin t receiving care frm a prvider in yur Cal MediCnnect plan s netwrk at the end f 12 mnths fr Medi-Cal services and 6 mnths fr Medicare services. Yur Cal MediCnnect plan can help yu with this transitin. YOU HAVE THE RIGHT TO CONTINUE TO RECEIVE MEDICALLY NEEDED SERVICES EVEN IF YOU MAY NO LONGER BE ABLE TO RECEIVE THEM FROM THE SAME PROVIDER. Hw t cntinue seeing a dctr utside f yur Cal MediCnnect Plan s netwrk: Befre chsing a Cal MediCnnect health plan, call the plan r HICAP t see if yur dctrs are cntracted. If they are nt cntracted with a Cal MediCnnect health plan: 1. Call yur Cal MediCnnect health plan and tell them abut yur scheduled care. 2. Tell yur plan that yu want t keep seeing yur dctr. The dctr must agree t wrk with the plan. Yur dctr r authrized representative can als call yur plan t arrange the services yu need. 7

Cntinuity f Care 3. If yu get care frm a dctr utside f yur Cal MediCnnect plan netwrk, let yur plan knw. 8

Other Prviders and Services Cal MediCnnect OTHER PROVIDERS AND SERVICES: Lng Term Services and Supprts Cal MediCnnect cvers all f yur Medi-Cal Managed Care Lng Term Services and Supprts, including In- Hme Supprtive Services (IHSS). IHSS prvides persnal care and ther services fr peple wh need help t live safely at hme. If yu have IHSS, jining Cal MediCnnect des nt change yur services r reduce yur IHSS hurs. If yu jin Cal MediCnnect, yur hurs may even be increased based n the results f yur Health Risk Assessment (HRA). Yu can still hire, fire, and manage yur IHSS prviders. If yu are in a nursing hme, yu will nt have t change nursing hmes - unless yur Cal MediCnnect health plan is cncerned abut the quality f care prvided at the nursing hme. Insert graphic Prescriptin Drugs Cal MediCnnect health plans cver all Medicare Part D drug benefits and any Medi-Cal prescriptin drug benefits. Because f this, yu will n lnger need a separate Part D plan. Yur ld Part D plan will send a cnfirmatin letter t let yu knw that yur drugs will nw be prvided by yur Cal MediCnnect health plan. This des nt mean yu are lsing yur Part D benefits. When yu first enrll in Cal MediCnnect, yur Care Crdinatr will wrk with yu t make sure that yu have at least a 30-day supply f yur current medicatins as yu transitin t receiving yur prescriptins thrugh yur Cal MediCnnect plan. While all Cal MediCnnect plans cver the same drugs, sme cver different generic names f the drugs. Yur plan will wrk with yu t make sure yu get the drugs yu need. 9

Other Prviders and Services Language Services If yu d nt speak English, yur health plan must prvide assistance in yur language. If yu speak American Sign Language (ASL), yu have the right t a sign language interpreter. Language assistance includes: medical interpreters, dctrs and staff wh speak yur language, and printed materials in yur language, such as cnsent frms and directins fr treatment. T ensure that an interpreter is present, either by phne, vide cnference, r in persn, make sure t tell yur plan that yu need ne. Yu shuld als ask fr an interpreter when yu make yur appintment. Interpreters are part f Cal MediCnnect and yu will nt have t pay. Yur plan can give yu a list f prviders that speak yur language. Ask yur plan and dctrs t make a nte in yur medical recrd that yu need an interpreter and dcuments in yur language. Medical Supplies During yur Health Risk Assessment (HRA), yu will be asked abut the supplies and medical equipment yu need and additinal supplies that yu may benefit frm. Each Cal MediCnnect plan has specific suppliers, s nce yu are enrlled, yu will get yur supplies frm yur plan s supplier. Yur plan will wrk with yu t make sure yu receive yur medical supplies withut disruptin. Insert graphic 10

MLTSS Managed Medi-Cal Lng-Term Services and Supprts (MLTSS) If yu chse nt t enrll in a Cal MediCnnect health plan, yu are required t enrll in a Medi-Cal Managed Care plan s that yu can cntinue receiving Medi-Cal services. A Medi-Cal Managed Care plan includes lng-term services and supprts, if yu have them. Will enrlling in a Medi-Cal Managed Care plan change my Medicare benefits? N. Yur Medicare benefits will nt change. Yu will still be able t g t yur Medicare dctrs, hspitals, and prviders. If yu have issues receiving yur Medicare services, call yur Medicare plan r 1-800-MEDICARE, (800) 633-4227, immediately. What will change when I enrll? When yu enrll in a Medi-Cal Managed Care plan, all f yur lng-term services and supprts will be managed by the plan. If yu receive medical supplies and equipment, yur health plan must still prvide these. Hwever, the health plan s vendrs will nw prvide yur supplies and equipment. Yur In Hme Supprtive Services (IHSS), Cmmunity-Based Adult Services (CBAS), Multipurpse Senir Services Prgram (MSSP), inpatient nursing facility, and subacute care facility services will nt change. Yu always have the right t hire, fire, and manage yur IHSS prviders. Hw can I cntinue receiving necessary services? Yur Medi-Cal managed care plan must cntinue t prvide all f yur current Medi-Cal services, including all medical supplies and equipment. Cntact yur plan immediately if yu are nt receiving necessary services. Will I be charged fr my MLTSS r Medicare services? N. Yu shuld nt receive a bill fr yur MLTSS r Medicare services. Yu are nt respnsible fr paying c-pays, c-insurance, r deductibles fr any Medi-Cal r Medicare services yu receive. The nly exceptin is fr Medi-Cal beneficiaries wh have a mnthly share f cst bligatins, wh must pay their share f cst. Otherwise, billing yu wuld be called Balance Billing and is illegal under bth federal and state law. Fr mre infrmatin n Balance Billing see page # X. 11

What is a managed care health plan? Pharmacies Yur health plan pays fr prvider services, medicatins, hspital care, and special equipment t help yu manage yur health. If yu have bth Medicare and Medi-Cal yu shuld nt be billed fr any f these services unless yu have a mnthly share-f-cst. Fr questins abut share-f-cst, cntact Health Care Optins at 1-844-580-7272, Mnday thrugh Friday, 8:00 am t 5:00 pm. TTY users can call 1-800-430-7077. Primary Care Physicians Medical Suppliers Health Plan Netwrk Nursing Facilities Specialists A managed care plan helps yu t manage yur health thrugh a netwrk f prviders. A plan s netwrk is a specific grup f health care prviders, including: primary care dctrs, specialists, pharmacies, nursing facilities, and suppliers f medical equipment, such as wheel chairs. As a member f a managed care health plan, yu must use the prviders in yur health plan s netwrk. Prviders wh are nt in yur health plan s netwrk are called ut-f-netwrk prviders. If yu are a Cal MediCnnect member and have an ut f netwrk prvider, yu may be able t cntinue seeing that prvider fr up t 6 mnths fr Medicare services and 12 mnths fr Medi-Cal services, if yur dctr and the health plan agree. Please see the Cntinuity f Care dcument n page XX fr mre infrmatin. What are the different types f dctrs included in a health plan s netwrk? Primary Care Prvider: This is the persn yu see first fr mst medical services.
