Neurological field officers Purpose : To provide a high quality, efficient and effective dietetic service for clients and families / caregivers, whanau, within an interdisciplinary team framework To provide a timely nutrition intervention to patients where appropriate to improve their health status, rehabilitation and recovery Services are delivered in accordance with WDHB and Medicine and Health of Older Poeple Strategic Plans KEY TASKS Provide high quality nutrition intervention to patients who have consented to a community dietetic visit Appropriate clinical assessments are conducted within specified timeframes. KEY TASKS outcomes from their intervention. Interventions are based on the findings of the assessment and the patient s goals. Nutritional standards for clinical practice and safety are maintained in accordance with professional and organisational standards. Interventions are evaluated regularly and discussed at dietitian and interdisciplinary meetings. Documentation accurately demonstrates interventions and progress and is in accordance with documentation standards set by Waitemata DHB and Waitemata DHB Dietitians audit protocol. Discharges occur in a reasonable timeframe as per the policy. Consultation with other interdisciplinary team members/ Residential Aged Care Staff occurs. Boundaries of practice are identified / recognised and a referral to the appropriate Clinician is generated Provides dietetic intervention for patients receiving enteral feeding and residing in a Residential Aged Care Facility. Provides training for Dietetic Students. Is able to support and guide a patient led intervention. Quality Assurance. Supervision is undertaken on a regular basis (as per Professional Supervision for Allied Health in WDHB policy). Annual review follows performance development guidelines with annual review and objectives which are reviewed on a regular basis Responsibility is taken for own learning by recognising training and skill needs and undertaking professional development activities on an ongoing basis. Completes Enteral Feeding e-learning Competency biannually Completes annual Waitemata DHB Dietitians Practice Review Requirements. Quality of service. Criteria for admission /prioritisation to service are followed Participation in interdisciplinary meetings and case conferences Contributes to the functioning and development of the interdisciplinary team There is participation in overall dietetic caseload management within Medicine and Health of Older People, covering for other teams where essential Networks with other dietitians within the service and externally Assists with the orientation of staff and with the training and education of student dietitians as required Initiates and participates in the implementation of service developments Provides education to Waitemata District Health Board employees as requested and external groups in consultation with Team leader Ministry of Health Criteria for admission to service are followed Attends monthly Community Dietitian Meetings Attend Regional Community Dietetic Meetings Attend WDHB Combined Dietitians meeting quarterly Service administration standards are met. PERSON SPECIFICATION POSITION TITLE: Dietitian Minimum Qualification NZ registered Dietitian Current practicing certificate with Endorsement on Scope of Practice: Dietitian prescriber. Preferred Completed training in Professional Supervision Completed training in Cognitive Behavioral Therapy Experience 2 years post graduate experience 4 years post-graduate experience. Has a wide dietetic knowledge base of all nutrition related diseases. Proven experience in the management of patients receiving enteral nutrition Experience working with older adults. Experience working crossculturally Experience in completing clinical audits Experience in dietetic related research Knowledge Understanding of evidence based practice.
