HOW TO USE THE CLIENT AND HEALTH COACH SUPPORT SYSTEM CLIENTS A. NEW CLIENT Cntinue t fill ut a Wellbeing Tracker r Health Tracker frm, attach it t their Wellbeing Evaluatin (if cmpleted). Put the frm in Sectin 2 New Clients. During yur weekly Client Check-ins, refer t their Tracker and recrd prgress. Once the Client has been n their prgram fr tw weeks, mve them t Sectin 3 Active Clients. B. BECOMES HEALTH COACH If a Client decides t becme a Health Cach, start a Health Cach/Business Cach Tracker and put it in Sectin 4 New Health Cach. Cntinue t leave their Wellbeing Tracker in Sectin 3 Active Clients (since yu ll still be caching them) C. INACTIVE If a Client becmes inactive (is n lnger rdering), mve them t Sectin 6 Inactive Clients s that yu can retain their infrmatin shuld they decide t cme back at sme pint. HEALTH COACHES A. NEW HEALTH COACH AND NEW CLIENT AT SAME TIME Cmplete a Wellbeing Tracker and place it in Sectin 2 New Clients. Cmplete a Health Cach/Business Cach Tracker and place it in Sectin 4 New Health Caches Senir Caches. B. CLIENT BECOMES A HEALTH COACH Cmplete a Health Cach/Business Cach Tracker and place it in Sectin 4 New Health Caches Senir Caches. NOTE: During yur Mentrship calls, refer t their Trackers and recrd prgress. C. REACHES SENIOR COACH Mve them t Sectin 5 Senir Cach t Executive Directr. CANDIDATES WHO NEED ADDITIONAL FOLLOW UP: Add them t yur Cnnectins campaign. If yu have their cmpleted Wellbeing Evaluatin and/r have started ne f the Trackers, put it in Sectin 1 Candidate Fllw-Up. DIGITAL METHOD: ELECTRONIC FOLDERS Instructins: Create a directry n yur cmputer titled Client and Health Cach Supprt System. Dwnlad the Instructins, Cntact List, and blank Trackers in the directry. Create 6 flders in the directry, each ne crrespnding t each Sectin. Mve the electrnic Tracking Frms frm ne flder t the next as the persn prgresses. 2. PAPER METHOD: 3-RING BINDER What yu ll need: Binder Clred Card Stck (7 pieces) r plastic dividers Three-Hle punch Blank tabs (if desired) Instructins: Print ut the Sectin Cvers see belw (suggestin: print n clred cardstck). Print ut all frms (three-hle punch). Arrange in a 3-ring binder. The Instructins, Cntact List, and blank Trackers g befre Sectin 1 in the binder. Each Sectin becmes a tab.
