Residence Manager Full time Monday Friday 9am 5pm Housing & Support Services Scatter Site BA/BS Degree and/or comparable experience Oversees the daily operation and maintenance of all apartments in the program. Supervises maintenance staff to ensure quality control and hygiene standards. Recruits apartments, outreach to community to identify potential new landlords, educate landlords regarding program policies, visits sites to assess appropriateness. Responsible for coordination and facilitation of client move-in including telephone and utility hook-ups. Monitors apartments for safety, hygiene and landlord service delivery. Liaison with landlords, to bring landlord s responsibilities to their attention and request compliance. Reports noncompliance of landlords to Program Director. Ensures ongoing client rent collection. Attends weekly staff meetings and weekly client review team case conferences when requested. Experience with housing coordination and supervision. Experience working with HIV/AIDS, homeless and substance abuse populations. Security Officer Part time Saturday & Sunday Shifts available 2pm 10pm 10pm 6am & On Call (on an as needed basis) Homeless Services Drop in Center & Hospitality House HSD/GED NYS Security Cert. F-80 /Fire Safety Coordinator Cert. required or must obtain certification within 3 months of hire Responsible for security of center. Screen, monitor and oversee all clients; Work closely with program administration and clinical supervision in the day to day treatment of clients. Directs visitors and handles all inquiries. Conduct rounds of premises, prepare reports, and notify supervisors of incidents. Handle all emergencies; perform crisis intervention as needed, calls 911 and other emergency numbers for back up as needed; try to work out stressful situations as appropriate. Report all incidents using the incidents reporting systems making sure all appropriate people are notified. Receives incoming mail and packages and gives to Admin. Assistant. On occasion will deliver packages as needed. Responsible for client, fire, and health safety. Oversee front desk and answer phones. Previous security experience required. Social Worker Full time Tuesday Saturday 11am 7pm Transitional Services Youth Drop In Center Masters Degree & LMSW or LMHC Engage and counsel clients in effective manner. Assist client in developing independent living skills through oneon-one sessions and weekly group sessions. Provide therapeutic interventions to clients in order to motivate them to work on their service plan goals. Link clients to appropriate mental health services, employment and academic resources. Provide client crisis intervention and stabilization services for the Youth. Provide immediate and ongoing care coordination around housing, medical, employment, education, mental health and substance abuse. Care coordination includes making referrals for services, monitoring service utilization, case conferencing with internal and external providers and follow up. Case conference monthly with clinical staff in Youth Services. Identify pertinent trainings for Youth Workers and attend monthly funders meetings. Provide weekly groups (topics are to include ADL skills, entitlements, health awareness and housing readiness, mental health, etc. Ensure proper charting of all client information is done on a timely basis. Maintain up to date documentation including training logs, supervision logs, service plan and weekly progress notes and Critical File. Assist Program Director with staffing and coverage issues. Ensure proper documentation in Log book, shift summaries from Youth Workers and coordinate required in house training with Program Director. Assist Program Director with site operation and safety. Participate in all scheduled supervision and team meetings. Attend 40 hours of training per year with a focus on youth services.
Social Worker/Family Therapist Full time Variable days Wellness & Recovery PREP Master Degree & LMSW or LCSW required or LMHC with at least two years of work related experience in counseling mental health and dual diagnosed clients. MSW Intern under the supervision of an LCSW with counseling experience will be considered. Provide clinical supervision to clinical staff and peers. Administer Mental Health and family Screenings and assessment tools and enter data into the AWARDS record system. Ensure all documentation is appropriate and accurate for billing. Complete progress notes within the required time for every service rendered. Complete a service plan and make referrals to additional services as needed. Make appointments with clients for further services and follow-up on missed therapy as needed according to the client service plan. Conduct group Counseling - and small process groups as needed to connect with clients and according to service plans; prepare group notes for group services and maintain an attendance form for each group contact. Conduct family counseling sessions for clients and family members as requested. Assist in the development of program materials and literature. Must possess the ability to prepare client care forms accurately and in a timely manner. Must have good time management skills. The ability to work independently, schedule clients and follow-up on clients care as well as the ability to work as a member of a multidisciplinary team is needed.

Collects client data and connects client to education, career exploration, vocational training and employers. Conducts vocational services orientation and referrals. Meet with clients on caseload according to regulations and policy to identify client needs and progress towards meeting vocational/career goals. Assist client with creating an individualized vocational plan that will outline the client's goals and career/employment objectives, as part of the multidisciplinary team. Conducts individual and group services in accordance with treatment plan including pre-vocational skills, employment readiness, resumes and application writing, and other options, vocational training and education entitlements, benefits and community resources. Conducts post-employment retention services. Assist with client crisis; communicates with managerial, clinical and support staff when clients' needs demand additional monitoring or attention when experiencing crisis, etc.
Vocational Counselor (continued) Ability to redirect and de-escalate crisis situations, conduct emergency management and has understanding of proper emergency procedures. Document all client care in EHR and records/reports all client/staff incidents as required. Meets with client according to care plan to review and discuss discharge plans, continuing care and community resources needs (continued treatment, entitlements, employment etc.). Create community employment and vocational entitlement resource banks, makes client referrals and follow-up on all referrals. Escort client to appointments as needed. Conduct post discharge follow-up and document in client record and reporting forms. Maintain the charts of clients in accordance with OASAS and agency requirements. Vocational services experience needed. Minimum of two years case management experience working with the substance abusing and homeless populations needed. Bilingual Spanish/English preferred.

Ensure proper charting of all client information on a timely basis. Participate in weekly supervision and all team meetings. Experience working with homeless youth.
