Care Coordinator Full time Monday Friday 9am 5pm Transitional Services Health Homes BA/BS w/1 year qualifying experience or HSD/GED w/2 years of qualifying experience Provides intensive care coordination activities for clients. Assure that members have access to, engage in and retain needed services as defined in the members care plan. Make assessments and appointment referrals including psych evaluations, TB and HIV testing, detox, etc. & complete formal assessments whenever possible. Assist clients with completion of forms for entitlements, etc. Complete psychosocial and social history when needed. Counsel Clients leading to interim, transitional and permanent placements. Complete accurate timely statistics as required by the program. Responds in a timely manner to calls from the community regarding homeless individuals in all areas of Staten Island. 3 to 5 years experience with the homeless, substance abuse, and mentally ill population needed. Excellent outreach and client engagement skills a must. Must have excellent communication skills both orally and in writing. Bilingual Spanish/English preferred.

Case Manager On Call (on an as needed basis) Wellness & Recovery PREP BA/BS Provides counseling and casework services to clients living with substance abuse histories in our PREP program. Meet with clients on caseload on a regular basis; Assist with client crisis; Oversee clients compliance in taking prescribed medication and maintain medication documentation; Make rounds of facility to engage clients as well as ensure client safety and document all findings in communication log; Oversee kitchen use and supervise snack and meal periods; as well as supervise client clean up; Maintain the charts of clients in accordance with OASAS and agency requirements; Participate in weekly Clinical Team Meetings. Experience working with the substance abusing population preferred, some case management experience needed. Minimum of two years experience working with the substance abusing and homeless populations. Bilingual Spanish/English preferred. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding confidential and sensitive issues.

Case Manager Part time Monday & Wednesday 9am 2pm & Friday 9am -1pm (14 hours a week) Housing & Support Services Scatter Site BA/BS Responsible for case managing clients in the Scatter Site Housing Program while assisting them with maintaining stable and permanent housing Design and implement individual service plans that reflect mutually agreed upon short and long term goals. Obtain and maintain all government benefits/entitlements and services for each client on caseload. Make appropriate referrals, such as, medical care, mental health services, vocational services, recreational activities, and substance abuse services. Conduct weekly home and/or office visits and hospital visits. Accompany clients to medical appointments. Manage medical and personal emergencies of assigned clients and make adjustments to care as needed. Conduct reassessments of service plans on a regular basis to ensure an ongoing awareness of the client s medical conditions and service changes needed. Maintain an accurate, confidential and current chart on all clients on caseload. Prepare monthly narrative report for assigned caseload on a timely basis. Attend regularly scheduled supervision meetings with Program Director. Experience in case management and working with HIV+ and homeless populations preferred. Excellent communication skills both orally and in writing needed.

Case Manager Full time Monday Friday 11am 7pm Transitional Services Youth Drop In BA/BS Manage and counsel clients enrolled at the Youth Drop-In Center. Assist client with their service plan through one-on-one sessions and weekly group sessions. Determine any immediate need and link clients to appropriate services. Monitor engagement in services and escort clients to appointments as needed. Case Manager will work 4 days in the office and 1-day off-site that focuses on Community Outreach/ Linkage for the Youth Drop-In Center. Provide client crisis intervention, stabilization services, and referrals to Clinicians in Youth Services or agency clinics as needed. Provide ongoing care coordination around housing, savings, health benefits, employment, education, mental health and substance abuse. Care Coordination includes making referrals for services, monitoring service utilization, case conferencing with internal/external providers and follow up. Provide weekly workshops (topics are to include school, ADL skills, budgeting, employment, health awareness, diversity, LGBTQ related topics and housing retention). Make regular entries in staff communication log according to policy/procedure and ensure proper charting of all client information is done on a timely basis. Responsible to maintain up to date documentation including intakes, service plans, weekly progress notes, savings forms, and medical documentation. Participate in scheduled supervision and all team meetings. Attend 40 hours of training per year with a focus on youth services/lgbtq. Two years of case management/crisis intervention/social services/lgbtq work experience background is needed. Knowledge regarding High School Equivalency Exam/Diploma, housing services, entitlements, and training essentials for the LGBTQ population is needed. Must have excellent communication skills, both orally and in writing, with individuals/groups regarding complex and sensitive issues.

Case Manager Full time Monday Friday 9am 5pm with flexibility to work evening hours Residential Services HUD40 BA/BS Oversees housing units and provides case management services to tenants within the program. Completes intakes, along with pre/post placement assessments and re-assessments, as required. Develop service plans in conjunction with team members and tenants. Meet weekly with tenants, working to develop independent living, apartment management and neighbor relation skills. Assist tenants in accessing nutritious food, as well as in obtaining and maintaining entitlements. Assist with vocational training services and job placement assistance. Provide ongoing coordination with primary care physicians, mental health and other providers. Respond to tenants requests for information and referrals. Complete required documentation on a timely basis. Meet with supervisor on a weekly basis for supervision. One year previous related experience preferred.

Certified Recovery Peer Advocate Part time 10 hours a week Wellness & Recovery Clinical Services Recovery HSD/GED Works with clients individually to help them achieve their treatment goals. Provide recovery support to clients by sharing life experience. Develop recovery plans in conjunction with clients. Support program participants in pre-crisis or crisis situations. Provide non-clinical crisis support for clients. Assist clients in the development of positive coping skills, strengths, and formal recovery supports. Teach participants recovery skills, based on shared life experiences. Accompany clients to appointments, as needed to provide support and assistance. Link participants with needed community resources and treatment services. Go into the community in order to actively engage clients who are currently enrolled in the program and who may have disengaged from services. One year recovery time and Certification as a Recovery Peer Advocate required. Provisional Certification will be accepted. Experience with outreach and substance use disorder (SUD) clients. Must be willing to disclose lived experiences in substance abuse. Clean and valid driver s license required. Must be able to navigate NYC public transportation system. Experience with Foothold EHR system preferred. Bilingual Spanish/English a plus.

Monitor, screen and oversee all clients entering the DIC and maintain building safety. Work closely with the Security & Clinical Staff to meet the needs of the clients. Monitor self-administration of medications, engages clients on a one to one basis as needed. Respond to mental health and medical needs. Responds to all emergencies as needed. Monitors house rules adherence by all clients. Experience working with the homeless, MICA, and/or substance abuse population needed.

Responsible for ensuring client safety, providing direct care services and maintaining building safety during all shift hours at our 24-hour transitional residence. Monitor self-administration of medications according to policy/procedure. Assist clients with ADL skills. Meal preparation/oversee kitchen use and supervise snack and meal periods/supervise client clean up. Oversee client activities, run socialization groups, ensure safety and security. Provide client transportation to program related activities as needed. Experience working with the homeless, MICA, and/or substance abuse population needed.

Oversee the clients on the unit. Conduct hourly rounds on the unit and work closely with staff to meet the needs of the clients. Monitor clients for adherence to policy/procedures. Monitor self administration of medication according to policy/procedures. Respond to client s health/mental health needs and emergencies as needed. Observe clients for changes in condition or behavior and promptly report these changes to the program director. Communicate and interact effectively and tactfully with the clients, visitors, families, peers, and supervisors. Complete records documenting care provided or other information in keeping with department policies. Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being. Attend to the individual needs of residents which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements. Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the Program Director such as lighting or equipment problems.

Includes maintaining current recreation contacts and development of new contacts on an ongoing basis. Make regular entries in staff communication log according to policy/procedure. Respond to client crisis as needed. Is responsible for all internal maintenance and housekeeping. Responsible for security of the premises at all times. Responsible for all direct care activities including meal preparation, assistance with showers and other personal hygiene activities as needed. Experience working with HIV+, homeless and/or substance abuse populations needed.

Client Care Safety Coordinator Part time Saturday & Sunday 3pm 11pm ---------------------------- On Call (on an as needed basis) Residential Services Empire State Supportive Housing Initiative HSD/GED F-80 /Fire Safety Coordinator Cert. required or must obtain certification within 3 months of hire Responsible for ensuring client safety, providing direct care services and maintaining building safety during shift hours at the site. Responsible for security and safety of the building including reporting and monitoring client activities in common areas of the building. Responsible for reporting repairs and maintenance need to building Maintenance Manager. Report all incidents using the agency incident reporting system as well as making sure all appropriate personnel are notified. Handle all emergencies; perform crisis intervention as needed, call 911 and other emergency numbers for back up as needed. Answer front desk telephone and take messages as needed. Direct visitors and handle all inquiries. Receive incoming mail and packages and distribute accordingly. CPR/AED certification preferred. NY State Security Certification preferred. Knowledge of homeless and substance abuse population preferred.

Clinical Supervisor Full time Monday Friday w/optional Saturday Flexible day hours Wellness & Recovery Masters Degree LCSW & Seminar in Field Instruction (SIFI) Certification The Clinical Supervisor will oversee and sometimes, conduct the assessment process with the clients. The Clinical Supervisor will provide supervision to the staff, including the Behavioral Health Specialists and the Peer Navigators. This supervision includes guidance and clinical support in addition to how to assess and manage other service needs that may emerge. The Clinical Supervisor is also responsible for the managerial aspects of the services which include tracking of staff time, assignments, reporting, and billing, as well as being the contact and liaison person for all referrals for services received. In addition, Project Hospitality is a strong believer in continuing education and growth in the field of social work. Each year we work closely with several Schools of Social Work and provide internship placement for our current employees and external students. The Clinical Supervisor would be responsible for providing the clinical supervision to students and employees placed in the agency and they will be the liaison between the agency and the graduate school. This would include development and submission of all plans and evaluations. Minimum of 6 months experience supervising employees and students required. Family Monitor Full time Monday Friday 3 pm 11pm Homeless Services Hospitality House HSD/GED Facilitates floor meetings and resident activities. Monitors families on floor and compliance with shelter rules. Documents interactions with families. Assist in distributing donations. Assist in the intake of new families. Responds to client health, mental health and family emergencies. Computer literate, Microsoft and internet required. Experience working with homeless a plus.

Fee For Service Counselor 10 hours a week Wellness & Recovery Clinical Services Recovery HSD/GED and CASAC or CASAC-T, Bachelors Degree Preferred Conducting groups for clients who are maintaining abstinence, discussing wellness activities, health education, stress reduction, issues of life priorities and addiction awareness. Other groups to be assigned as directed by Program Director. Conduct detailed alcohol and drug assessments. Determine immediate and long term service needs for the development of a comprehensive treatment plan. Conduct Intake Assessments which include entering all screening, assessment and intake data into AWARDS and accompanying progress notes within 24 hours of date services are rendered. Conduct individual sessions, as required, according to client s treatment plan and enter progress note into AWARDS within 24 hours of date session is conducted. Prepare intake assessments, screening results, progress notes and sign-in sheets for services rendered each day and for remittance of payments. Invoice for services rendered and submit weekly with required client services documentation. An understanding of the pharmacological dimensions of addiction and substance abuse. A theoretical knowledge of a client s psychological and physical needs and an understanding of group work practices and principles needed. Knowledge of the statutory benefits and support services available in NYS is needed. Knowledge of OASAS 822 outpatient program a plus. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.

Guide homeless clients in learning about permanent housing opportunities and assist in completing their housing applications on those clients identified for placement. Establish relationships with realtors and private landlords in order to meet the milestone objectives of the contract. Complete subsidized housing applications on all clients who meet the criteria for Section 8 and HRA housing programs in an expedited manner in order to meet and receive milestone objectives/ and payment incentive to Homeless Services. Provide advocacy and follow up on all applications submitted to Section 8 and HRA. Make referrals to special housing agencies for those with unique needs. Responsible for an assigned complement of bed sites. Accountable for seeing that all respite bed sites are clean, safe, comfortable, heated or cooled properly, and meet required standards for space and egress. Travels to all sites daily to ensure continuing and proper set-up of supplies. Complete, in a timely fashion, all necessary paperwork and daily logs. Assist with other driving needs as necessary. Experience working with the homeless and substance abuse populations. Outreach Coordinator Full time Tuesday Saturday 11am 7pm Transitional Services Youth Drop In HSD/GED Some College Preferred Engage youth in different neighborhoods on Staten Island to inform them of our program. Distribute literature, flyers and brochures regarding all program activities at the Youth Drop-In Center. Conduct at least one youth oriented workshop per month; and also support the youth consultant s workshops. Track and organize all outreach sign-in sheets for the Youth Drop-In Center. Conducts post-employment retention services.
