Case Manager 11am 7pm Homeless Services Hospitality House BA/BS Responsible for designing a social service plan in cooperation with new residents. Set and follow-up on long and short term client goals and objectives. Secure entitlements for clients and serves as an advocate for families in cases of conflict. Case Manager On Call (on an as needed basis) Wellness & Recovery PREP BA/BS Provides counseling and casework services to clients living with substance abuse histories in our PREP program. Meet with clients on caseload on a regular basis; Assist with client crisis; Oversee clients compliance in taking prescribed medication and maintain medication documentation; Make rounds of facility to engage clients as well as ensure client safety and document all findings in communication log; Oversee kitchen use and supervise snack and meal periods; as well as supervise client clean up; Maintain the charts of clients in accordance with OASAS and agency requirements; Participate in weekly Clinical Team Meetings. Experience working with the substance abusing population preferred, some case management experience needed. Case Manager 9am -5pm Housing & Support Services Scatter Site BA/BS Responsible for case managing clients in the Scatter Site Housing Program while assisting them with maintaining stable and permanent housing Design and implement individual service plans that reflect mutually agreed upon short and long term goals. Obtain and maintain all government benefits/entitlements and services for each client on caseload. Make appropriate referrals, such as, medical care, mental health services, vocational services, recreational activities, and substance abuse services. Conduct weekly home and/or office visits and hospital visits. Accompany clients to medical appointments. Manage medical and personal emergencies of assigned clients and make adjustments to care as needed. Conduct reassessments of service plans on a regular basis to ensure an ongoing awareness of the client s medical conditions and service changes needed. Maintain an accurate, confidential and current chart on all clients on caseload. Prepare monthly narrative report for assigned caseload on a timely basis. Attend regularly scheduled supervision meetings with Program Director. Experience in case management and working with HIV+ and homeless populations preferred. Case Manager 9am 5pm Residential Services HUD BA/BS Masters Degree Preferred Oversees housing units and provides case management services to tenants within the program. Completes intakes, along with pre/post placement assessments and re-assessments, as required. Develop service plans in conjunction with team members and tenants. Meet weekly with tenants, working to develop independent living, apartment management and neighbor relation skills. Assist tenants in accessing nutritious food, as well as in obtaining and maintaining entitlements. Assist with vocational training services and job placement assistance. Provide ongoing coordination with primary care physicians, mental health and other providers. Respond to tenants requests for information and referrals. Complete required documentation on a timely basis. Meet with supervisor on a weekly basis for supervision. One year previous related experience preferred.

Case Manager 11am 7pm Must work one Saturday per month Transitional Services Youth Drop In BA/BS Manage and counsel clients enrolled at the Youth Drop-In Center. Assist client with their service plan through one-on-one sessions and weekly group sessions. Determine any immediate need and link clients to appropriate services. Monitor engagement in services and escort clients to appointments as needed. Case Manager will work 4 days in the office and 1-day off-site that focuses on Community Outreach/ Linkage for the Youth Drop-In Center. Provide client crisis intervention, stabilization services, and referrals to Clinicians in Youth Services or agency clinics as needed. Provide ongoing care coordination around housing, savings, health benefits, employment, education, mental health and substance abuse. Care Coordination includes making referrals for services, monitoring service utilization, case conferencing with internal/external providers and follow up. Provide weekly workshops (topics are to include school, ADL skills, budgeting, employment, health awareness, diversity, LGBTQ related topics and housing retention). Make regular entries in staff communication log according to policy/procedure and ensure proper charting of all client information is done on a timely basis. Responsible to maintain up to date documentation including intakes, service plans, weekly progress notes, savings forms, and medical documentation. Participate in scheduled supervision and all team meetings. Attend 40 hours of training per year with a focus on youth services/lgbtq. Two years of case management/crisis intervention/social services/lgbtq work experience background is needed. Knowledge regarding High School Equivalency Exam/Diploma, housing services, entitlements, and training essentials for the LGBTQ population is needed. Must have excellent communication skills, both orally and in writing, with individuals/groups regarding complex and sensitive issues.

Observe clients for changes in condition or behavior and promptly report these changes to the program director. Communicate and interact effectively and tactfully with the clients, visitors, families, peers, and supervisors. Complete records documenting care provided or other information in keeping with department policies. Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being. Attend to the individual needs of residents which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements. Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the Program Director such as lighting or equipment problems.

Responsible for security and safety of the building including reporting and monitoring client activities in common areas of the building. Responsible for reporting repairs and maintenance need to building Maintenance Manager. Report all incidents using the agency incident reporting system as well as making sure all appropriate personnel are notified. Handle all emergencies; perform crisis intervention as needed, call 911 and other emergency numbers for back up as needed. Answer front desk telephone and take messages as needed. Direct visitors and handle all inquiries. Receive incoming mail and packages and distribute accordingly. CPR/AED certification preferred. NY State Security Certification preferred. Knowledge of homeless and substance abuse population preferred.

Clinical Supervisor 9am 5pm Flexibility required Residential Services HUD40 MSW & LMSW or LCSW Responsible for the supervision and oversight of HUD Case Management Team. Ensure all aspects of case management services are adequately & appropriately exercised and implemented. Overseas casework services performed by Case Management Supervisors and Case Managers. Collaborates with operations staff/supervisors to provide support of housing services to 40 scatter site residents/dwellings. Conduct staff meeting. Strong supervision skills required. Clinical Supervisor w/optional Saturday Flexible day hours Wellness & Recovery Masters Degree LCSW & Seminar in Field Instruction (SIFI) Certification The Clinical Supervisor will oversee and sometimes, conduct the assessment process with the clients. The CS will provide supervision to the staff, including the Behavioral Health Specialists and the Peer Navigators. This supervision includes guidance and clinical support in addition to how to assess and manage other service needs that may emerge. The Clinical Supervisor is also responsible for the managerial aspects of the services which include tracking of staff time, assignments, reporting, and billing, as well as being the contact and liaison person for all referrals for services received. In addition, Project Hospitality is a strong believer in continuing education and growth in the field of social work. Each year we work closely with several Schools of Social Work and provide internship placement for our current employees and external students. The Clinical Supervisor would be responsible for providing the clinical supervision to students and employees placed in the agency and they will be the liaison between the agency and the graduate school. This would include development and submission of all plans and evaluations. Minimum of 6 months experience supervising employees and students required.

Kitchen Aide 7am 3pm Homeless Services Hospitality House HSD/GED DOH Food Protection Certificate required. Assist in all necessary kitchen duties Assist in preparing meals and snacks. Set-up kitchen and cafeteria for meals. Serve meals to clients and assist with cleanup. Maintain the cleanliness of the kitchen and cafeteria areas. Ensure all supplies are available for use in kitchen and cafeteria areas. Experience working in the food services industry and/or with the homeless population needed. Peer Specialist On Call (on an as needed basis) Wellness/Recovery OASAS Diversion HSD/GED Peer Recovery Certification Works with outreach team; conducting street and community opiate intervention outreach activities. Conduct and monitor client enrollment. Provide peer mentoring services; conduct groups and train clients in Narcan administration. Escort clients as needed. Connects clients to support groups and health homes. Must have 1-year recovery time and experience with outreach and substance use disorder (SUD) clients.

Program Assistant 9am 5pm Transitional Services Health Homes HSD/GED with at least 3 years medical office experience Bachelor s Degree preferred Performs billing reconciliation in billing data bases monthly. Enter clients into billing databases & close old clients out. Perform reviews in one billing cycle to ensure core services were provided. Includes reading notes, checking encounter type with date, check cosigner etc. Perform reviews monthly on HML submission. Review HML submission dates with encounter type and encounter reference number. Run e-pace for clients Medicaid eligibility. Approve client as per eligibility logic. Track all clients internally and save in shared drive. Update billing database weekly and report back to Sr. Director of Administration. Perform other administrative tasks as assigned by Sr. Program Director, including but not limited to photo copying, scanning, ordering stationary for program, mailing, filing etc. Coordinate with outreach supervisor regarding enrollment and outreach performance census. Organize all opt-out for the month, tracks them in database. Coordinate with retention team for all clients in outreach mode for the month. Turn clients back to enrollment whenever instructed by retention specialist. High school diploma or GED with at least 3 years medical office experience, Bachelor s Degree preferred. Extensive knowledge of Microsoft excel required. Medical billing experience is preferred. Experience working with nonprofit agency is a plus.

Recreation Coordinator Part time 11am-7pm Homeless Services Carpenter House HSD/GED Responsible for providing individual/group recreational activities to residents to sustain and improve their physical/psychosocial functioning. Arranging recreational programs both inside and outside of facility. Coordinate van services for all recreational activities. Supervise learning enrichment programs for adults and children. Coordinate lecture schedule as arranged by Social Services Director. Act as liaison with agencies providing recreational services. Maintain log of educational and recreational activities and attendance. Experience in a social service setting. Some experience with the homeless population preferred. Needs ability to establish workload priorities and balance diverse projects. Must have exceptional communication skills both orally and in writing with individuals and groups regarding complex and sensitive issues or regulations. Registered Nurse Part time/consultant (4 hours per week) Wellness & Recovery Wellness Center BSN and NYS RN License Provide individual and group services in an outpatient mental health clinic. Review and sign all intake health screening documents for Wellness Program. Educate clients on medical and health issues. Monitor medication and medication documentation. Review laboratory test results and record in client s record. Conduct smoking cessation session for clients. Coordinate with Medical Director and Clinical Director as needed. Document all services in electronic health records system as required.

Registered Nurse On Call (on an as needed basis) Wellness & Recovery PREP BSN and NYS RN License Assess clients and analyze information obtained to develop appropriate interventions. Monitor the self administration of medications and treatments to residents. Assess clients upon admission to develop interim care plan and further assess in depth to contribute to the complete care plan. Update care plans as warranted and required. Attend and participate at Plan of Care meetings as scheduled. Respond to emergencies; assess the condition of the resident and take appropriate action; notify the physician when a change in condition occurs. Maintain client clinical files; keep charts updated. Correlate accurate and complete documentation and its impact on reimbursement and federal regulations. Screen, monitor and oversee all clients; Work closely with program administration and clinical supervision in the day to day treatment of clients. Directs visitors and handles all inquiries. Conduct rounds of premises, prepare reports, and notify supervisors of incidents. Handle all emergencies; perform crisis intervention as needed, calls 911 and other emergency numbers for back up as needed; try to work out stressful situations as appropriate. Report all incidents using the incidents reporting systems making sure all appropriate people are notified. Receives incoming mail and packages and gives to Admin. Assistant. On occasion will deliver packages as needed. Responsible for client, fire, and health safety. Oversee front desk and answer phones. Previous security experience required. Senior Maintenance Worker 9am 5pm With evening flexibility Residential Services HUD40 HSD/GED Oversee staff and distribute work orders as well as provide housekeeping and general maintenance activities for residence in our program. Clean all common areas on a regularly scheduled and as needed basis. Clean and prepare rooms for residents moving in or out of the facility. Perform light plumbing and electrical repairs. Make minor repairs to door locks, furniture, walls, ceilings and floors. Strip and buff all floors on a regularly scheduled basis. Arrange for garbage and trash storage and removal on a regularly scheduled basis. Keep all areas, including, but not limited to, resident rooms, common and storage areas, clean and fire safe in accordance with all regulatory requirements, codes and statutes applicable to the residence. Paint resident rooms as needed and assist the Resident Manager to establish and maintain a schedule for painting common areas. Utilize landscaping and other decorating activities with the residents and staff in a clinically appropriate manner. Remove snow and/or ice as required. Perform transportation activities as required and directed by the Resident Manager. Assist residents in acquiring and practicing appropriate housekeeping and/or maintenance skills, which will enhance their independent living skills, in collaboration with the clinical staff. 2 years previous maintenance experience, including painting and wax/stripping of floors Knowledge of HVAC, plumbing and electrical systems. Supervisory experience needed. Strong interpersonal skills required. Bilingual English/Spanish preferred. Ability to establish workload priorities.

Site Supervisor 9am 5pm Community Initiatives Single Stop BA/BS Responsible for overseeing daily operations of the Single Stop program and Single Stop Health Insurance Navigation program at the Project Hospitality help centers ensuring that the help centers are working toward the completion of all contract goals. Responsible for all work being recorded accurately in all program databases. Address the needs of staff & clients with Program Director. Responsible for assisting clients with public benefits screenings, enrollments, and referrals to other on-site and off-site services inclusive of health insurance navigation, legal and immigration assistance, tax preparation, sliding scale health care, financial counseling, other PH programs, and all Single Stop services. Responsible for assisting clients who are victims of domestic violence and overseeing their connection to other needed services. Ensure Single Stop and health insurance navigation staff are working toward the completion of all contract goals, and attending to staff and program needs in consultation with the Single Stop Program Director. Monitor outreach activities of staff and ensure all program activities are properly documented in program databases. Maintain partnerships with on-site and off-site providers, representing Project Hospitality at partnership meetings and trainings and supporting on-site Single Stop providers (Immigrant Justice Corp, Family Justice Center, Workforce One, Food Bank, network of other legal providers, etc). Must have 3 years of applicable experience in public benefits enrollment and working with vulnerable population such as victims of domestic violence and immigrant communities. Bi-lingual English/Spanish required. Must possess excellent communication skills, computer knowledge, and the cultural sensitivity to work with a broad range of clients.

Social Worker Part time 17.5 hours a week Variable days Homeless Services Breaking the Cycle MSW & LMSW Conduct formal intake interviews and make assessments and referrals as needed. Complete psycho-socials and social history when applicable. Counsel clients and maintain complete, accurate and up to date case notes and logs. Engage and assess homeless clients. Assist clients in completing entitlement forms as needed. Make referrals for various services, such as, food, clothing, bathing, psychiatric evaluations, medical evaluations, TB and HIV testing. Make referrals to other service areas within Project Hospitality. Participate in weekly supervision, weekly staff meetings and on-going case conferences. Transport clients to appropriate referral sites when necessary. Two years experience working with the mentally ill, homeless populations and substance abuse preferred. Experience coordinating educational workshops. Strong leadership qualities needed. Some supervisory experience required. Knowledge of DSM IV preferred. Bilingual Spanish/English preferred.
