Purpose : The Clinical Nurse Specialist will provide clinical expertise and case management for selected patients requiring breast reconstruction ensuring patient treatment is well planned and coordinated. Education and clinical teaching Assessment and management of patients referred to breast cancer or reconstructive services Provide and facilitate skilled care coordination and intervention for patients with breast cancer or patients who are considering/requiring reconstructive surgeries Actively work to reduce inequalities and the disproportionate burden of illness for Maori and PI populations Leads the nursing response in clinical care Provides range of Clinical Activity Sessions Clinical Nurse Specialist Undertakes audit and reports on key performance indicators for clinical service provision Develops a sustainable and high quality nurse-led component of Cancer Coordination Role models expert clinical practice, excellent communication skills and a variety of effective leadership skills Provides patient education to people with cancer disease, and to those considering Reconstructive surgery to improve patient outcomes and gain participation with their treatment Supports other staff in the management of people with cancer disease or those under the Reconstructive service. Acts as a nursing resource providing expert advice and education to nursing staff and health care professionals / family-whanau within the scope of Breast cancer/ Breast reconstruction nursing, demonstrating skilled mentoring/coaching, teaching and supervision. May teach at post registration level to enhance evidence based practice Initiates assessment and treatments according to bundles of care/pathways, liaising with surgical teams Initiates ordering of diagnostic tests in order to reach differential diagnosis according to best practice guidelines and evaluates outcomes Implements agreed care plan and treatments, using standing orders /protocols managing care to discharge/ transfer Manages practice and initiates changes to reduce barriers to discharge and self-care Follows up care outcomes in liaison with specialist services and general practice to reduce readmission The effective management of complex and challenging situations Diagnostic and monitoring function Administers and monitors therapeutic interventions and regimens Uses health promotion, education and self-care approaches to reduce health problems The teaching-coaching function Helping role Maintains a database of patients seen and includes demographics of ethnicity that can be audited Work with cultural support services such as Maori Health and Asian Health to improve outcomes for patients and tailor care appropriately Initiates care collaboratively with surgical and multidisciplinary staff.
