Holiday entitlement: 5 weeks per year pro rata Hours : Part-time 20-25 hours per week (Flexible) Accountable to : Clinically : General Practitioners Professionally : To patients under the UKCC (NMC) Code of Conduct Managerially : Practice Manager Key Tasks 1. To provide nursing services to the patients of the practice, including treatment, screening and advice within the professional competence of the post holder. 2. To provide appropriate care and treatment in conjunction with the GP,or independently, as appropriate, and according to practice policy, protocols, etc 3. To provide general and specific Chronic Disease Management following nationally agreed guidelines. To contribute to practice achievement in the Quality and Outcomes Framework and other local initiatives e.g. including INR management. 4. Maintain accurate patient records and statistics. 5. Administer immunisation and vaccination within agreed protocols for both children and adults. 6. Maintain infection control standards. 7. Liaise with other members of the primary health care team, social services and voluntary organisations to provide appropriate patients services. 8. Awareness of responsibilities for Health and Safety at work, personally and for colleagues and patients. 9. To be prepared to undergo additional training to develop skills and competencies as required by the practice or identified in the post holders Personal Learning Plan.
10. To be responsible for the regular and appropriate inspection, calibration, maintenance and replacement of equipment. This will include clear recording, systematic pre-planned schedules and reporting of faults. 11. To maintain confidentiality at all times. 12. To be willing to work at any time when the practice is open. 13. To be willing to work at a branch surgery and also visit patients at home as and when requested to do so. Any other tasks as requested/directed by the Doctors and/or Practice Manager from time to time. A more detailed description of many of the tasks required of this post is attached. In light of the practice commitment to training a tailored Nurse Induction Programme will be designed for the successful candidate to give information about the practice and to fill any training gaps identified.
Clinical Nursing Practice Venepuncture Undertake venepuncture following the referral/direction of a GP and opportunistically where appropriate Wound Management Perform wound care using aseptic technique. Undertake uncomplicated dressings, including leg ulcers. Suture and clip removal. Assist with minor surgery. Screening Undertake opportunistic screening- BP checks, height, weight, BMI, urinalysis, smoking status. Carry out New Patient checks. Health Promotion Recognise patient readiness to change in supporting their attempts to change unhealthy behaviour. Provide healthy eating advice to individuals or to groups, including the underweight. Provide smoking cessation advice to patients individually in opportunistic consultations. Provide individual advice to patients about increasing activity or exercise. Offer advice on recommended safe limits for alcohol consumption. Develop and maintain the Health Promotion material within the practice. Refer as appropriate. Vaccinations and Immunisations Administer injections under individualised prescription or Patient Group Direction including those for patients travelling abroad. Ensure safe storage, rotation and disposal of vaccines and drugs. Specific Vaccination Programmes eg Meningitis, annual flu vacs. Routine childhood immunisations and as required Ear Care Undertake ear irrigation after the patient has been assessed by GP or Senior Nurse. Advise patients about safe ear care. Mental Health Recognise and is aware of appropriate referral mechanisms for family violence, vulnerable adults, substance abuse and addictive behaviours.
Recognise psychological needs of patients presenting with depression or suicidal tendency. Anticoagulation Clinics To manage and facilitate anticoagulation clinics for patients, currently taking or, being initiated on warfarin treatment. To follow Derbyshire PCT guidelines for the level 4 anticoagulation management service. This involves: - performing a capillary blood test - using a computerised system to dose and schedule future monitoring - providing all aspects of patient education associated with warfarin treatment and management as well as lifestyle counselling - managing over-anticoagulated patients as per PCT guidelines - managing call and recall of patients and non-attenders - maintaining equipment and performing regular quality control checks - performing annual audits. Travel Health Advice Offer a holistic approach to travel health including comprehensive advice for patients prior to travelling including: - Vaccinations and medications - Malaria prophylaxis and bite avoidance - Safe sex - Food hygiene - Sun protection - First aid and emergency medication Infection Control Support patients and colleagues in adopting sound infection control measures. Apply infection control measures within the practice according to local and national guidelines. This includes : - Universal hygiene precautions. - The collection and handling of laboratory specimens. - Segregation and disposal of waste materials. - Decontamination of instruments and clinical equipment - Reporting and treatment of sharps injuries. - Dealing with blood and body fluid spillages. - Educate colleagues in adopting sound infection control measures. Child Health/Child Protection Recognise the signs, symptoms and categories of child abuse. Understand the importance of recognition, observation, documentation and communication. Is aware of child health procedures, statutory local procedures and points of referral.
Has knowledge of other practitioners and agency roles. Understand basic legal issues around child abuse. Emergency Management Is competent in anaphlaxis and resuscitation techniques. Care and Programme Management Men s Health Raise awareness and give basic advice on issues covering: - Testicular self-examination - Incontinence and prostatic disease. - Family Planning - Sexual Health - Make appropriate referrals Family Planning Advise patients on available methods of contraception and refer patients as appropriate. Support patients attending for emergency contraception and refers patients as appropriate. Perform interval health check for patients using hormonal contraception. Teach patients correct condom use. Advise patients on reducing risk of sexually transmitted diseases and refer patients appropriately. Women s Health Advise patients on national and local Cervical Cytology and mammography screening programmes. Undertake a programme of cervical cytology sampling under supervision until competent. Audit a minimum of twenty consecutive smears each year to demonstrate adequate sampling rates. Promote breast self-awareness, demonstrate breast self-examination and provide supporting literature. Advise patients about reducing the risk of sexually transmitted diseases and make appropriate referrals. Take swabs for microscopy as directed. Give patients basic advice about peri-menopausal and menopausal symptoms and make appropriate referrals. Advise on the increased peri-menopausal and post-menopausal risk of osteoporosis and heart disease. Offer health promotion/lifestyle advice to patients. Advise on basic incontinence and make appropriate referrals.
Diabetes Support patients in the management of non insulin dependent diabetes. Undertake opportunistic screening for the diagnosis of diabetes. Support and provide education for diabetic patients. Provide emergency treatment for hypo/hyperglycaemic emergencies. Make appropriate referrals. Contribute to the maintenance of the practice diabetic register. Respiratory Disease Management Has a basic understanding of asthma care; its diagnosis, asthma treatments and the principles of self-management. Use a spirometer to aid the diagnosis and the management of COPD. Know about the factors which contribute to the development of COPD. Contribute to the management of patients within the framework of national and local guidelines for asthma and COPD. Manage emergency treatment of asthma attacks. Advise patients on their asthma control including the use of self-management plans. Teach and advise patients how to use various inhalers and peak flow meters Contribute to the maintenance of the practice respiratory register. Make appropriate referrals. Cardiovascular Disease Contribute to the implementation of the National Service Framework. Contribute to the maintenance of the practice CHD register. Advise patients on lifestyle management and drug compliance for reducing the risk factors of: - Hypertension - Angina - Myocardial infarction - Cerebrovascular accident Clinical Practice Leadership Assist in clinical audit and the setting and monitoring of standards of care. Teach and direct others in clinical setting, including unqualified staff. Act as an assessor/mentor and teacher to other staff, Work effectively in the team.
