JOB PURPOSE To lead and develop Cardiac Rehabilitation Services in Secondary Care while coordinating an integrated service. 3. ORGANISATIONAL POSITION Lead Cardiac Specialist Nurse Cardiac Specialist Nurse Cardiac Rehabilitation /Specialist nurse This Post 1. SCOPE AND RANGE The post is for the cardiac specialist multidisciplinary team who provide a service in primary, secondary and tertiary care settings. The post holder will work closely with Managed Clinical Network for Coronary Heart Disease in the Borders The care of the Coronary Heart Disease (CHD) population of NHS Borders will be carried out in a holistic manner incorporating physical and psychological care to enable to live with chronic disease. The post holder will have a responsibility for the procurement of stock and supplies relating to cardiac rehabilitation. There will also be an opportunity to develop skills within cardiology chest pain service to be able to cover services when cardiac specialist nurses on study/leave etc..
2 2. MAIN DUTIES/RESPONSIBILITIES Undertake health promotion and provision of information for patients and carers. The post holder will provide a high quality cardiac rehabilitation service for cardiac patients from secondary and tertiary care founded on research findings and expert clinical practice. The post holder will Provide specialist advice and support for staff caring for and providing cardiac rehabilitation across NHS Borders. The post holder will act as a key contact for cardiac patients/relatives/other health providers following discharge. Provide, by working closely with other members of the multidisciplinary teams, a patient/carers focused approach to quality care. The post holder will maintain accurate patient records and statistics as required by the service and NHS Borders. The post holder will ensure phase one cardiac rehabilitation is delivered and coordinate referrals to primary care. The post holder will assist in the development and implementation of guidelines for cardiac rehabilitation. The post holder will co-ordinate the activities of the multidisciplinary team in primary, secondary and tertiary cares settings. The post holder will work in partnership with patients, public, statutory charitable and voluntary organisations for the benefit of cardiac patients. The post holder will participate in developing cardiac rehabilitation educational programmes for patients, carers and health and social care professionals ensuring that working knowledge is evidence based. The post holder will participate in local and national audit and research projects relating to cardiac rehabilitation. Potential development oppurtunity in chest pain/heart failure nursing services 3. SYSTEMS AND EQUIPMENT Proficient in using Electrocardiograph (ECG) machine, sphygmomanometer, stethoscope, scales, Height stick, Body Mass Index (BMI) calculation, Resuscitation equipment including defibrillator, dynamap. Handling of Sharps as per NHS Borders procedure. Information Technology, computer skills, systems and packages, MS Office, Outlook, Word, PowerPoint. Maintain accurate patient s records and evaluation as per NHS Borders guidelines.
3 7.DECISIONS AND JUDGEMENTS The post holder will make clinical and managerial decisions for patients allocated to them and communicate these with members of the multidisciplinary team. The post holder will have the ability to recognise situations where you require assistance and advice from senior staff. The post holder will recognise your own ability and limitations and identify these to your line manager. The post holder will work unsupervised as an independent autonomous practitioner across NHS Borders. The post holder will ensure that their practice is evidence based. The post holder will provide telephone support for patients, carers and the MDT as required. The post holder will be expected to organise their own workload and adjust according to the demands of the service. Adhere to NHS Borders policies and procedures and the Nursing and Midwifery Council (NMC) Code of Professional Conduct, or relevant professional bodies. 8.COMMUNICATION AND RELATIONSHIPS The post holder will work with multidisciplinary teams in primary, secondary, tertiary, patient care groups and managed clinical network where appropriate to achieve NHS Borders and personal objectives. The post holder will liase with the community multidisciplinary team ensuring a seamless transition between primary and secondary care settings. The post holder will develop good relations with the multidisciplinary team ensuring good communication and dissemination of information. The post holder will work in partnership with patients, public, voluntary organisations for the benefit of cardiac patients and carers. The post holder will maintain close communication with colleagues in the wider cardiac rehabilitation community. Chair the local heart manual facilitators meetings reporting periodically to the lead cardiac specialist nurse and the managed clinical network for Coronary Heart Disease. The post holder will maintain patient confidentiality at all times in accordance with NHS Borders guidelines. Ability to use various methods of communication with patients, carers, stakeholders of the MDT e.g. phones, fax, , clinical and written / correspondence to achieve positive outcomes in managing chronic disease.
4 Manage verbal and physical aggression as per NHS Borders guidelines. 9.PHYSICAL DEMANDS OF THE JOB Driving skills essential element of the job to assist in the progress of patients in the primary care setting working in partnership with multidisciplinary and multi agency teams. Moving and handling as per NHS Borders guidelines. Adopting the NHS Borders Personal Safety Advice at all times. Ensure that adequate consideration is given to health, safety and welfare of self, other staff, patients and visitors at all times, including reporting of clinical and non-clinical incidents in accordance with the NHS Borders policies. 10.MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Developing innovative approach to delivery of patient focused cardiac rehabilitation services. Ensure that equitable cardiac rehabilitation services are available across NHS Borders. In partnership develop and implement local guidelines for cardiac rehabilitation as part of the MCN for CHD priorities. Develop and implement guidelines in cardiac rehabilitation assisting in the development and provision of a comprehensive and menu based programmes for all CHD patients Borders wide. Achieving a balance between operational and development aspects of this complex role. Providing the opportunity for training and development of other health professionals involved in the delivery of cardiac services across NHS Borders. Ensuring that Local and National Standards are being met. High level of autonomy and clinical decision-making is required on a day-to-day basis.

KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB Qualification: First Level Registered General Nurse, Advanced Life Support (provider registered with Resuscitation Council UK) Experience: relevant qualified nursing experience in Acute/Cardiac nursing. Skills: Evidence of good interpersonal, communication and organisational skills. Personal qualities: Leadership, empathetic, compassionate, team player, cheerful and friendly. Research and Training: Relevant degree qualification or working toward a degree. Other: Car driver / own car 12.
