Purpose of the role : To coordinate a robust continuous quality improvement programme within the Department of Anaesthesiology to contribute to the efficiency and effectiveness of the service. To assist, advise and initiate research within the Department of Anaesthesiology: Ensuring WDHB quality standards & the Health & Disability Standards are complied with Compliance with WDHB & Anaesthesia policies Analysing trends from data collected from clinical indicators, audits, adverse events Recommending & implementing improvements to minimize risk Liaison with clinicians, complaints officer and legal advisor Ensure ANZCA Standards of Practice are followed and investigate deviation from recommendations. KEY TASKS Co-coordinates and reports audit activities within the Department of Anaesthesia Co-ordinates quality management issues within Anaesthesia or affecting Anaesthesia practice EXPECTED OUTCOMES Leads internal audit within anaesthesia Organise Morbidity and Mortality meetings Co-ordinate audit data to be reviewed and notify clinician involved Discuss areas of concern with Clinical Director Complete annual audit by WDHB Risk Manager Development & monitoring of clinical indicators Develop departmental polices from incident reports to prevent recurrence Investigate issues or near miss incidents and correspond with appropriate authorities Investigating complaints or serious incidents relating to Anaesthesia An open, unbiased and transparent process is utilised A Root Cause Analysis framework is used for incident investigation / complaint review A Severity Assessment Code is applied to all incidents Liaison and consultation with Legal Advisor, Complaints Officer and other key persons is evident Recommendations are developed for resolution and discussed with Clinical Director Recommendations are actuated and followed to satisfactory resolution and revisited to ensure non reoccurrence Completes report which is articulate, clear and identifies the processes used and the evidence obtained to recommend particular outcome Peer review process is used as part of incidents / complaints the Director of Nursing & Midwifery. 6/06/2017 Page 2
