Mental Health Nurse - Credentialed Position reference Position type Classification Remuneration Service Area/division/state Effective date 18806 Part time fixed term until 28 June 2019 working up to 30.4 hours per week Community Mental Health Practitioner (CMHP) Level 5/4 $83,974 per annum pro rata plus 9.5% superannuation, salary packaging and shift allowances Integrated Complex Care - Dandenong SE NDIS & CBS/South East Victoria/Victoria October 2017 This position will work within the guidelines of the Integrated Complex Care (ICC) Service to enable, facilitate and coordinate care planning for eligible consumers. Integrated Complex Care (ICC) services support consumers with severe mental illness who have complex needs and will benefit from improved coordination of their care over an extended period of time. They are available for population groups that are harder to reach or are underserviced, and cannot currently access National Disability Insurance Scheme (NDIS) or Partners In Recovery (PIR). The four core elements are: Clinical nursing services Family support and liaison Care coordination and liaison (clinical and non-clinical services) Improving access to psychiatrist and psychological care ICC services are focused on enabling, facilitating and coordinating delivery of the Consumer Care Plan.
Service Delivery In line with Mind s Model of Recovery Oriented Practice Framework and the ICC operational guidelines, mental health nurses engaged under the ICC will work closely with psychiatrists and/or GPs to provide coordinated clinical care and treatment for people with severe and persistent mental disorders Services will be provided in a range of settings, such as in clinics or at a consumer s home Mental health nurse functions will include, but are not limited to, the following: 1. Provide clinical nursing services for consumers with severe mental illness: - Establish a therapeutic relationship with the consumer - Provide clinical care and treatment planning and ensure adherence to treatment plan - Liaise closely with family and carers as appropriate - Regularly review the consumer s mental state - Administer, monitor and ensure compliance by consumers with their medication - Provide coaching to promote general health & well-being - Provide psycho-education as appropriate - Monitor and provide information on physical health care to consumers 2. Coordinate clinical services for consumers with severe mental illness: - Maintain links and undertake case conferencing with GPs, psychiatrists and allied health workers such as psychologists (health professionals may be eligible to claim case conferencing items under the MBS) - Coordinate services for the consumer in relation to GPs, psychiatrists and allied health workers, including arranging access to interventions from other health professionals as required - Contribute to the planning and care management of the consumer - Liaise with relevant non-clinical supports as appropriate - Contribute to the achievement of consumer service delivery targets as directed by the Manager - Support broader psychological services and programs - Coordinate /Participate in case conferencing, case coordination and case reviews - Work collaboratively with consumers and their families and natural supports providing information education and skills training as appropriate - Assist with the promotion and implementation of the ICC program Team Work Work effectively and collaboratively with Mind colleagues, ICC Manager and service partners, in accordance with the values of Mind Support colleagues with recommendations and referrals if considered necessary and appropriate for the clinical needs of a particular consumer With consumer consent, share relevant information with primary care and clinical mental health colleagues Actively and respectfully participate in team meetings, professional development opportunities and quality improvement processes, when relevant to employees practice Maintain effective multi-agency and multi-disciplinary links and accountabilities to ensure effective and efficient follow up for consumers Data Management Ensure that all consumer file information is entered in a timely manner Meet all data entry requirements for Mind and the South Eastern Melbourne PHN Provide clear, concise and timely documented evidence of clinical interventions Complete and submit contract data as per funding agreements and maintain a high standard of record keeping and other relevant documentation within defined time frames Use the agreed record system and formats required for evaluation purposes Ensure accurate records for billing and reporting purposes Meet all other administrative and reporting requirements to ensure a high level of efficiency Professional Development Undertake regular line management and professional supervision, appropriate to the primary work of the service and Mind Participate in reflective practice Workplace Health & Safety Monitor and assess risk Contribute actively to the maintenance of a safe workplace Ensure all safety issues are reported and addressed as they arise Accountability Proactively comply with direction from the designated line Manager and other managers within Mind Use all Mind resources in line with organisational policy
The position reports operationally directly to the Service Manager. The ICC clinician will participate in regular professional supervision with their relevant professional group supervisor. Core Requirements Tertiary qualifications in Mental Health Nursing. Credentialed in the field of mental health nursing Current valid Driver s Licence Current National Police Record Check Current Working With Children Check (Employee) Not listed on the Disability Worker Exclusion Scheme list 1 Knowledge Demonstrated advanced ability in the provision of mental health risk assessment, crisis planning, relapse preventions, recovery planning, brief psychological therapy techniques and assertive care coordination for people experiencing mental distress and suicidality 2 Attribute Demonstrated competency in understanding relevant legislation pertaining to mental health 3 Attribute Ability to work with a high level of autonomy and exercise professional judgement in decision making and clinical practice 4 Value Ability to work with in a multi-organisational team while maintaining the interests of Mind 5 Knowledge Knowledge of current psychiatric diagnostic and classification systems and assessment tools 6 Attribute Strong interpersonal and team skills and behaviours, incorporating verbal and written communication, flexibility, resilience
This position is part time fixed term working up to 30.4 hours per week until 28 June 2019. Working days will generally be Monday to Friday worked between the hours of between the hours of 8am to 6pm as required. Afternoon, evening or weekend shifts may be required to be worked by the incumbent. Please note that the work pattern may be subject to change to meet the needs of the service. You are able to salary package a tax free amount, which is currently up to $15,900 per FBT year, plus other items including meal and entertainment and accommodation. The amount available for salary packaging will be subject to Commonwealth legislation as it applies from time to time and on the basis that Mind will not incur any liabilities for FBT. Mind offers salary packaging through an outsourced provider (currently RACV Salary Solutions). Other conditions are as per Mind s industrial agreement. Employer superannuation contribution will be paid according to the legislative requirement. Appointment is subject to a six month probationary period of employment.
