Responsible to: Direct reports: Key responsibilities: Primary Purpose: Clinical Team Leader Frankston Access & Clinical Team NIL Intake phone referral screening, assessment and follow up pathway support. This position will be part of the team responsible for co-ordinating the clinical delivery of treatment and care within headspace Frankston for young people aged 12 25 years. Referrals to the service are primarily by telephone in addition to walk in referrals from young people, their families and friends and therefore consultative and therapeutic timely response is required for this service. Intake Clinician will ensure that the care delivered to the young people is of the highest possible standard, reflects contemporary therapeutic practice and that young people and their families receive timely, responsive and suitable services to meet their needs. In addition, the Clinician must be energetic and enthusiastic and capable of working within headspace Frankston youth hub model to deliver services within the community and enhance health care access for the target age group. 1

Key Responsibilities Referrals, Initial Screening Assessment & Review Provide a first point of contact and conduct initial telephone screening with young people to engage them, identify needs and risks and triage responses accordingly. Provide clinical advice and support to youth workers who support and screen young people who walk in and assist with links to support services for families. Follow guidance from the Access team Leader regarding assessment scheduling and follow-up for young people accessing the Youth Access Team and Primary Health Team to ensure best use of resources to meet needs. Provide bio-psychosocial assessments, initial care plans and pathway support for young people addressing needs and risks, in a shared decision making approach with the young person and their family (as appropriate). Provide and /or ensure access to Single Session Family Consultations for families when appropriate. Assertively monitor young people s mental health and wellbeing as required. Complete service activity and outcome measures as per service protocol Assessments are completed with attention to needs and risks. Appropriate internal and external referral pathways followed for young people and their families Activity and clinical data collected for the HAPI and SEMPHN databases and other data as required Work plan activities are delivered Communication and escalation processes for risk management are followed Monitor caseload and present as required to clinical review and handover meetings. Maintain safe practice at the outreach sites 2

Support and consultation Provide mental health related information, psycho-education for mental health and drug and alcohol problems, information on services available and how to access these. Provide secondary consultation for referrers, co-located services and in-reach service providers to assist with appropriate care pathway options. Effective and timely assessment and early interventions are provided for young people and their families. Advocate on behalf of young people with other workers, schools, employers to enhance service access. Foster a co-operative team environment which values collaboration to ensure a focus on efficient systems and client needs. Provide and/or ensure access to care coordination as required Communication and escalation processes for risk management are followed Linkages developed and sustained with referral services Continuous Improvement Contribute to continuous quality improvement in relation to service delivery. Developing linkages with referral services. Participation in evaluation and ongoing monitoring of the program. Contribute to Continuous Quality Improvement Activities to support the development and monitoring of the team s work Include clients in the evaluation of the service where appropriate. Collaborate with other support services and community based activities to integrate support and provide optimal service provision. Participation in accreditation processes linked to the headspace Model Integrity Framework and the National Standards for Mental Health Services 3
