Independent Licensed Clinician Office- Based (EBCAP0365) East Bay Community Action Program (EBCAP) has a full time opening for a compassionate, enthusiastic, and skilled individual to provide office-based services at our expanding Behavioral Health Services Division at the 6 John Chafee Blvd. location in Newport, RI. Join our growing team and enjoy the dynamics of a varied caseload. The Clinician will work collaboratively with the Health Center Care Team members on care management plans, interventions, and treatment goals including self-management goals. The successful candidate will provide brief screening, such as SBIRT (Screening, Brief Intervention, Referral to Treatment) Model for a portion of the work week, to identify potential behavioral health clients and build his/her caseload. The Clinician will manage treatment issues to ensure the suitable level of care and appropriately assess risk and work through immediate crisis. The ideal candidate will provide treatment planning, coordination of services, continuing evaluation of mental health and/or addiction disorders, outreach, and discharge planning for his/her assigned caseload. Rhode Island independent licensure (i.e., LICSW, LMHC) is required. Dual licensure with LCDP is a plus. This position is Monday through Friday, however, either an occasional evening or Saturday morning may be required. Bilingual Spanish or Portuguese preferred. Independent Licensed Clinician KIDS CONNECT (EBCAP0400) East Bay Community Action Program (EBCAP) is seeking a full time Independent Licensed Clinician for our Kids Connect Program at 8 John Chafee Boulevard in Newport, RI, which is the main facility of our Head Start/Early Head Start program. The child/family-focused clinician will assess and create holistic, therapeutic, integration assessments, and create and implement classroom plans for children eligible for the Kids Connect Program funded by the RI Department of Human Services. BEHAVIORAL HEALTH CARE MANAGER/CMT (EBCAP0358) East Bay Community Action Program (EBCAP) is seeking a full time Behavioral Health Care Manager for our new Community Health Team (CHT) located at 6 John H. Chafee Boulevard Newport, RI. We are seeking a highly professional and motivated individual who enjoys working with patients in a community health setting. Join our expanding dynamic team and work with health care workers in primary care practices in Newport County participating in Rhode Island Care Transformation Collaborative and provide multidisciplinary team based care to referred patients. Due to the importance of this health team to our Community, EBCAP is offering a sign on bonus of $800.00. The Behavioral Health Care Manager is a member of the CHT working with health care members to support the overall health needs of the patients serviced by CMT, including but not limited to behavioral health and social service determinants. The ideal candidate will work with the Nurse CHT Manager, Community Health Workers and Nurse Care Managers to support the behavioral health and social needs of the patients. The successful candidate will provide home, community and office based screenings and assessments, referrals, and care coordination as well as short term supportive counseling, utilizing evidencebased strategies for patients who require behavioral health services including but not limited to substance abuse and addictions services. Community health workers (EBCAP0362) East Bay Community Action Program (EBCAP) is seeking three full time Community Health Workers for our Health Center located at 6 John Chafee Boulevard in Newport, RI. The Community Health Workers will work in partnership with members of a multidisciplinary team comprised of health staff, behavioral health master level clinicians and primary care staff. This team will promote patient-centered care by screening to identify alcohol and substance abuse issues. The successful candidates will access patients dealing with complex medical, behavioral health and/or substance abuse who require a more intensive home and community-based intervention by offering intervention, resources and referrals, support, care coordination and ongoing case management. Due to the importance of this health team to our Community, EBCAP is offering a sign on bonus of $600.00. Candidates must be committed to providing behavioral health care coordination and disease management education and work collaboratively with medical staff and community service partners. The successful candidates must have either a minimum of an Associate s or Bachelor s degree in a social science, research or public health-related field. Candidates must be certified through the RI Certification Board (RICB) as a Community Health Worker or they must meet the requirements to sit for the CHW certification examination within six months. They must have a minimum of one year work experience in substance abuse setting, health coaching, motivational interviewing and/or related field.
