BACKGROUND TO THE POST PRIMARY AND SECONDARY INTERVENTION SERVICES (PSIS) Age UK Bromley & Greenwich has been recently commissioned by London Borough of Bromley and Bromley Clinical Commissioning Group to deliver personalised support to Bromley s older and adult residents. This is an exciting opportunity to be part of a refreshingly new approach to supporting people who may have needs as a result of having long-term health conditions, age related fraility, learning disabilities, mental health and physical disabilities. This would also include support for carers including younger carers and assisting adults who may be experiencing barriers to employment and training. This innovative project will deliver new choices for local people to help them maintain their health and wellbeing as well as their independence. Occupational Therapist August 2017 Page 3 of 13

Job Description Job Title: Reporting to: Responsible for: Occupational Therapist Health & Wellbeing Manager Handyman JOB PURPOSE: As a key professional of Age UK Bromley & Greenwich Pathways Teams for support to Elderly Frail, Younger Adults with long-term health conditions and their Carers provide a specialist community based Occupational Therapy service to eligible citizens of London Borough of Bromley. Working closely with Integrated Care Networks, Care Navigators, GP s, Health & Social Care professionals and other partners to identify those at risk and who may be close to crisis point. Through personalised OT interventions reduce, delay and prevent reliance on statutory services and facilitate accessing non-statutory and universal support services. Main Duties and Responsibilities Receive, screen and prioritise referrals received from Single Point of Access Team (SPA) of clients who are close to crisis point under Level 4/5 Rockwood Frailty scale. Manage clinical and professional aspects of complex casework with Elderly Frail, Younger People with Long-term Conditions and their Carers. Carry out person centred occupational therapy assessments and design evidence based OT interventions, personalised support planning and reviewing. To be professionally and legally accountable for all aspects of own clinical and professional work and people for whom they provide professional support and supervision. To independently manage casework, where appropriate, requiring specialist standardised and non-standardised OT assessment of adults and frail elderly with a range of complex needs and multi-pathologies; using clinical reasoning, analytical and communication skills to determine support needs. To undertake a range of risk-assessment and action planning functions incorporating safeguarding, capacity and positive risk-taking principles, including specialist manual handling risk assessments, adaptation/equipment and assistive technology and housing needs assessments. Provide or broker the delivery of a range of outcomes identified with people and their families/carers including provision of a broad range of assistive technologies (community/health equipment), environmental redesign solutions, specialist advice and information To co-ordinate and provide relevant input into multi-agency liaison, planning and safeguarding meetings to ensure integrated, effective working and clear communication arrangements. This will include feedback communications and discussion with the supported person and their family/carers. To be responsible for the maintenance of professional/ethical standards including timely, accurate and comprehensive documentation in line with Health and Care Professions Council (HCPC), College of Occupational Therapists (COT) and local standards of practice for self and supervised staff (where appropriate) To be familiar with key legislation such as Care Act 2014, Housing Grants Construction and Regeneration Act 1996, Mental Capacity Act 2006, Children and Families Act 2014 and other relevant Acts. To utilise a variety of verbal and written communication mechanisms with colleagues and external partners to ensure collaborative, multi-agency working ensuring adherence to information governance and sharing protocols and preservation of confidentiality To frequently use a range of verbal and non-verbal communication skills to communicate sensitively and empathically with people and their families/ carers to progress all aspects of their support Professional/Supervisory To be responsible for supervision and work allocation to the Handyman and with Health & Wellbeing Manager work towards offering placements for OT Students. Carry on average an active case load of 30 OT cases. Record timely case notes of contacts with clients, carers and other professionals, assessment reports and support plans. OT assessments and support plans must evidence they are person centred and personalised, strengths and community assets based and cost efficient. Be actively involved with professional clinical and peer groups; COT specialist sections, and other professional developmental activities within the service. To be responsible for the maintenance and development of own competency to practice and maintain professional registration, taking responsibility for ensuring own knowledge and skills are up to date by engaging in continuous professional development (CPD) and lifelong learning. General portfolio (in line with HCPC requirements), which reflects development of knowledge, skills and evidence-based practice. TERMS & CONDITIONS OF EMPLOYMENT Salary Pension Hours Annual leave Based at: 33,000 per annum pro rata Age UK Bromley & Greenwich does not have its own Pension Scheme. On completion of the six-month probationary period, staff are eligible to join the Pension s Trust (employer s contribution 1% + minimum 1% personal contribution) 21 hours per week Normal working week is Monday to Friday 9:00 till 17:30 with up to one hour unpaid lunch break. The nature of the role is such that some flexibility and additional hours may be required from time to time, including some evening or weekend work. Time off in Lieu will be granted for extra hours worked in the evenings or weekends. Overtime payments are not normally made. 27 days per annum (2 days allocated over the Christmas Period). + 8 public holidays pro rata The post holder will be primarily based at Community House, Bromley. They may be required to travel to other parts of the boroughs Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
