1. What is Chse Pharmacy? Chse Pharmacy is a web-based applicatin which has built upn and re-branded the existing Cmmn Ailments Service (CAS), intrducing a new mdule called Discharge Medicines Review. This Chse Pharmacy applicatin will enable accredited Cmmunity Pharmacies and Cmmunity Pharmacists t: Prvide cnsultatin and treatment fr a defined list f 26 cmmn ailments (CAS); Access an electrnic Discharge Advice Letter (DAL) generated by the Natinal Welsh Clinical Prtal (WCP) and Medicines Transcribing and electrnic Discharge (MTeD) system, t supprt an electrnic Discharge Medicines Review (DMR). 2. Wh develped the Chse Pharmacy applicatin? The Chse Pharmacy applicatin was develped by the NHS Wales Infrmatics Service (NWIS) in partnership with Welsh Gvernment, Cmmunity Pharmacy Wales (CPW), Ryal Pharmaceutical Sciety (RPS), Health Bards and NHS Wales Shared Services Partnership. The prject adpted a user-centred design prcess with frnt line users frm acrss Wales and has been designed and develped accrding t their requirements. 3. Wh can use the Chse Pharmacy applicatin? Only accredited Cmmunity Pharmacies and Cmmunity Pharmacists can access and use the Chse Pharmacy applicatin. This includes lcum cmmunity pharmacists. Cmmunity pharmacists requesting access t the applicatin are required t have signed an acceptable use statement. Further details f hw yu can request access t the Chse Pharmacy applicatin are available by cntacting Shared Services n 01792 607214. Pharmacists must be accredited t prvide the relevant services available within the applicatin. Further details f the accreditatin prcess fr services are available frm www.wcppe.rg.uk. 4. Des Chse Pharmacy integrate with the Patient Medicatin Recrd (PMR)? N. There is n planned integratin between the Chse Pharmacy applicatin and the Cmmunity Pharmacy Patient Medicatin Recrd (PMR). These are t be treated as separate applicatins. 2

5. Hw d I create a Chse Pharmacy user accunt? New users are required t cmplete a pharmacist listing frm which can be prvided by Shared Services. Please cntact Shared Services n 01792 607214 t request a frm. When cmpleted, these frms can be submitted via: Email: awpd@wales.nhs.uk Pst: Cntractr Services, NWSSP PCS, The Oldway Centre, 36 Orchard Street, Swansea, SA1 5AQ. Fax: 01792 607394 These details will then be added t the All Wales Pharmacy Database (AWPD). Once added nt the AWPD, an apprval letter will be sent advising the cmmunity pharmacist f their NHS Windws lgin details and NHS email accunt. On receipt f their apprval letter, the cmmunity pharmacist must cntact the NHS Wales Primary Care Service Desk n 0333 200 8048, quting the incident number prvided in the apprval letter, t receive their NHS email passwrd. The same passwrd is used t lg int the Citrix sessin. Once in receipt f their NADEX/NHS email passwrd, users shuld access the Chse Pharmacy applicatin and select the sign up ptin n the Pharmacist Sign in page. Within this screen yu will create a Chse Pharmacy passwrd. Yur Health Bard service delegate will then receive an email t apprve the apprpriate access cntrls t yur accunt. 6. What infrmatin will be prvided in my Chse Pharmacy apprval letter? Cmmunity Pharmacists will receive an apprval letter when their applicatin t access the Chse Pharmacy applicatin has been received and apprved. In the apprval letter, users will receive: Display name: The user name which will be displayed in the Chse Pharmacy applicatin. E.g. Lindsay Jnes NHS Windws lgin (NADEX): This is used t lg n t Citrix and a user s NHS Wales email accunt via webmail. E.g. LI000305 NHS Email address: User s NHS email accunt. Fr example, GPHC0123456@wales.nhs.uk. This will be used t lg int the Chse Pharmacy applicatin. Prxy Accunt: Linked t GPhC email accunt t enable users t search fr yu n the NHS email glbal address list. Incident number: This is required fr users t receive their Windws lgin (NADEX) and NHS email passwrd frm the Primary Care Service Desk (PCSD). 3

A user s NHS email passwrd will nt be prvided in this apprval letter. In rder t btain passwrds, please cntact the Primary Care Service Desk (PCSD) n 0333 200 8048 and qute the incident number detailed in the apprval letter. 7. D I need t cmplete any training? N. Training can be prvided by service delegates n request. Training dcumentatin will als be prvided t cmmunity pharmacies. A WCPPE e-learning mdule fr the Cmmn Ailments Service which includes infrmatin abut infrmatin gvernance and system use is available http://www.wcppe.rg.uk/learning/learning-resurces/cmmn-ailmentsscheme 8. Hw will I receive a cpy f the Discharge Advice Letter (DAL) frm the hspital? Electrnic Discharge Advice Letters (DALs) generated by the natinal Welsh Clinical Prtal (WCP) Medicines Transcribing and e-discharge (MTeD) applicatin will be made available within the applicatin t supprt a DMR. Instances where an electrnic DAL is nt available, cmmunity pharmacies will be reliant n hspitals psting, faxing r prviding a cpy t the patient. 9. Why d I receive a blcked cntent message when trying t access the Chse Pharmacy web link and/r links within the applicatin? Depending n cmputer settings, internet brwser settings may prevent websites and webpages frm displaying cntent. All web links need t be added t a cmmunity pharmacy s internet white list. Please cntact yur IT supprt team t add the Chse Pharmacy applicatin link and its embedded links t yur internet white list. 10. Can patients register with separate Cmmunity Pharmacies t receive the CAS and DMR service? Yes. Patients can separate their registratin fr the DMR and CAS service acrss different cmmunity pharmacies. 11. Hw can a patient register fr the DMR service? A patient can register with a cmmunity pharmacy fr the DMR service in hspital via the WCP MTeD applicatin r by presenting at a cmmunity pharmacy after discharge frm hspital. 4

Patients can als be apprached and registered fr the DMR service prir t any hspital admissins. 12. Why can t I find my patient? The Chse Pharmacy applicatin perfrms a patient search against the Welsh Demgraphic Service (WDS). The WDS maintains a register f Welsh residents demgraphic details and their GP details. If the patient is nt n WDS, they are unable t use the Chse Pharmacy applicatin fr the purpses f receiving a DMR. 13. Wh shuld I cntact if my patient s Welsh Demgraphics Service (WDS) details are incrrect? In the event a patient s WDS details are incrrect, please advise the patient t cntact their GP t update their recrd. 14. What infrmatin is presented in the electrnic DAL within the Chse Pharmacy applicatin? Fllwing engagement and agreement with CPW, RPS, Health Bards and the Wales Infrmatin Gvernance Bard (WIGB), the infrmatin made available in the electrnic DAL t supprt the DMR service nly includes: GP and Patient Details; Admissin and Discharge Details; Knwn Allergies; Medicatins at time f discharge; Medicatin stpped during hspital admissin including name and reasn. 15. Why can t I view an available electrnic Discharge Advice Letter within the Chse Pharmacy Applicatin? The electrnic DAL is presented using Adbe Acrbat PDF reader. Withut this sftware being installed n yur cmputer, it will nt be pssible t view the DAL r previusly cmpleted DMRs. 16. Can I use Chse Pharmacy t cmplete a discharge medicines review if an electrnic DAL is nt available? Yes. The DMR service within the Chse Pharmacy applicatin is nt slely dependent n an electrnic DAL being available. 5

Only thse DALs generated by the natinal Welsh Clinical Prtal (WCP) Medicines Transcribing and electrnic Discharge (MTeD) system will be made available electrnically within the Chse Pharmacy applicatin. In instances where a Health Bard is yet t implement the natinal MTeD system, the patient will be prvided with a paper cpy f their DAL n discharge frm hspital. The cmmunity pharmacist can then cnduct the electrnic DMR within the Chse Pharmacy applicatin using the paper frm instead f using the MTeD electrnic DAL. Hwever, this means that cmmunity pharmacists will need t re-enter medicines infrmatin int the electrnic DMR frm. 17. At what pint is the DMR cmpleted in the Chse Pharmacy applicatin? The DMR is cmpleted nce part 2 infrmatin has been entered int the electrnic DMR frm. It is nt pssible t submit a DMR until part 2 has been cmpleted. Once entered, the electrnic DMR frm can be submitted and the status is updated t cmpleted. All cmpleted DMRs can then be reviewed under the DMR Histry tab. 18. Can I share any discrepancies identified in DMR Part 1 with a patient s GP thrugh the Chse Pharmacy System? Any discrepancies identified in the electrnic DMR frm cannt be shared r sent t a patient s GP via the Chse Pharmacy applicatin. Hwever, cmpleted DMR frms can be printed. 19. Hw is explicit cnsent sught and recrded fr the DMR service? Explicit cnsent is required frm the patient t undertake a DMR within the Chse Pharmacy applicatin. The patient will be invited at r befre the pint f discharge frm secndary care, t cnsent t share the medicines in their DAL with a nminated cmmunity pharmacy fr the purpse f receiving a DMR nly. If the patient cnsents, this will lead t an electrnic ntificatin being sent t the nminated cmmunity pharmacy using the ENAS Alert Service. This alert will nt cntain Patient Identifiable Infrmatin (PII) but will infrm the cmmunity pharmacy that a patient has nminated them t receive infrmatin abut their medicines fllwing discharge frm hspital. 6

Once the ENAS alert has been received by the cmmunity pharmacist, he/she may then lg int the Chse Pharmacy applicatin t view patient demgraphic data nly t establish cntact with the patient t schedule a DMR. The cmmunity pharmacist must btain explicit cnsent befre viewing a patient s DAL. This cnsent is recrded in the Chse Pharmacy applicatin. 20. Is access t Chse Pharmacy audited? Yes. Access t Chse pharmacy is recrded and audited. Once registratin requirements have been met, the user will then be prvided with rle-based access t the system and given their user name and passwrd credentials. All subsequent attempts t enter the system are lgged by the AC3 (accunt cntrl 3) auditing prtcl that is in place. The Data Prtectin (1998) and Cmputer Misuse Acts (1990) cver data in this applicatin where unauthrised access t this system is unlawful under UK law. Users must nt share their username r passwrd with anyne. Unauthrised access t this applicatin and the services prvided will be investigated and the ffenders prsecuted. 21. Can patients pt ut frm receiving services prvided by the Chse Pharmacy applicatin? Yes. Patients can pt ut f the service at any time. T d this, the cmmunity pharmacist can de-register the patient frm within the applicatin. Equally, it is pssible fr a patient t transfer their registratin t anther cmmunity pharmacy. At this pint, all histrical cnsultatins and reviews will n lnger be made available t the previusly registered cmmunity pharmacy. 22. Can patients pt back int the services prvided by Chse Pharmacy? Yes. Patients can pt back in at any time with any f the Chse Pharmacy pathfinder sites ffering the specific service they require. 23. Wh shuld I cntact t reset my Chse Pharmacy passwrd? Cmmunity Pharmacists can select the frgtten passwrd link n the Chse Pharmacy sign in page. An email will be sent t the user s NHS Wales email accunt advising them f a new passwrd. If the user des nt have access t an NHS Email accunt, they must lg a call with the Primary Care Service Desk n 0333 200 8048 (Mnday t Friday 8am - 6pm) r primarycare.servicedesk@wales.nhs.uk. 7

24. Can I change my passwrd within the Chse Pharmacy applicatin? Yes. Passwrds can be changed within the applicatin by selecting the Prfile tab n the hme page and selecting the Change Passwrd ptin. 25. Why has my Chse Pharmacy accunt been lcked? Users have three attempts t enter their Chse Pharmacy passwrd. After the third failed attempt, a user s accunt will be lcked. 26. Hw d I unlck my Chse Pharmacy accunt? If an accunt has been lcked, an autmatic email is sent t yur Health Bard s service delegate. In the event yu have frgtten yur passwrd, please cntact the Primary Care Service Desk n 0333 200 8048 (Mnday t Friday 8am - 6pm) r primarycare.servicedesk@wales.nhs.uk. 28. Hw d I claim payment fr the services prvided? Cmmunity pharmacies will receive payment fr bth cmpleted Cmmn Ailments cnsultatins and Discharge Medicines Reviews (DMRs) n a mnthly basis. If a DMR has been cmpleted using the Chse Pharmacy applicatin, cmmunity pharmacists d nt need t enter claims directly int the Natinal Electrnic Claim and Audit Frms (NECAF) database. Data will be extracted by the Chse Pharmacy applicatin supprt team and shared with NHS Wales Shared Services Partnerships fr entering int NECAF. 29. Wh shuld I cntact if I have questins r cncerns abut Chse Pharmacy? Shuld users have any cncerns r questins reading the Chse Pharmacy applicatin and the services it prvides, they shuld cntact their Health Bard service delegate: 8
