A Guide to Our Services

Welcome to Saint Francis Hospice At Saint Francis Hospice, we are dedicated to providing people with a life-limiting illness the high quality and expert care and support they deserve, as well as ensuring that family, friends and carers also have the guidance they need. The hospice is staffed by a multi-professional team including doctors, nurses and physiotherapists and we work in close partnership with local hospitals, GPs and district nurses, so that together we can provide the right support and advice, in the place people want to be. We are a charity and provide all of our care free of charge. Services we provide include: Inpatient Unit Outpatient (Day Therapy Services) Hospice at Home 24/7 Community and Crisis Support Education Family Support Physiotherapy Occupational Therapy Pastoral Care Complementary Therapy. 2

Services Inpatient Unit (IPU) At our hospice in Havering-atte-Bower, we have an 18-bed Inpatient Unit that is staffed 24/7. Some people come to the Inpatient Unit for help with their symptoms which we put right before discharging them back home. For some, the hospice is the best place for their end of life care, too. Here, patients and their families will be provided with the best care and support possible. Our Inpatient Unit is staffed by nurses and health care assistants as well as doctors and consultants, who are supported by a range of specialists who are able to provide emotional and physical support as needed. All of our rooms are bright and airy and look out onto our beautiful gardens. Those staying on the Inpatient Unit will have access (as required) to: Management control for pain relief Management of medication Physiotherapy, complementary therapy and occupational therapy Family support teams, including social workers and bereavement counsellors Private space, including overnight accommodation for families, carers and friends Further facilities for visitors, including lounge and games room for young people. Forward planning for discharge. 3

Outpatients (Day Therapy Services) Day Therapy is an informal and welcoming service for people at any stage of their life-limiting illness. Our aim is to help those who wish to live independently in their own homes by enabling them to receive medical treatment and complementary therapy here at the hospice. Each person, and their carer, who attend the Day Therapy Unit will be individually assessed so that we have a real understanding of their needs. There is access to: Specialist palliative care, provided by our professional medical support team Health, social care, and family support teams Therapies such as complementary therapy, occupational therapy and physiotherapy Pastoral care Clinical treatment interventions, such as blood transfusions A range of groups, including a six-weekly breathtaking course run by a physiotherapist, monthly carer drop-ins and creative writing sessions A creative art therapy group Social drop-in sessions. Attending our Day Therapy care services is also a great opportunity to find mutual support and companionship from others coping with life-limiting illness. All referrals must come from a health care professional, for example a GP, district nurse, or hospital consultant. Hospice at Home Our Hospice at Home service is delivered by senior staff nurses and health care assistants, to those who have expressed a wish to die at home. We know that the responsibility and emotional demands of looking after someone with a life-limiting illness at home can be overwhelming, so that s why we are here. Our Hospice at Home service will: Assess each person s individual needs and conduct ongoing daily reviews Ensure those under their care are given the same standard of palliative care that they would receive in our hospice Provide care at home in 2-4 hour sessions which can run consecutively to support people in crisis who need 24 hour care. Working alongside other healthcare professionals, we can arrange additional services to ensure that medical, emotional and spiritual needs are met in the setting of each individual s choice, such as physiotherapy, complementary therapy, or family support. Physiotherapy Our physiotherapists help people in our care to achieve optimum comfort and independence in their every day life. Working with families and carers, our physiotherapists offer advice, guidance and a professional perspective. Services include: Mobility problems Joint stiffness Breathing difficulties Pain Swollen limbs Relaxation 5

Family Support Service Family Support Services are a multi-professional team of social workers, counsellors and bereavement counsellors supporting those under our care and helping their loved ones with the impact of illness. Support services are also available for relatives, friends, and carers, including children and young people. Advice, guidance, practical and emotional support is provided in various ways: Occupational Therapy Following an initial consultation our occupational therapists will address immediate needs to help people in our care get the most out of everyday life, live comfortably at home and enjoy the things they used to do. Physical help is often the first requirement and, with adaptations or specialist equipment, we will help patients gain independence and improvements in their quality of life. Most of the work done by our OTs is carried out in people s homes. We also support people in our Inpatient Unit, who are returning home and need mobility aids and adaptations. 6 Liaising with agencies eg. employers, schools, local authorities Providing information on grants and benefits Counselling and psychological support Bereavement counselling (both before and after bereavement) Supporting children and young adults Support groups including walking and writing groups Regular carer drop-in sessions Referring to external agencies Providing advocacy. Art Therapy Creative art therapies provide a chance to discover new skills, aid relaxation, allow time for reflection, and the opportunity to create treasured keepsakes. Card making and silk painting are just some of the activities that we offer to encourage a sense of accomplishment and satisfaction.

Specialist Community and Crisis Support (SCCS) Our team of Clinical Nurse Specialists work in our community to provide the best palliative care in the comfort of individual s own home or care home. They are experts in symptom control. They also operate our Specialist Advice Line, a 24/7 telephone support service for those who need immediate attention or support with a crisis- for example, a flare up of symptoms or pain. Working closely with other healthcare professionals, our team provides: Comprehensive telephone assessments Consultancy visits at home to meet people and assess individual needs Advice regarding complex symptom management and medication Home visits where appropriate Support and advice to other health care professionals Outpatient nurse led clinics at the hospice and planned outreach sessions in the community Education in palliative care to health professionals. All referrals to the SCCS team must come from a health care professional, GP, district nurse, or hospital to 01708 758606. If you need to speak to the SCCS team, they are available on our Specialist Advice Line 24/7, 365 days a year on 01708 758643. 7

Complementary Therapy We provide a range of complementary therapies on-site in the hospice. Alongside current medication or treatments these therapies can help put people in a more peaceful frame of mind when dealing with day-to-day life, as well as tackling aches and pains associated with life-limiting conditions. We are able to provide aromatherapy, massage, reflexology and Reiki courses provided in four initial treatments. For carers, we also offer a relaxing Indian head massage as part of our commitment to providing a safe, non-clinical area for people to escape the demands of illness. Pastoral Care Our Pastoral Care team is on-hand to meet any religious and spiritual requirements as well as attend to a full complement of pastoral duties. We are committed to a person-led approach, listening to the needs of those under our care and doing our best to guide people towards finding your own answers and meaning. Our Sanctuary is open daily for people to pray and reflect on their situation in a tranquil and serene setting. We hold weekly services as well as remembrance services four times a year. Ministers or spiritual leaders are also more than welcome to visit people at any time on our Inpatient Unit. 8

How to be Referred Referrals to Saint Francis Hospice need to come via a GP, a health professional e.g. consultant or district nurse. Every referral is assessed at our regular referrals meetings, where our specialist team assesses which service(s) has been requested and is appropriate. OrangeLine OrangeLine is our new telephone support service available to any local person who is experiencing isolation or loneliness. Call OrangeLine to connect to a friendly voice at the end of the phone. Volunteering We are reliant on the generosity of volunteers, who donate their time across the entire organisation, saving us over 2m a year in staff costs. At the moment we have 800 volunteers who help us with events, in our shops or at the hospice and we tailor the roles around their availability and skills. Donations made to our shops allow us to keep raising funds. Not only is buying second-hand clothes great for the environment and for your pocket, but you ll be amazed at the range of other items we stock. Browse electrical goods, toys, wedding dresses, clothing and furniture. We also offer a house clearing and decluttering service. Anything of great value is sold on Ebay.
