Purpose To assist in leading the clinical work of Planning and Funding as they relate to primary care. To provide advice on primary care developments and implementation of primary care health strategy (PHCS) for the district. In partnership with the DHB and Primary Care provide clinical leadership in the strategic development of primary care. Work at the interface of primary and secondary care to improve clinical integration. Provide clinical leadership in the delivery of Primary care to the sector. KEY TASKS Provision of Clinical Leadership to DHB and Primary care. EXPECTED OUTCOMES Recognized as the first point of call within Waitemata DHB for clinical leadership advice on Primary Care. Attendance at core clinical governance groups both within primary and secondary sector and within and across the region. Ensuring regular communication between the DHB provider arm and General practice teams. Work across Primary and Secondary care to assist in the development of clinical pathways. Jointly lead initiatives that result in improved clinical pathways across the system. Develop clinical innovation within Primary care. Through collaboration, develop innovative clinical solutions within the community that result in better health outcomes, reduction in inequalities. Improved population health gain. Provide clinical leadership in the development of Primary care Strategy. Work in partnership with management both with the DHB and primary care to develop future direction for Primary care. Funding and Planning Strategic and Annual Planning Processes. Ensure current and future primary care resource needs are identified, prioritised and planned. Clinical accountability for Health Targets as related to Primary care. Input into PHO Performance Programme. Target negotiation Performance monitoring and appropriate remedial action recommended where required. Clinical specification of funding contracts and clinical performance against contract. Funding contracts have appropriate clinical input as required. Input at a strategic level into GP training and workforce development. Input into workforce development plans. Stakeholder Management Builds and develops harmonious and positive relationships with key primary care stakeholders to enhance the reputation and image of Waitemata District Health Board.
