Importance of LEAPP University of Pittsburgh Learner All learners are encouraged to participate in programs both in real time, distance based, or on-site Education The focus of education includes a range of interventions aimed at improving knowledge, skills, and changing practices through lectures, preceptorships, consultations and tailored to individual needs. Practice The focus is on building the knowledge, skill and capacity of health professionals in HIV and related disorders thereby influencing the way they practice. Portal A portal implies a single place that to go to find information and are networked and supported in a community of practice at the health discipline or clinic level, between health professionals, and among clinics, institutions, and programs

Best Practices Standardized internal processes ensure frequent Coach- Coordinator communication and consistent assessment methods. Patient populations are different, so approaches with clinical sites need to be individualized. Clinic environment ( culture ) is different, so systems are going to be different. Having supports and practices in place to provide feedback to care providers on what s working and what s not is an essential ongoing process (CQI). Working with clinic leadership and influentials is critical. Requires a continuous feedback loop between the AETC and the targeted clinic.
