INSTRUCTIONS This booklet is divided into the following three sections: Section I These scholarships require the Portsmouth High School Local Scholarship Application which is available on the PHS website or in the Guidance Department. Complete one application leaving the name of the scholarship line blank. Make copies for each scholarship for which you are applying and then fill in the name of the scholarship. (Ask teachers ASAP if you need a recommendation.) Submit completed packets with all necessary attachments to the Guidance Department secretary on or before the deadline. (One staple only or paperclip.) (Do not put in envelopes or folders.) The guidance secretary will attach a transcript to all applications and forward them to the proper committees. Section II These scholarships require you to write a letter to the scholarship committee. Some may require additional information and or attachments. Staple these to your letter. Put your name and the name of the scholarship on each page. You are responsible for submitting or mailing the letters/attachments to the contacts provided. (Do not enclose in envelopes, if returning to Guidance.) Section III These scholarships require their own applications. (You may obtain an application either on our website or from the guidance secretary.) You are responsible for submitting/mailing the completed applications with all the necessary attachments. If a transcript is required, requests should be submitted at least five (5) school days prior to the due date. Pay close attention to all deadlines as failure to meet the deadlines could result in being ineligible for the scholarship. Actual amounts may vary. Occasionally, a listed scholarship may not be awarded. The applications for Section I are available on our website (http://cityofportsmouth.com/school/phs/departments/guidance.htm) or in the Guidance Office. STEARNS MEMORIAL Scholarship (Renewable for past recipients) Given in memory of Ruth A.Stearns to a PHS graduate continuing on to an accredited program in the health care field. Applicants must also provide an essay describing their plans, the school they plan to attend, relevant information about financial need, and factors that may help the committee with their decision. NOTE: Financial need is not necessarily the determining factor. ATTACHMENT: Essay ASSOCIATION OF PORTSMOUTH TEACHERS Up to three scholarships awarded to high school graduates who are the sons or daughters of A.P.T. members and/or scholarship contributors. ATTACHMENTS: Letter describing self and goals - Include name of member/contributor ASSOCIATION OF PORTSMOUTH TEACHERS MEMORIAL Scholarship One or more granted to the son or daughter of an Association of Portsmouth Teachers member. ATTACHMENTS: Letter describing self and goals - Include name of member GEORGE T. COGAN MEMORIAL Scholarship (Renewable for past recipients) Multiple scholarships awarded to male graduates of PHS or St. Thomas Aquinas High School. Awards based on character, scholarship, leadership, and need. Applicants must have been residents of Portsmouth, NH for 4 years prior to high school graduation. Applicants must file the FAFSA and submit with the local scholarship application a copy of the page of the SAR that indicates the EFC. Past recipients re-applying will also need two recommendations, transcript from current college and SAR. ATTACHMENTS: SAR, 2 recommendations (teacher, clergy, counselor, employer, etc.), Confirmation of 4-year residency in Portsmouth, NH $200 HENRY AND PHYLLIS COITO Memorial Scholarship Awarded to a graduating PHS senior accepted to a four-year program. Applicant must submit an essay (750 words max) answering the question, If we gave you one do-over for your high school years, what would it be? as well as a graded essay from an English class. Please submit 3 copies of each. NOTE: Payable the beginning of college sophomore year. ATTACHMENTS: Essay and graded English essay CREEK ATHLETIC CLUB Scholarship Scholarship is awarded to a graduating senior continuing in a 2 year, 4 year, or vocational/ technical program who lives in the New Franklin Elementary School District and either attended New Franklin Elementary School or St. Patrick s School. The recipient must have lettered in a varsity sport. Preference will be given to students who reside in the City of Portsmouth s Creek neighborhood. ATTACHMENTS: One recommendation from a Varsity Coach in the sport in which the applicant lettered Page 4

Portsmouth High School Scholarship Booklet - Section I Apply using PHS Local Scholarship Application - Submit Applications to PHS Guidance Office DEMERITT Scholarship (Renewable for past recipients) Multiple scholarships given to current seniors who must be residents of Portsmouth, NH. Applicants must file the FAFSA and submit with the application a copy of the page of the SAR that indicates the EFC. Past graduates must also submit their college transcript, a letter of recommendation from their professor or advisor, and SAR. ATTACHMENTS: Recommendation and SAR DONDERO SCHOOL PTA Scholarship Given to a senior who attended Dondero School. Specific eligibility: 1. Has demonstrated service to his/her community. (Church, scouts, coaching, training, special programs, post-elementary service to Dondero, etc.) 2. Is in good academic standing. 3. Is in financial need. ATTACHMENT: Statement JOHN W. DURGIN III MEMORIAL Scholarship Granted to a promising graduating senior of Portsmouth High School who has been accepted at an accredited engineering college for the study of engineering (with preference for civil engineering). The grant will not actually be made until the student matriculates. Requires a 300 word essay concerning your goals as an engineer, why engineering is a career choice for you, and two academic or community references. ATTACHMENTS: Essay plus two references $1000 or two EVER READY INVESTMENT CLUB ASSOCIATION Scholarship This scholarship is granted to a graduating female of Portsmouth High School. We are looking for a candidate who is going to college, needs some financial assistance and has expressed, through an essay, her interest in pursuing a business career as well as her plans to take business related subjects. She must be accepted to enter school in the fall of 2010. Payment will be made directly to institution or institution bookstore. ATTACHMENT: Essay $750 SALVATORE AND JOANNE GRASSO Scholarship Fund This scholarship shall be awarded to a graduating senior who has been accepted to an institute of higher learning with the intention of entering the field of Education. The student must be in the top 35% of the graduating class and must submit an essay of no less than 300 words on the topic Why I am Selecting Education as A Career. Also, include references from three teachers (or a guidance counselor and two teachers) attesting to the ability of the student to succeed in the field of education. Financial need will be the last criteria to be considered. ATTACHMENT: Essay and Three (3) References GRAY Scholarship (Renewable for past recipients) Multiple scholarships given in varying amounts to current seniors, who are residents of Portsmouth, NH, to aid with expenses for higher education. ATTACHMENTS: Two teacher recommendations Page 5

Portsmouth High School Scholarship Booklet - Section I Apply using PHS Local Scholarship Application - Submit Applications to PHS Guidance Office $1000 or two $1000 or minimum of one or two $1000 GREENLAND PARENT ORGANIZATION Scholarship Awarded to a graduate of Greenland Central School and who had attended grades 6-8. He/ she must be a graduating senior and planning to attend a 2 or 4 year school of higher learning. Specifics that will be looked at are: service to the community, academic achievements, extracurricular activities Financial information not a factor in consideration. NOTE: Renewable for up to three remaining years of college with a 3.0 or better annual grade point average. Renewals must be claimed no longer than 5 years from start of college. Student must submit official transcript within 3 months of end of academic year to qualify for renewal. ATTACHMENT: Written statement explaining eligibility and desire for scholarship. GREENLAND WOMEN S CLUB Scholarship This scholarship will be awarded to a resident of Greenland, NH who will be attending full time, an accredited two or four year college, university or vocational school. the scholarship will be awarded on the basis of scholastic ability and character. Requires: 1. Personally written statement by candidate to include: a. Information about the school you hope to attend and why b. A description of your career plans and ambitions. 2. Resume All information concerning candidates will be held confidential by the committee. ATTACHMENT: Statement and resume. Please DO NOT include financial information. HARBOUR WOMEN S HEALTH - CHARLENE M. EASTER MEMORIAL Scholarship Given to a graduating female senior entering the health field or women s studies, going on to a 2 or 4 year school. Include a letter about yourself and why you deserve this scholarship. ATTACHMENT: Letter JOHN F. and OLGA LEARY Scholarship (Renewable for past recipients) Multiple scholarships given to graduating seniors from Portsmouth High School who must be residents of Portsmouth, NH. Awards are based on applicant s involvement in community service to the Citizens of Portsmouth. Requires resume documenting community service, an essay of not more than 500 words on the topic How this scholarship will influence me in providing service to my community. Also, requires one letter of reference from someone who can attest to your community service involvement. ATTACHMENT: Resume, Essay, and Recommendation $350 ALICE M. LEE Scholarship Granted to a graduating senior of Portsmouth High School who has been accepted for enrollment at an accredited college or university for the study of teaching and education. ATTACHMENT: Brief statement about career goals in teaching/education LITTLE HARBOUR SCHOOL PTA Scholarship Awarded to a graduating PHS senior who graduated from the 5th grade at Little Harbour School. In order to be eligible for this scholarship student must: 1. have demonstrated service to the school or community, 2. be in good academic standing, 3. be attending an accredited two or four year college or vocational program. ATTACHMENT: List years and activities at Little Harbour Page 6

Portsmouth High School Scholarship Booklet - Section I Apply using PHS Local Scholarship Application - Submit Applications to PHS Guidance Office CARRIE LEE MACKAIL Scholarship Given in memory of Carrie Lee Mackail to a current graduating female Portsmouth High School senior who has been accepted to an accredited institution of higher learning. The applicant should be in good academic standing. This scholarship is based on the applicant s commitment to academic success, financial need, leadership, community service, character and determination. Please attach to the application a letter of recommendation from someone who knows you well and a letter stating why you believe you deserve this scholarship. ATTACHMENT: Recommendation and letter MCINTOSH Scholarship (Renewable for past graduates) Multiple scholarships awarded to graduates of PHS. Amounts vary and can be used for tuition and expenses at a school or college by a student pursuing a career in nursing. After all nursing awards have been made, health related majors may be considered. Applicants must file the FAFSA and submit with the local scholarship application a copy of the page of the SAR that indicates the EFC. NOTE: Past graduates must submit renewal application and requirements. ATTACHMENT: SAR and confirmation of nursing or health related major. $200 GORDON MULLAVEY Scholarship Given to a deserving Portsmouth High School graduate continuing in a 2-year, 4-year, or vocational program. Significant consideration will be given to students who have demonstrated strong motivation and diligence to their high school studies. Requires a letter of recommendation from a teacher. ATTACHMENT: Teacher recommendation PHIL NELSON ART Scholarship Given in memory of Phillip C. Nelson to a graduating Portsmouth High School senior who has been accepted at an accredited art school. This scholarship is based upon the applicant s talent, financial need, character, and determination. Please attach to the application a letter of recommendation by an individual who knows you well, and a letter stating your goals and why you believe you should receive this scholarship. ATTACHMENT: Letter and Recommendation NEW FRANKLIN SCHOOL PTO Scholarship Given to a senior from Portsmouth High School who attended New Franklin School graduating from the 5th grade and has contributed his/her assistance to the school since graduating. Must be attending an accredited two- or four-year college or vocational program. NOTE: The scholarship is payable after successful completion of the first semester. ATTACHMENT: Note years and activities at NFS PORTSMOUTH ELKS LODGE Scholarship One or more awards will be granted to worthy graduating seniors. Financial need, academic performance, and sincerity are included in the criteria. Preference may be given to a daughter, son, or grandchild of an Elk member. ATTACHMENT: Essay on the value of community involvement. Page 7

Portsmouth High School Scholarship Booklet - Section I Apply using PHS Local Scholarship Application - Submit Applications to PHS Guidance Office $150 PORTSMOUTH FEMALE ASYLUM Scholarship Three to four scholarships will be given to Portsmouth High School graduating females who wish to continue in any helping profession such as nursing (RN, LPN or CNA), teaching, child care, gerontology, dental assistant, medical careers, interpreters for the deaf, speech and physical therapy, social work, and will consider other professions in any 2-year, 4-year, or technical program. Awards will be based on career choice and financial need. Renewable for term of study. ATTACHMENT: Short essay explaining goals for your future education and two letters of recommendation. PORTSMOUTH PARAPROFESSIONAL ASSOCIATION Scholarship Scholarship given to any graduating senior, based on academic achievement and financial need. Note: Payment will be made upon successful completion of the first semester of the post secondary program. Preference is given to a son or daughter of P.P.A. member. A written statement is required from each applicant stating why you are applying for this scholarship and what your educational goals are. ATTACHMENT: Statement, Name of Paraprofessional $1500 RYE EDUCATION FOUNDATION - MARGUERITE KELLY Scholarship Awarded in memory of Marguerite Kelly, longtime educator and school board member with the Rye School System, to a Portsmouth High School senior who is a resident of Rye, NH. Preference will be given to students who have attended both Rye Elementary and Rye Junior High School. Applicant should write an essay (with sincerity and reflection) on the topic of the significant influence a particular teacher has had on the applicant s school life and how that influence may apply to the applicant s future. The applicant must be accepted to an accredited post-secondary school. ATTACHMENT: Essay SANDERSON-ROLSTON-WILSON Scholarship Given to current or former residents of Greenland, NH entering an accredited institution of higher learning. Preference given to engineering, science, or mathematics. Other courses of study are acceptable, if no qualified applicants are available. Academic promise and need are considerations. Please write a letter about yourself and future plans. Also, two letters of recommendation are required. Indicate years of residence in Greenland in letter or resume. NOTE: Payment to student upon entering second year of college. ATTACHMENT: Essay, two letters of recommendation BRENT SQUIRES MEMORIAL Scholarship Given to a student who attended Dondero School for at least three years. The required essay will be weighted heavily in the selection process. The applicant should write with care and thought on the topic of The Influence of Family Life On My Values. ATTACHMENT: Essay GEORGE BRADFORD WARD Scholarship Granted to a graduating senior desiring to further his or her education beyond the high school level; selection to be made on the basis of merit and need and without regard to sex, race, or religion. This scholarship is administered by St. John s Lodge of Masons. ATTACHMENT: Two written references Page 8

BRUCE WELLDE MEMORIAL Scholarship This award is granted to a Portsmouth High School graduating senior who intends to enter some branch of the medical profession, such as but not limited to medical research, teaching, or medical practice. Applicant must submit two letters of reference and a brief personal essay on their interest in the medical field. ATTACHMENT: Essay and two letters of referrence S. MICHAEL WENTWORTH Scholarship Given to a PHS graduating senior, in good academic standing, accepted at an accredited institution of higher learning. The applicant must have demonstrated leadership, good character, determination, and financial need. The applicant must intend to pursue a career in education. Please submit an essay stating why you believe you deserve this scholarship and a letter of recommendation. ATTACHMENTS: Essay, letter of recommendation JOHN J. WHOLEY Scholarship Fund Awarded to a member of the current graduating class of Portsmouth High School who is a resident of the City of Portsmouth, NH and who intends to further his or her education in the field of political science, government, or pre-law. HERBERT A. WILSON Memorial Scholarship Granted to a worthy and deserving graduating senior of an area high school who is a resident of Greenland, NH and who has been accepted at an accredited college or university. HAVEN SCHOOL ATHLETIC Scholarship Given in memory of the men and women who raised money to support the Haven Elementary School Athletic Program.
