The property is legally described as follows: A ±3,225 square feet building known as the Vearl Scott Neighborhood Family Center located at 1201 Seventh Avenue South, St. Petersburg, Florida within City owned CAMPBELL S PARK ADDITION, Block 1, Lot 1, according to the plat or map thereof as recorded in Plat Book 947, Page 87, Public Records of Pinellas County, Florida. Pinellas County Tax Identification Number: 24/31/16/13069/001/0010 The intent of entering into a Use and Operating Agreement for the building is to make available certain program(s)/service(s) to low to moderate income persons within the corporate limits of the city of St. Petersburg. All proposals must include, at a minimum: 1) the proposed monthly or annual payment for use of the building, net to the City of St. Petersburg; 2) any extraordinary terms or conditions; 3) details of the proposed program(s)/service(s) and number of persons to which services are to be provided; 4) copy of Articles of Incorporation and By Laws; 5) copy of Certificate of Status from the State of Florida for 2010; 6) copy of IRS letter approving status as 501(c)(3) dated prior to December 31, 2009; 6) copy of Board of Directors roster with no related family members and certifying signature of Board President; 7) a disclosure of all persons or entities having a beneficial interest in the proposal; 8) copy of most current audit showing stable financial condition; and 9) particulars of the Bidder s qualifications and financial stability necessary to successfully operate the facility for the term of the Agreement. I. Project Objective: A. Provide services to low to moderate income persons for the term of the Agreement. B. Provide a written monthly report, attached hereto as Exhibit C, which summarizes information on all new or recertified persons during the reporting month and through the term of the Agreement. The successful bidder will be responsible for any and all costs and expenses related to building renovations, operation, utilities and maintenance of the facility. The Agreement will be for a term not to exceed thirty six (36) months and the Lessee shall not have the right to encumber the property in any manner whatsoever. II. Submittal Requirements Bidders shall submit five (5) copies of the proposal to include, but not be limited to the following: A. The proposed monthly or annual payment, net to the City of St. Petersburg for the use of the building; B. Any extraordinary terms or conditions related to the Agreement; C. Details of the proposed services as they relate to the required components identified in Section I of these instructions; i.) Addressing the project objective and program components. ii.) Development of needs assessment. iii.) Expertise of bidder related to implementation of the program components.
D. Description of the Bidder s entity (corporation, partnership, etc.) and identification of all parties including disclosure of all persons or entities having a beneficial interest in the proposal; E. Particulars of the Bidder s qualifications and financial stability necessary to successfully operate the facility for the term of the Agreement; and F. Letter from the City s Housing and Community Development Department stating that the proposed project objective is compatible with CDBG Program guidelines and supportive to the overall program goals of the City of St. Petersburg.
